Abstract

The pharmacokinetic profile of valproic acid in pediatric subjects in Asian populations is lacking. One of the most useful ways to develop a personalized dosing strategy is to use population pharmacokinetics. The focus of this systematic review is to describe the population pharmacokinetic profile of valproic acid (VPA) and determine the variables influencing pharmacokinetic models of the drug in children with epilepsy that have been published in Asian journals. A comprehensive literature search was carried out from 2014 to 2024 on Science Direct and PubMed.  Keywords used valproic acid, valproic, VPA, sodium valproate  and population pharmacokinetic, pharmacokinetic model. In this review, four studies were considered. The pharmacokinetic model was developed by looking at several covariates that significantly affect the population pharmacokinetics of VPA using TDM data from pediatric epilepsy patients. The clearance value and volume of distribution of VPA can be seen to be in the range of 0.148 - 3.19 L/h, and 0.317 - 24 L/KgBB. In addition, subjects with SCN1A A/A genotype showed a decreased therapeutic effect of VPA. Significant covariates affecting the pharmacokinetics of VPA were age, dose, and the use of comedication with other anti-seizure drugs.

Abstract

Salah satu penyakit degeneratif yang paling sering diderita pada pasien geriatri yaitu hipertensi. Hipertensi dikenal dengan meningkatnya tekanan darah yang melebihi batas normal yaitu 140 mmHg untuk tekanan darah sistolik dan 90 mmHg untuk tekanan darah diastolik. Tujuan dari penelitian ini adalah untuk mengetahui bagaimana gambaran pola peresepan obat antihipertensi pada pasien geriatri. Penelitian ini bersifat observasi/deskriptif dengan pengambilan data retrospektif. Data diambil dari catatan rekam medik pada pasien geriatri. Kriteria inklusi yang digunakan yaitu pasien geriatri dengan diagnosis hipertensi primer (I10 Esseential [Primary] Hypertension) yang dirawat inap di RSUP Dr. Sardjito. Hasil penelitian dari 98 pasien, 45,9% diberi resep monoterapi, 35,7%menggunakan dua kombinasi obat antihipertensi, 11,2% diberi tiga kombinasi obat antihipertensi, dan 7,1% diberi empat kombinasi obat. Pada penelitian ini yang paling banyak diresepkan pada pasien geriatri rawat inap RSUP Dr. Sardjito yaitu amlodipin sebesar 27,55%. Calcium channel blocker khususnya amlodipin merupakan obat antihipertensi yang paling sering digunakan, dan ditemukan pada pasien geriatric dianggap efektif dan dapat ditoleransi dengan baik.

Abstract

Type 2 diabetes mellitus is a chronic disease with a globally increasing prevalence. The treatment of diabetes mellitus involves oral blood glucose-lowering agents, including glibenclamide. Although effective, glibenclamide can cause severe hypoglycemia, leading people to often combine it with herbal remedies such as the root extract of Eurycoma longifolia (ELRE), commonly known in Malaysia and Indonesia as the herbal medicine Tongkat Ali. ELRE is known for its anti-diabetic effects through enhanced insulin sensitivity and antioxidant activity. However, the pharmacokinetic interaction between ELRE and glibenclamide, particularly involving CYP3A4, which metabolizes glibenclamide, has not been well studied. This study employed an in silico approach using pharmacokinetic virtual screening server, followed by molecular docking analysis to evaluate the potential interaction. The results indicated that nearly all of the investigated ELRE chemical constituents showed high predicted affinity, with two compounds, 14,15β-dihydroxyklaineanone and niloticin, exhibiting higher affinity than glibenclamide for CYP3A4. These compounds may act as competitive substrates, potentially influencing glibenclamide metabolism.
