Abstract

This study aims to analyze the effectiveness of the BPJS patient admission workflow following the implementation of the Electronic Medical Record (EMR) system in the outpatient registration unit, specifically regarding service speed, data accuracy, and adherence to Standard Operating Procedures (SOPs). This is a descriptive qualitative study utilizing an observational approach. Data were collected through direct observation of the BPJS patient registration workflow; interviews with registration staff, IT personnel, and patients; and a review of SOP documentation and system reports. The implementation of the EMR system proved effective in enhancing BPJS patient registration services. Results indicate that the EMR implementation improved the effectiveness of the BPJS patient admission workflow; registration time decreased from an average of 12–15 minutes to 6–8 minutes per patient due to the integration of the EMR with VClaim and the Mobile JKN queue system for verifying SEP (Participant Eligibility Letter), eligibility status, and fingerprints. Patient data accuracy also improved. Enhancements are needed regarding network infrastructure (including backup internet), the refinement of SOPs to better integrate the EMR with BPJS systems, and continuous training for registration staff to optimize EMR usage, thereby making the BPJS patient admission workflow more effective and efficient

Abstract

Abstract

Abstract

Electronic Medical Records (EMRs) are increasingly required to support efficient, accurate, and integrated hospital services. This study evaluated the implementation of EMRs in inpatient services at Bakti Pajajaran Regional Public Hospital using the Human-Organization-Technology Fit (HOT-FIT) model. A descriptive qualitative case-study design was applied. Data were collected from February to March 2026 through HOT-FIT-based observations and semi-structured interviews with six purposively selected informants who had at least one year of experience using the EMR system: the head of the medical records unit, an inpatient physician, an inpatient administrator, a nurse, a nutritionist, and an information technology officer. The findings indicate that the Human component was generally adequate, as users possessed relevant professional or technical qualifications and were able to operate the EMR according to their duties. The Organization component was supported by hospital policies, basic training, interprofessional coordination, and system access, although full integration had not yet been achieved because several units still used a hybrid workflow. The Technology component was supported by computers, servers, LAN/Wi-Fi, and VPN access; however, unstable or slow network connections remained the dominant operational barrier. Overall, the Fit among human, organizational, and technological components was favorable but constrained by network reliability. Regular user training, routine system evaluation, strengthened technical support, improved bandwidth, and backup connectivity are therefore recommended to sustain EMR implementation and optimize its net benefits for inpatient care

Abstract

The implementation of electronic signatures in electronic medical records (EMRs) is intended to strengthen authentication, document integrity, accountability, and legal certainty in digital health services. This study reviewed the legality and security of electronic signature use in EMRs at Bakti Pajajaran Regional Public Hospital. A normative-empirical legal design with a descriptive qualitative approach was applied. Primary legal materials and relevant secondary literature were examined, while field data were obtained through in-depth interviews, observation, and documentation from 30 purposively selected informants representing medical records personnel, nurses, midwives, nutritionists, radiographers, information technology staff, and the head of the medical records installation. The study found that all 237 healthcare providers had used electronic signatures in the hospital information environment; however, none of the clinical electronic signatures were certified. The only certified electronic signature was used by the hospital director for administrative documents through the SRIKANDI application. Twenty of 32 reviewed medical record forms had been converted to electronic format, whereas 12 forms, particularly consent-related documents, remained manual. Major barriers included limited storage infrastructure, financing constraints, insufficient periodic socialization, and the absence of a dedicated standard operating procedure. These findings indicate that administrative adoption alone does not ensure adequate legal assurance and information security. The hospital should accelerate certified electronic signature implementation, establish specific governance procedures, strengthen security infrastructure, and provide sustainable organizational and financial support.

Abstract

The implementation of Electronic Medical Records (EMR) is an important component of digital transformation in healthcare services. This study aimed to evaluate the implementation of EMR within the Khanza Hospital Management Information System (SIMRS) using the HOT-FIT method at KMM Sukabumi Eye Specialist Clinic. A descriptive qualitative design with an evaluative approach was applied to eight informants. Data were collected through direct observation, in-depth interviews, document review, and literature review. The findings showed that the Human, Organization, and Technology dimensions generally supported EMR implementation and generated benefits such as facilitating staff work, reducing duplicate documentation, improving access to patient information, and supporting service quality. Nevertheless, implementation had not yet reached an optimal level. Identified constraints included the longer adaptation period required by new users, uneven training, geographically distant IT support, incomplete utilization of Khanza SIMRS features, and monitoring and evaluation that were not conducted regularly. Basic technological infrastructure, including computers, LAN/Wi-Fi, servers, UPS/stabilizers, and daily data backup, was available; however, printer/scanner availability remained limited and network disruptions occasionally slowed BPJS bridging and service processes. Regular and equitable training, stronger and more responsive technical support, optimization of system features according to clinic needs, additional supporting devices, and scheduled monitoring and evaluation are therefore recommended to improve the sustainability and effectiveness of EMR implementation.
