Abstract

Poor management of breast care can result in premature cessation of breastfeeding. The main reason for early cessation of breastfeeding is due to pain caused by breast milk dams. One indicator of a child's health is exclusive breastfeeding. In Indonesia, exclusive breastfeeding coverage increased from 69.62% in 2020 to 71.58% in 2021 and 72.04% in 2022. This community service aims to enhance community awareness regarding the efficacy of warm breast compresses as an alternative therapeutic approach for managing breast engorgement in postpartum mothers. This study utilized a quasi-experimental design with pre- and post-test assessments. The target population for this research consisted of postpartum mothers on the third postpartum day residing within the service area of the Garung Community Health Center, Wonosobo Regency in August-September 2024 who experienced breast milk dams with a total of 6 respondents using probability sampling techniques with simple random sampling. Researchers divided respondents into 2 groups. The Wilcoxon Test was employed for data analysis. The statistical test results using the Wilcoxon Test, postpartum mothers with warm compresses with p value= 0.000, can be concluded p < 0.05. This indicates a statistically significant effect of warm compresses in alleviating breast pain among postpartum mother.

Abstract

The 3-month injectable contraceptive is the most widely used method in the working area of Puskesmas Garung, accounting for 50% of the total active contraceptive coverage, which stands at 78.90%. However, this method has several drawbacks, such as an effectiveness rate of only 98% and a failure risk of 60 per 1,000 users according to WHO, which is higher compared to IUDs, implants, MOW, and MOP. The high usage of the 3-month injectable contraceptive may lead to suboptimal birth control, potentially increasing maternal mortality rates (MMR). This study aims to identify predisposing and reinforcing factors influencing the selection of the 3-month injectable contraceptive. Using a descriptive qualitative method with consecutive sampling involving 9 respondents, data were collected through questionnaires and analyzed univariately. The findings indicate that age, education, and knowledge (predisposing factors) are the most influential factors in choosing this contraceptive method. Therefore, healthcare providers are encouraged to offer in-depth counseling on the effectiveness and drawbacks of the 3-month injectable contraceptive to encourage mothers to switch to long-acting contraceptive methods (LACMs).

Abstract

Wasting in children under five remains a significant public health problem, especially in developing countries like Indonesia, where nutritional issues such as wasting, stunting, and underweight are still prevalent. This study aimed to develop and implement a community-based innovation program named Tangan Baliku (Integrated Management of Underweight Children) to address the high incidence of wasting in the working area of Purbaratu Primary Health Center. This innovation was designed and implemented in April 2022 through cross-sectoral collaboration, focusing on promotive, preventive, curative, and educational approaches. The methodology used was a descriptive participatory approach involving community empowerment through optimizing the existing mother–toddler class, home visits, nutrition counseling, supplementary feeding, and regular monitoring. The results show that Tangan Baliku effectively increases early detection and management of underweight children, improves mothers' knowledge of nutrition, and encourages higher participation in toddler health services. It also helps in reducing the number of children with poor nutritional status through structured follow-up and referral systems. In conclusion, the Tangan Baliku program is a comprehensive and sustainable strategy that addresses wasting by strengthening maternal involvement, community participation, and cross-sector collaboration in child nutrition improvement. This program contributes to better health outcomes for children under five and supports the development of a healthy and high-quality future generation.

