Abstract

Typhoid fever is an intestinal disease that occurs suddenly due to infection with the Salmonella typhi bacterium. According to the WHO, approximately 81% of people contract typhoid fever. Based on data obtained from Aisyiyah Pariaman Hospital, typhoid fever ranked 4th out of the 10 most common diseases at Aisyiyah Pariaman Hospital in 2024, with 262 cases of typhoid fever in the Muzdalifah ward. The objective of this study is to provide nursing care for patients with typhoid fever. The case study method was used, based on data obtained through observation, interviews, physical examinations, and documentation. The study was conducted in the Muzdalifah Ward of Aisyiyah Hospital in Pariaman from September 19 to 23, 2025, involving one patient, Mr. J. The study identified the following nursing issues: hyperthermia, nausea, sleep disturbances, risk of nutritional deficit. The evaluation results and implemented interventions showed that nausea and the risk of nutritional deficiency were resolved by the 4th day, while hyperthermia and sleep disturbances were resolved by the 5th day. In this case study, it is hoped that the patient and family will increase their knowledge of personal hygiene and maintain their health to avoid diseases, particularly typhoid fever.

Abstract

Based on data from the World Health Organization, cardiovascular disease is the leading cause of death, worldwide, as many as 19.8 million people died from cardiovascular disease and 38% of these deaths were caused by Coronary Heart Disease (CHD). Based on data from the Indonesian Health Survey, Coronary Heart Disease caused death, as much as 0.85% or as many as 765,000 cases. From the survey results, West Sumatra is ranked 10th in the provinces with Coronary Heart Disease with a prevalence of 0.87%. Based on data obtained at Padang Pariaman Regional Hospital, Coronary Heart Disease cases were ranked 15th in 2025 with 56 cases. This case study aims to obtain an overview of the application of nursing care to patients with Coronary Heart Disease, including use, establishing nursing diagnoses, planning nursing, carrying out, implementing, evaluating, acting and documenting. The methods, data collection used in this case study are observation interviews, physical examinations and medical records. This case study was conducted in the HCU room of Padang Pariaman Regional Hospital from February 25 to March 1, 2026. The results of the study obtained nursing problems in the form of ineffective breathing patterns, decreased cardiac output, fatigue, acute pain and the risk of ineffective peripheral perfusion. Nursing problems were resolved on the 5th day with nursing actions provided in the form of airway management, cardiac care, pain management, susigi management and shock prevention. It can be concluded that the treatment of Coronary Heart Disease patients requires medical action and patient awareness in managing lifestyles to improve the quality of nursing care in hospitals. It is hoped that patients and their families can understand the importance of awareness in maintaining a healthy lifestyle to prevent complications due to Coronary Heart Disease.

Abstract

Acute Decompensated Heart Failure (ADHF) is a cardiovascular emergency characterized by an acute deterioration in heart function that requires immediate treatment. ADHF is a global health issue, with approximately 3 million cases reported annually. In Indonesia, the prevalence of heart disease is 0.85%, while in West Sumatra Province it is 0.87%. Medical record data at Padang Pariaman Regional General Hospital show an increase in ADHF cases, with 100 cases in the past year in the HCU ward. This case study aims to provide an overview of the implementation of nursing care for patients with ADHF. The method used was a case study employing a nursing process approach through observation and interviews, conducted over 5 days from February 25 to March 1, 2026, in the HCU Ward of Padang Pariaman Regional General Hospital on one patient diagnosed with ADHF. The results of the case study revealed five primary nursing problems: decreased cardiac output, ineffective airway clearance, acute pain, activity intolerance, and sleep pattern disturbance. Interventions were implemented over 5 days, focusing on cardiac care, airway management, pain management, energy management, and sleep support. Evaluation showed a gradual improvement in the patient’s condition, with sleep disturbances resolved by day 2, acute pain resolved by day 3 with a pain score of 2, airway clearance resolved by day 4, The decreased cardiac output resolved by day 5, with an oxygen saturation of 98%, and activity intolerance was partially resolved. It is hoped that the patient and family will follow the healthcare providers’ recommendations, maintain a healthy lifestyle, and attend regular follow-up visits at a healthcare facility to prevent recurrence.

Abstract

                Dengue Hemorrhagic Fever (DHF) is an infectious disease caused by the dengue virus and transmitted through the bites of Aedes aegypti and Aedes albopictus mosquitoes. DHF remains a major public health problem in Indonesia due to its high incidence rate. This scientific paper aimed to describe the implementation of nursing care for a patient with DHF. The method used was a case study applying the nursing process, including assessment, nursing diagnosis, planning, implementation, and evaluation. The subject of the study was one patient (Ms. N) who was hospitalized for five days in the Internal Medicine Ward of Prof. M. Yamin Hospital, Pariaman. The results revealed four nursing diagnoses: hypovolemia, hyperthermia, risk for bleeding, and risk for nutritional deficit. After comprehensive nursing interventions were provided, the patient's condition improved as evidenced by normal body temperature, increased platelet count, adequate fluid balance, and improved nutritional intake. The study concludes that systematic, comprehensive, and continuous nursing care is effective in improving patient outcomes and accelerating recovery in patients with DHF.

Abstract

Chronic Kidney Disease (CKD) is a progressive and irreversible impairment of kidney function. Globally, CKD is estimated to affect approximately 843.6 million individuals worldwide, with mortality rates continuing to increase annually, making it the 12th leading cause of death globally. In Indonesia, the main risk factors for CKD are hypertension (34.1%), diabetes mellitus (8.5%), and obesity (21.8%). Meanwhile, in the Muzdalifah Inpatient Ward of Aisyiyah Pariaman Hospital, the number of CKD cases increased from 61 cases in 2023 to 79 cases in 2025. The purpose of this study was to implement Medical-Surgical Nursing Care for a patient with CKD using a case study method and anamnesis approach. This study was conducted in the Muzdalifah Inpatient Ward from February 18 to 22, 2026.The assessment findings in Mrs. M revealed symptoms including lower back pain with a pain scale of 6, lower extremity edema, shortness of breath, nausea, generalized weakness, and decreased urine output. The nursing problems identified were impaired gas exchange, hypervolemia, ineffective peripheral tissue perfusion, acute pain, unstable blood glucose levels, and risk for decreased cardiac output. Nursing interventions provided included respiratory monitoring, hypervolemia management, circulatory care, pain management, hyperglycemia management, cardiac care, and health education. Evaluation results showed that impaired gas exchange and acute pain were resolved on the third day, hypervolemia and ineffective peripheral tissue perfusion were resolved on the fifth day, unstable blood glucose levels were partially resolved, and no signs of decreased cardiac output occurred during the hospitalization period.
