Abstract

A coagulometer is a tool used to check coagulation parameters, such as prothrombin time (PT) and activated partial thromboplastin time (aPTT), for which there are currently various detection methods. Examining PT and aPTT coagulation function requires a fast turnaround time and immediate results because the stability of coagulation factors is less than four hours, so a fast and precise examination is needed. Point-of-care testing (POCT) is a good alternative. This study aimed to determine the difference in aPTT and PT values between POCT and optical fully automatic. This comparison will be useful when selecting coagulometer devices. Methods: This study used a cross-sectional study with a non-probabilistic sample of 100 citrate samples. Each sample was tested for aPTT and PT using POCT and optical fully automatic testing. The results of each examination were analyzed using the Mann–Whitney, Bland-Altman Plot, and correlation statistical tests. In this study, precision tests were also carried out on both methods. methods. There was a significant difference (p < 0.001) in the comparative test results for the aPTT and PT values obtained using the POCT method compared to the optical fully automatic method. The average difference between the two methods was 6.6 seconds for the aPTT parameter and 4.98 for the PT parameter. Fourteen samples were found to have different interpretation values for the aPTT parameter, and 28 samples for the PT parameter. There is a significant difference between the POCT method and the optical fully automatic method. The precision test on the POCT method produced good results.

Abstract

High Fluorescence Cell (HFC) is a cell with high nucleoplasmic and nucleic acid content, enabling it to emit a high fluorescent signal. Mind Ray Series BC 760 automated hematology analyzer uses a technology called Sheath Fluid (SF) Cube to detect these cells using the fluorescence staining method. An increase in HFC may be associated with atypical or pleomorphic lymphocytes caused by viral infection rather than immature or monomorphic lymphocytes caused by malignancy. Objective: The objective of this study is to determine the difference in high fluorescence cell in pediatric patients with Acute Lymphoblastic Leukemia (ALL) and infection at Adam Malik Hospital. Methods: This study is an observational analytical study with a prospective cohort design. The study sample was 35 people with a diagnosis of ALL of 15 people and infection of 20 people. This study was conducted on patients who came to the pediatric clinic and were hospitalized with a diagnosis of ALL and infection at Adam Malik Hospital who met the inclusion criteria and agreed to participate in this study. A high fluorescence cell blood test was performed.  criteria. A complete blood test of 2 ml of blood in an EDTA vacutainer was gently homogenized. Analysis with a full blood count (FBC) test was performed using an automated hematology analyzer (Mindray BC 760) to obtain the number of lymphocytes and high fluorescence cells. Descriptive statistical analysis was used for demographic data. Statistical analysis used the Mann Whitney statistical test. The research data were analyzed statistically with the help of SPSS and the difference was considered significant if p value <0.05. Results: From the study sample of 35 people, 24 boys (68.6%) and 11 girls (31.4%), with the mean age of 6 years old. The diagnosis of ALL was 15 people (42.9%) while patients with infection were 20 people (57.1%). The median absolute HFC  in ALL was 0.2 x 109/L, and the median in infection was 0.15 x 109/L. Conclusion: There was no significant difference between HFC in patients with ALL and infection (p=0.211).

Abstract

Tuberculosis (TB) remains a major global health concern, with children representing a substantial proportion of cases. Diagnosis in paediatric populations is challenging due to non-specific clinical features and limited bacteriological confirmation, making chest radiography and molecular rapid tests essential. This study aimed to evaluate the association between chest radiographic abnormalities and molecular rapid test results in paediatric pulmonary TB patients. A retrospective cohort study was conducted at Haji Adam Malik General Hospital from January to May 2025, involving 60 children diagnosed with pulmonary TB. Data were collected through consecutive sampling, comprising demographic characteristics, chest radiographic findings, and molecular rapid test (GeneXpert). Statistical analysis was performed using the chi-square test to evaluate diagnostic parameters. Results: Among 60 patients (46.7% male; mean age 10 years), 48.3% showed abnormal chest radiographs, predominantly infiltrates (60%) and pleural effusion (23.3%). GeneXpert results were evenly distributed, with 30 positive and 30 negative cases. A significant association was found between radiographic abnormalities and molecular test results (p < 0.001). Diagnostic parameters for radiographs included sensitivity 0%, specificity 3.3%, and accuracy PPV 0% and NPV 3,2%. Chest radiography is useful for initial paediatric TB screening but less accurate than molecular tests.

Abstract

Prothrombin time (PT) and activated partial thromboplastin time (aPTT) tests are important screening tests for assessing coagulation pathway function and monitoring anticoagulant therapy. These tests use a coagulation analyzer with different detection methods, namely optical and electromechanical methods. The working principles of the two methods can cause variations in test results, especially in samples with icteric interference. Therefore, it is necessary to evaluate the comparison of PT and aPTT values in icteric samples using both methods.  This study aimed to evaluate the differences in PT and aPTT values in icteric samples using optical and electromechanical methods. This study used a prospective analytical design with a cross-sectional approach, conducted at Adam Malik Hospital, Medan, from July to August 2025. A total of 69 samples that met the inclusion criteria were selected using sequential sampling techniques. Each sample is tested for PT and aPTT using optical and electromechanical methods. The results of each test were analyzed using the Mann-Whitney, Kappa, correlations tests, and Bland-Altman plots. Significant differences were observed in PT and aPTT values between the optical and electromechanical methods (p<0.001), while INR values showed no significant difference (p=0.426). Interpretation agreement was moderate for PT (κ=0.599; p<0.001) and strong for aPTT (κ=0.661; p<0.001). Despite numerical differences, both methods provided comparable clinical interpretations, and total bilirubin showed no association with PT or aPTT values. The optical and electromechanical methods produced comparable clinical interpretations despite differences in PT and aPTT values, and total bilirubin levels were not associated with PT or aPTT results.