Abstract

Breast milk smoothness occurs because the blockage of venous and lymphatic flow increases, causing breast swelling, inhibiting milk flow, and pain in the breast and increasing body temperature, if not treated immediately there can be breast milk dams, mastitis and even breast infections. This causes pain, to reduce breast swelling non-pharmacologically can be done with the development of transdermal patch compress red ginger extract. The research purposes is a resulted in the development of a viable red ginger extract transdermal patch compress and proved its effectiveness as an alternative therapy for smooth breastfeeding in postpartum mothers. Produce and analyze its effectiveness as an alternative therapy for smooth breast milk in postpartum mothers. Method: This type of research is R&D, and quasi-experimental Pre-Posttest With Control Group Design. The sample amounted to 30 postpartum mothers divided into 2 intervention groups with the development of transdermal patches of red ginger extract and breastcare and a control group with breasrcare. The variables measured were breast milk volume, decreased swelling and breast tenderness. Consecutive sampling technique. Data collection using questionnaires and observations. Data analysis using univariate and bivariate using Wilcoxon and Mann Whitney. The development of transdermal red ginger extract patches is validated and feasible to be applied as an alternative therapy to smooth breast milk of puerperal mothers. Test results of effectiveness of increasing breast milk volume from 2,47 to 71,07 (p = 0.000), decreasing breast swelling from 3,45 to 1,32 (p = 0.000), breast pain from 2,91 to 0,14 (p = 0.001). The application of the development of Transdermal Extract Red Ginger Patch is feasible and effective as an alternative therapy against the smooth breastfeeding of puerperal mothers.

Abstract

Program inovasi komunitas Wanoja Sehat Siap Nikah “WASSINI” dilaksanakan untuk meningkatkan kesiapan remaja putri dalam menghadapi pernikahan dan perencanaan kehamilan sehat. Kegiatan ini dilatarbelakangi oleh kurangnya edukasi dini mengenai kesehatan pranikah dan prakonsepsi pada remaja usia 17–18 tahun di wilayah kerja Puskesmas Purbaratu, Tasikmalaya. Tujuan program ini adalah untuk meningkatkan kesadaran serta peran aktif remaja, keluarga, dan kader masyarakat terkait pentingnya edukasi pranikah dan prakonsepsi. Metode kegiatan meliputi analisis situasi, koordinasi internal dengan pembimbing dan petugas kesehatan, pembuatan media edukasi, serta penyuluhan langsung di posyandu remaja. Hasil kegiatan menunjukkan peningkatan pengetahuan dan kesadaran remaja mengenai persiapan pranikah dan prakonsepsi. Program WASSINI memberikan dampak positif terhadap kesiapan remaja menuju kehidupan reproduksi yang sehat serta direkomendasikan untuk dilanjutkan dan diperluas dengan dukungan lintas sektor.

Abstract

Breast engorgement is a common problem among postpartum mothers, especially primigravida, which can cause pain, swelling, and hinder the breastfeeding process. Conventional management such as warm compresses often provides only temporary relief, thus alternative therapies that are more effective and natural are needed. Cocoa (Theobroma cacao L.) contains flavonoids, polyphenols, and cocoa butter with anti-inflammatory, antioxidant, and emollient properties. This study aimed to determine the effect of cocoa cream on reducing breast engorgement in primigravida postpartum mothers. This research employed a quasi-experimental design with a pretest-posttest control group approach. A total of 36 primigravida postpartum mothers were recruited and divided into two groups: the intervention group (cocoa cream) and the control group (warm compresses). Measurements were conducted on days 1, 3, 5, and 7 using breast circumference measurement and the Visual Analog Scale (VAS) for pain. Data were analyzed using Paired t-test and Independent t-test with a significance level of p<0.05. Independent t-test results showed no significant differences at pretest (p=0.674) and day 3 (p=0.068). However, significant differences were observed from day 5 (p=0.003) and became more pronounced on day 7 (p=0.000), with the intervention group demonstrating greater reduction in pain and swelling compared to the control group. Cocoa cream was more effective than warm compresses in reducing breast engorgement among primigravida postpartum mothers, particularly after the fifth day postpartum. Cocoa cream may be recommended as a safe, natural, and non-invasive complementary therapy to support successful breastfeeding.

