Abstract

Background: Depression is a psychiatric complication of stroke that often occurs and had a negative impact. Every year, 15 million people who experience strokes globally are at risk of developing post-stroke depression. Cognitive disorders are often associated with the presence of post-stroke depression. Understanding of risk factors for post-stroke depression can help in clinical identification and provide early intervention to improve stroke rehabilitation better. Therefore, it is very important to identify patients at risk for depression. This study aims to analyze cognitive impairment factors in the incidence of post-stroke depression.Subjects and Method: The meta-analysis was conducted from Pubmed, Science Direct, and Springer Link databases using "stroke", "depression", "post-stroke depression AND risk factor" keywords. The article chosen was an article published from January 1, 2009 to September 30, 2018. The data were analyzed using RevMan 5 program.Results: 6 studies consisting of 3140 patients were selected for this study. The results of a meta-analysis showed that cognitive impairment was a risk factor for post-stroke depression. Cognitive disorder was positively and significantly associated with post-stroke depression (OR = 1.24; 95% CI= 1.03 to 1.49; p = 0.02).Conclusion: Cognitive disorder is a risk factor for post-stroke depression.Keywords: depression, post-stroke, cognitive impairment, risk factor, meta-analysisCorrespondence:

Abstract

Background: Diabetes Mellitus (DM) is a group of metabolic diseases characterized by hyperglycemia resulting from abnormalities in insulin secretion that affect blood sugar levels. Chronic hyperglycemia in DM is associated with long-term damage to several organs of the body. One of the interventions that can be given to lower blood sugar levels is acupressure therapy. The purpose of this study was to determine the effect of acupressure therapy on blood sugar levels in patients with diabetes mellitus.Subjects and Method: This study is a systematic review and meta-analysis with PICO, P: Diabetes mellitus patients, I: Acupressure, C: No acupressure, O: Blood sugar levels. The process of searching for articles is done through a database of journals including: Pubmed, Google Scholar, Science Direct, Scopus, Spinger Link, CNKI and Hindawi. The keywords used in the literature search were: “acupressure” AND “diabetes” AND “blood sugar” AND “blood glucose” AND “standard controlled trial”, “acupressure for diabetes”, “diabetes standardized controlled trial”, “acupressure randomized controlled trial”. trial”, diabetes acupoint, “diabetes and placebo”, “acupressure” AND “diabetes”. The articles included in the study were full text articles with a randomized controlled trial study design and the results were reported with the effect size, mean standard deviations (SMD). Articles were collected using PRISMA flow diagrams and analyzed using the Review Manager 5.3 application.Results: A meta-analysis of 9 articles reviewed in this study came from Taiwan, China, Indonesia, Egypt and Iran with a sample size of 451. The study results showed that acupressure therapy was able to reduce blood sugar levels in diabetes mellitus patients with SMD blood sugar values. acupressure was 0.56 units lower than no acupressure and was statistically significant with a significance value of (SMD=-0.56; 95% CI= -0.76 to -0.37; p<0.001).Conclusion: Acupressure therapy has the effect of lowering blood sugar levels in diabetes mellitus patients.Keywords: acupressure, blood sugar, diabetes mellitus.Correspondence: Muslihatin Khuril Rosyida. Abstract

Background: Hypertension is currently starting to show a trend in prevalence that continues to increase at a younger age and is a major cause of premature death in the world. In adolescents, hypertension is more common who are overweight or obese. This study aims to examine the effect of overweight and obesity on the incidence of hypertension in adolescents using a meta-analysis.Subjects and Method: Meta-analysis was carried out using the PRISMA flow chart and the PICO model. Population: adolescents aged 10 to 19 years. Intervention: overweight and obesity. Comparison: normoweight. Outcome: hypertension). The databases used are PubMed, Science Direct, Springer Link, and Google Scholar with the keywords (hypertension OR "High Blood Pressure") AND "BMI Status" AND (adolescent OR teenager) AND "cross sectional". There were 9 cross-sectional studies published in 2012-2022 with odds ratio (OR) effect size that met the inclusion criteria. Analysis was performed with RevMan 5.3.Results: A meta-analysis was conducted on 9 articles with a cross-sectional study design originating from China, South Korea, India, Turkey, Tunisia, Lithuania and Poland involving 63,239 adolescents aged 10-19 years. The results of the meta-analysis showed that overweight adolescents were 2.44 times more likely to experience hypertension compared to normal weight adolescents (aOR= 2.44; 95%CI= 1.87 to 3.19; p<0.001), and obese adolescents were more likely to experience hypertension 4.53 times compared with normal weight adolescents (aOR= 4.53; 95% CI= 3.10 to 6.61; p<0.001).Conclusion: Being overweight and obese can increase the risk of developing hypertension in adolescents. Abstract

