Abstract

Mental disorders are conditions characterized by disturbances in cognitive function, emotional regulation, and behavior, which affect an individual’s ability to perform daily activities (American Psychiatric Association, 2022). The World Health Organization (WHO) defines mental disorders as clinically significant disturbances accompanied by psychological distress and impaired functioning in important areas of life (WHO, 2025). Globally, approximately 970 million people experienced mental disorders in 2023, increasing to more than 1.1 billion in 2024. In Indonesia, the prevalence of mental disorders in West Sumatra Province is 1.3% (Indonesia Health Survey, 2023). One of the common problems found in severe mental disorders such as schizophrenia is the risk of violent behavior, which is a maladaptive response due to the inability to control aggressive impulses that may harm oneself, others, and the environment. Data from RSJ Prof. Dr. HB. Saanin Padang indicate that the risk of violent behavior is the second most common nursing problem after hallucinations. This study aims to describe psychiatric nursing care for patients with a risk of violent behavior in Wisma Nuri at RSJ Prof. Dr. HB. Saanin Padang in 2026. The method used is a case study with the implementation of the Nursing Intervention Strategy (SP), including physical exercises, medication adherence, verbal emotional control, and spiritual approaches over five days. The results showed an improvement in the patient’s ability to control emotions and behavior. It can be concluded that comprehensive psychiatric nursing care is effective in reducing the risk of violent behavior.

Abstract

Hallucinations are disturbances in sensory perception in which individuals experience false sensations such as hearing voices, seeing, tasting, touching, or smelling things that are not actually present. These disturbances can affect cognition, emotional control, and behavior, thereby influencing an individual’s functional abilities (American Psychiatric Association, 2022). The World Health Organization reported that approximately 970 million people worldwide experienced mental disorders in 2023, increasing to more than 1.1 billion people in 2024. In Indonesia, mental health remains a significant concern, with a prevalence of 1.3% in West Sumatra Province (Indonesia Health Survey, 2023). The Prof. HB. Saanin Mental Hospital in Padang is recorded as the hospital with the highest number of psychiatric patient visits in the region, reaching 38,332 visits in 2024 and remaining high in 2025. This study aims to describe psychiatric nursing care for patients with auditory hallucinations in Wisma Melati at Prof. HB. Saanin Mental Hospital, Padang, in 2026. Nursing care was provided through theimplementation of the Implementation Strategy (SP), which includes practicing rebuking the hallucinations, medication adherence, engaging in conversations, and performing scheduled activities. The five-day implementation showed an improvement in the patient’s ability to control auditory hallucinations. The results of this study emphasize the importance of the nurse’s role in providing comprehensive psychiatric nursing care to help patients control violent behavior and prevent risks to themselves and their environment.

Abstract

Hallucinations are sensory perception disturbances that occur without any real stimulus. One common type of hallucination is auditory hallucinations, which involve the perception of hearing voices or sounds that do not originate from external stimuli. Based on medical records at Prof. HB. Saanin Mental Hospital, Padang, the most common nursing problem in the past three years was hallucinations, with 12,419 patients. The purpose of this case study is to apply psychiatric nursing care to a client with auditory hallucinations at Prof. HB Saanin Mental Hospital, Padang. The method used in this scientific paper is a case study through a nursing process approach, which includes assessment, diagnosis, intervention, implementation, and evaluation. The assessment results indicate that the client experienced signs and symptoms of hallucinations, including hearing whispers. The primary diagnosis was hallucinations, with a co-diagnosis of self-care deficit. After providing this nursing care, the patient was able to independently control the hallucinations using implementation strategies (SP): SP 1: reprimanding, SP 2: taking medication regularly, SP 3: having conversations, and SP 4: carrying out scheduled daily activities. These results indicate that the implementation of implementation strategies is effective in helping the patient control hallucinations. It is hoped that through this research, everyone will understand the importance of psychiatric nursing care, thereby increasing awareness in maintaining and preserving mental health, and implementing behaviors that support optimal mental well-being in everyday life