Abstract

Kanker serviks masih menjadi salah satu penyebab utama kematian pada perempuan, namun dapat dicegah melalui deteksi dini menggunakan pemeriksaan Inspeksi Visual Asam Asetat (IVA) Test. Rendahnya cakupan pemeriksaan IVA Test di wilayah kerja UPTD Puskesmas Panumbangan menunjukkan masih rendahnya kesadaran masyarakat terhadap pentingnya deteksi dini kanker serviks. Kegiatan pengabdian kepada masyarakat ini bertujuan memberdayakan kader kesehatan melalui pembentukan KAGENCAR (Gerakan Kader Melek Pemeriksaan IVA Test) sebagai upaya meningkatkan edukasi dan partisipasi wanita usia subur dalam pemeriksaan IVA Test. Kegiatan dilaksanakan di Desa Sumberjaya dengan tahapan pengkajian masalah, analisis prioritas masalah, penyusunan inovasi, pengembangan media edukasi berupa lembar balik dan buku panduan, pelatihan kader, pembentukan organisasi KAGENCAR, serta simulasi penyuluhan menggunakan media edukasi. Hasil kegiatan menunjukkan bahwa program KAGENCAR berhasil membentuk kader khusus IVA Test yang memiliki pengetahuan dan keterampilan dasar dalam memberikan edukasi kepada masyarakat mengenai kanker serviks dan pentingnya pemeriksaan IVA Test. Lembar balik dan buku panduan menjadi media pendukung yang membantu kader menyampaikan informasi secara lebih sistematis dan mudah dipahami. Program KAGENCAR menjadi salah satu strategi pemberdayaan masyarakat yang mendukung peningkatan promosi kesehatan serta berpotensi meningkatkan partisipasi wanita usia subur dalam deteksi dini kanker serviks melalui pemeriksaan IVA Test secara berkelanjutan.

Abstract

Reproductive health is a fundamental competency that must be mastered by midwifery students because it is closely related to professional practice in maternal and child health services. This study aims to analyze determine the level of knowledge of students of the Faculty of Midwifery at the Medistra Health Institute, Lubuk Pakam, regarding reproductive health. The study design used a quantitative descriptive approach with a cross-sectional method. The study sample consisted of 100 students selected using a purposive sampling technique. Data were collected using a structured questionnaire and analyzed using SPSS software with descriptive statistical tests and Pearson correlation. The results showed that 72% of students had a good level of knowledge, 20% had a sufficient level of knowledge, and 8% had a poor level of knowledge. The correlation analysis showed a positive relationship between knowledge level and semester (r = 0.416; p < 0.05), meaning that the higher the semester, the better the level of knowledge. The conclusion of this study indicates that the level of student knowledge about reproductive health is quite good, but it is necessary to improve continuous education through contextual learning and field-based training.

Abstract

Breast engorgement is a common problem in postpartum mothers, caused by milk stasis due to constriction of the lactiferous ducts or inadequate emptying of the mammary glands. In Indonesia, this condition affects approximately 10–20% of breastfeeding mothers, or around 2.3 million women. The 2024 Indonesian Health Survey reported an incidence of 24.8% among lactating mothers in North Sumatra Province. This study aimed to determine the effectiveness of cabbage leaf compresses in reducing breast engorgement among postpartum mothers at the Nining Pelawati Midwife Clinic, Lubuk Pakam District, in 2025. A quasi-experimental design with a one-group pretest–posttest approach was used. A total of 35 postpartum mothers experiencing breast engorgement were selected through total sampling. The intervention consisted of cold cabbage leaf compresses applied twice daily for 20–30 minutes. Data were analyzed using the Wilcoxon test to compare engorgement levels before and after the intervention. Before treatment, most respondents experienced moderate (65.7%), severe (22.9%), and mild (11.4%) engorgement. After the intervention, engorgement levels decreased to mild in 65.7% of respondents, moderate in 28.6%, and severe in only 5.7%. Statistical analysis showed a significant difference between pre- and post-intervention conditions (Z = -3.140; p = 0.002). In conclusion, cabbage leaf compresses are significantly effective in reducing breast engorgement in postpartum mothers. This non-pharmacological method is recommended as a safe, simple, and cost-effective alternative for managing postpartum breast swelling.