Background: People who have a stroke are generally more likely to have difficulty managing daily activities such as walking, bathing, dressing, and participating in activities in the community and showing symptoms of depression that affects the quality of life. Telehealth is here to help people communicate with health workers without having to come directly to health facilities. The study aims to analyze how much effect telehealth has in improving the quality of life and reducing the risk of depression in post-stroke patients.Subjects and Method: This was a meta-analysis study using PICO. Population: post-stroke patients. Intervention: telehealth. Comparison: no telehealth. Outcome: quality of Life and depression. This study used articles from several databases namely Pubmed, Google Scholar, Science Direct, and Cochrane Library using the keywords "Stroke" AND "telehealth" OR "Telerehabilitation" OR "Tele-medicine" OR "Mobile health" OR "mHealth " OR "eHealth" AND "Quality of Life" OR "Health-Related Quality of Life" AND "Depression" AND "Randomized Controlled Trial" OR "RCT" to identify relevant studies. The articles included in the study were articles in English with RCT study design, published between 2012 and 2022. Steps in the systematic review were carried out based on guidelines from PRISMA flow diagram. Quantitative analysis was performed using Revman 5.3.Results: 8 RCTs article from America, Asia, and Europe showed that post-stroke patients who obtained telehealth services on average had 0.87 units higher quality of life than those without telehealth (SMD=0.87; 95% CI =0.30 to 1.44; p=0.003). 8 RCTs article from America and Europe showed that post-stroke patients who obtained telehealth services on average had 0.44 units lower depression than those without telehealth (SMD= -0.44; 95% CI=-0.79 to -0.09; p= 0.010).Conclusion: Telehealth can improve quality of life and lower the risk of depression in post-stroke patients. Abstract

Background: Hypertension is a condition that causes a person to experience a drastic rise in blood pressure. Social support and self-efficacy are needed to improve skills, self-confidence and a sense of security for patients to be more compliant with taking medication. This study aimed to analyze the effect of social support and self-efficacy on medication adherence in hypertensive patients.Subjects and Method: This study is a meta-analysis using PICO, namely population: hypertensive patients; Intervention: good social support, good self-efficacy; Comparison: low social support, low self-efficacy; Result: medication adherence. The articles used in this study were obtained from two databases, namely PubMed and Google Scholar. Keywords in the article search “social cognitive theory (SCT)” AND “self efficacy“ AND ”social support “AND” hypertension” AND “medication adhe-rence”, cross-sectional. The included article is full English with a cross-sectional study design for 2012–2022 and reports the adjusted odds ratio (aOR) in multivariate analysis. Selection of articles using the PRISMA flowchart. Article analysis using the Review Manager 5.3 application.Results: 7 articles with a cross-sectional study involving 708 people with hypertension from 2 conti-nents namely Asia and Africa for systematic review and meta-analysis. The data collected showed that hypertensive patients with strong social support were twice as likely to adhere to medication as hypertensive patients with weak social support (aOR= 2.16; 95%CI= 1.82 to 2.57; p <0.001), Hyper-tensive patients with self-efficacy Those who are strong are more likely to adhere to medication twice than hypertensive patients with weak self-efficacy (aOR= 2.11; 95% CI = 1.27 to 3.52; p <0.004).Conclusion: Social support and self-efficacy improve medication adherence in hypertensive patients. Abstract

Background: The impact of social isolation and loneliness on health and well-being is recognized globally as a public health problem. Social isolation can be experienced at any age, especially the elderly. This study aims to analyze and estimate the influence of social isolation and loneliness on cardiovascular disease and death in the elderly.Subjects and Method: The meta-analysis was carried out according to the PRISMA flow chart and the PICO model. P: Elderly, I: Social isolation and loneliness, C: Not social isolation and not lonely, O: Cardiovascular disease and death. Search for articles in this study through databases that include PubMed, Google Scholar, Science Direct and Scopus with keywords "social isolation" AND "loneliness" AND "cardiovascular disease" AND "elderly". “Social isolation” AND “loneliness” AND “mortality” AND “elderly”. Full paper article with cohort design, adjusted Hazard Ratio for effect size, research outcome are cardiovascular disease and death. Analysis was performed using Revman 5.3.Results: There were 18 articles with cohort designs originating from America, Finland, Canada, Tai-wan, Australia, the United States, Japan, Denmark, Amsterdam, England, Sweden, Jerusalem, Europe with a total of 1,375,816 research samples. A meta-analysis of 13 cohort studies concluded that seniors who were socially isolated had a 1.23 times the risk of developing cardiovascular disease compared to those who were not socially isolated, and the effect was statistically significant (aHR= 1.23; 95% CI= 1.10 to 1.38; p= 0.003). In addition, the elderly who are socially isolated have a risk of dying 1.16 times compared to those who are not socially isolated, and this effect is statistically significant (aHR= 1.16; 95% CI= 1.07 to 1.21; p= 0.003). A meta-analysis of 13 cohort studies concluded that lonely elderly people had a 1.10 times greater risk of developing cardiovascular disease compared to lonely people, and the effect was statistically significant (aHR= 1.10; 95% CI= 1.01 to 1.20; p= 0.003). In addition, lonely elderly people have a risk of dying 1.19 times compared to being lonely, and the effect is statistically significant (aHR= 1.19; 95% CI= 1.02 to 1.39; p= 0.002).Conclusion: Social isolation and loneliness increase the risk of cardiovascular disease and death in elderly. Abstract