Abstract

The risk of violent behavior is one of the psychiatric nursing problems in patients with severe mental disorders caused by the inability to control emotions, which may potentially harm oneself, others, or the surrounding environment. Data from the World Health Organization (WHO) in 2024 showed that mental disorders have increased, affecting more than 1.1 billion people worldwide. Based on medical record data at Prof. HB. Saanin Mental Hospital Padang, cases of violent behavior were reported as many as 213 cases in 2023, 138 cases in 2024, and 159 cases in 2025.This study aimed to provide mental health nursing care for patients with a risk of violent behavior. The method used in this scientific paper was a case study approach with the nursing process, which includes assessment, diagnosis, planning, implementation, and evaluation. The implementation of nursing care was carried out from February 9 to 14, 2026, with five meetings. Nursing interventions were provided through deep breathing exercises, hitting a pillow as a safe outlet for anger, medication adherence education, effective verbal communication, and spiritual activities. The results showed that the patient became calmer, more cooperative, able to control anger, did not use harsh language, and adhered to medication. In conclusion, mental health nursing care can reduce the signs and symptoms of the risk of violent behavior. It is expected that nurses and families play an active role in preventing relapse.

Abstract

According to WHO data in the journal (Andari, 2023), the number of gastritis cases in the world every year is estimated to reach around 1,800,000 to 2,100,000 people. Meanwhile, in the Southeast Asian region, the incidence of gastritis is recorded as many as 583,635 cases per year from the total existing population. The purpose of this study is to provide nursing care for the elderly with gastritis at the Tresna Werdha Sabai Nan Aluih Sicincin Social Home. Gastritis is an inflammation of the gastric mucosa that can cause complaints such as heartburn, nausea, vomiting, and bloating, and can cause complications if not treated properly. The research method used was a case study by taking one patient as the research subject. The nursing care process is carried out through the stages of review, enforcement of nursing diagnosis, planning, implementation, and evaluation. The assessment will be carried out from February 11, 2026 to February 16, 2026. The results of the nursing diagnosis that emerged, namely chronic pain, fatigue, and health management were ineffective. Interventions include pain management, energy management, as well as health education and monitoring of patient conditions. After undergoing the application for 5 days, results were obtained that showed a reduction in pain, energy after rest, and an improvement in the patient's ability to manage their health. It is hoped that patients can maintain a healthy lifestyle, regulate their diet regularly, avoid foods that trigger gastritis, and regularly carry out health checkups so that the condition does not recur.

Abstract

Hallucinations are a client's false perception through the five senses of an environment without any real stimulus or stimulation. According to WHO data from 2023, the prevalence of mental disorders worldwide was 970 million individuals worldwide. This data increased in 2024 to exceed 1.1 billion people. This increase in prevalence confirms that the global burden on mental health is increasingly severe. Based on initial data obtained from Prof. HB. Saanin Mental Hospital, Padang, the most common problem in 2025 was hallucinations, with 6,062 patients. This study aims to implement Mental Health Nursing Care for Mr. K with Hallucinations at Wisma Flamboyan, Prof. HB. Saanin Mental Hospital, Padang. The implementation of Nursing Care for Mr. K was conducted on February 4 to February 10, 2025 with a visit of six days of meetings. The method of this Scientific Paper is a Case Study. The results of the provision of Mental Nursing Care were obtained that Mr. K was able to control Hallucinations independently by using the implementation strategy (SP), namely SP 14. In this study, it is expected that Mr. K understands the provision of Mental Nursing Care and the client is able to control hallucinations independently and signs of hallucination symptoms decrease.

Risk of Violent Behavior is a condition where a person commits an act that can be physically dangerous, both to themselves and others, accompanied by uncontrolled rage, noise, and anxiety. Based on medical record data from Prof. HB. Sa'anin Mental Hospital Padang, the nursing diagnosis of Risk of Violent Behavior is still high, namely in 2023 there were 213 cases, in 2024 there were 138 cases, and in 2025 there were 159 cases. The purpose of this case study is to obtain an overview of the implementation of psychiatric nursing care for patients with Risk of Violent Behavior. The method used is a descriptive case study with a nursing process approach including assessment, diagnosis, intervention, implementation, and evaluation. Nursing actions are given through deep breathing exercises, hitting pillows, education on taking medication regularly, good verbal communication, and spiritual activities. The results show that patients are calmer, more cooperative, able to control anger, do not speak harshly, and are compliant in taking medication. It can be concluded that psychiatric nursing care is effective in reducing signs and symptoms of the risk of violent behavior. It is hoped that nurses and families will play an active role in preventing recurrence.