Background: WHO states that as many as 300 million children have experienced physical punishment or psychological violence. These traumatic events can cause Post Traumatic Stress Disorder (PTSD) which has an impact on the lives of victims of physical and sexual violence. The purpose of this study was to analyze and estimate the average influence of physical and sexual violence in childhood on the incidence of Post Traumatic Stress Disorder (PTSD) using a meta-analysis study.Subjects and Method: This research is a systematic review and meta-analysis with the selection of PRISMA flow diagrams. Population= adults aged >18 years. Intervention= experience of physical and sexual violence as children. Comparison= no experience of physical and sexual violence as children. Outcome= PTSD. Article searches through journal databases include: Pubmed, Google Schoolar, Science Direct, Researchgate and Springerlink. The articles used in this study are articles that have been published from 2000-2022. The keywords used in the database are “Child Physical Abuse”[tw] OR “Childhood Physical Abuse”[tw] OR “Child Maltreatment”[tw] OR “Physical Violence”[tw] OR "Physical Abuse"[Mesh] OR " Child Abuse"[Mesh] AND “Sexual Abuse”[tw] OR “Child Sexual Molestation”[tw] OR "Child Abuse, Sexual"[Mesh] OR "Sexual Trauma"[Mesh] AND “Post-Traumatic Stress Disorder*” [tw] OR “Posttraumatic Stress Disorder*” OR “Post-Traumatic Neuroses”[tw] OR “Post-traumatic Neuroses”[tw] OR “PTSD”[tw] OR "Stress Disorders, Post-Traumatic"[Mesh]. Inclusion criteria included full-text articles with a cross-sectional study design. The analysis used multivariate logistic regression with adjusted Odds Ratio and published in English. Articles that met the requirements were analyzed using the Revman 5.4 application.Results: Meta-analysis of 10 articles showing for childhood physical abuse (aOR=1.87; 95% CI=1.47 to 2.37; p<0.001) and childhood sexual violence (aOR=2.67; 95% CI=2.02 up to 3.52; p<0.001).Conclusion: Experience of physical and sexual violence in childhood increases the risk of developing Post Traumatic Stress Disorder as an adult. Abstract

Background: Community health center (puskesmas) management information system or often referred to as SIMPUS is a local health system aimed at providing health information at primary health care level. P Care (Primary Care) application is an information system that is designed to serve BPJS patients. The bridging system links SIMPUS with P care application. These three systems have been used in all puskesmas in Sukoharjo district since 2017. This study aimed to evaluate the implementation of community health center management information system (Simpus), Primary Care (P Care), and bridging data system, in Sukoharjo District, Central Java.  Subjects and Method: This was a qualitative study. The study was conducted in Sukoharjo, Kartasura, Bendosari, Mojolaban community health centers, in Sukoharjo District, Central Java, from October to December, 2017. Four community health centers were selected randomly from 13 community health centers in Sukoharjo. The dependent variables were information service, performance, efficiency, and security. The independent variable was health information system (SIMPUS, P care, and bridging system). The data were collected by in-depth interview, direct observation, and document review. Data credibility was checked by triangulation method.Results: Three health information systems (SIMPUS, P care, and bridging system) have been implemented in Sukoharjo District. However, their performance is yet to be improved. Loading time was lengthy and error often occured. Patients often complained of the lengthy service. Some items in the information system form were left blank. Data entry occasionally was inconsistent with examination outcome. Security system of the health information existed, but sometimes unauthorized individuals can access and use that information. There was a lack of human resources and hardware. The health information system had not met the information need.Conclusion: Three health information systems (SIMPUS, P care, and bridging system) have been implemented in Sukoharjo District. Abstract

Background: The performances of the doctors and the nurses are especially important in accomplishing health care in a continuous and effective way. It is important to examine all organizational, individual, social factors that especially affect the performances of the doctors and nurses holistically. The purpose of this study was to assess factors affecting work performance among nurses in delivering health service for the national health insurance patientsat Dr. Moewardi Hospital, Surakarta.Subjects and Method: A cross-sectional study was carried out in Dr. Moewardi Hospital, Surakarta, Central Java. A sample of 150 nurseswho provided health services to the National Health Insurance patientswas selectedfor this study by proportional random sampling. The dependent variable was work performance. The independent variables were work satisfaction, work motivation, and supervision. The data were collected by questionnaire and analyzed by path analysis run on Stata 13.Results: Work performance was affected by work satisfaction (b=0.88; 95%CI=0.05 to 1.71; p=0.037), work motivation (b=0.81; 95%CI=-0.06 to 1.68; p=0.070), and supervision (b=0.97; 95%CI=0.18 to 1.76; p=0.016). Work performance was also indirectly affected by motivation. Conclusion: Work performance is affected by work satisfaction, work motivation, and supervision.
