Medication adherence is a crucial factor in the successful management of type 2 diabetes mellitus (T2DM), particularly among older adults who are more vulnerable to treatment failure due to age-related physiological decline and multiple comorbidities. This study aimed to determine the level of medication adherence and analyze the factors associated with medication adherence behavior among older adults with type 2 diabetes mellitus at Kembaran 1 Primary Health Center. This quantitative study employed a cross-sectional design involving 62 older adults with type 2 diabetes mellitus selected using a total sampling technique. Medication adherence was assessed using the Medication Adherence Rating Scale-10 (MARS-10), while the associations between variables were analyzed using the Chi-square test. The results showed that 64.5% of the respondents had a high level of medication adherence, whereas 35.5% demonstrated low adherence. Bivariate analysis revealed that age (p = 0.008), educational level (p = 0.021), employment status (p = 0.013), body mass index (BMI) (p = 0.015), and type of antidiabetic therapy (p = 0.001) were significantly associated with medication adherence behavior. In contrast, sex (p = 0.675), duration of diabetes mellitus (p = 0.510), and the presence of comorbidities (p = 0.519) were not significantly associated with medication adherence. These findings indicate that medication adherence among older adults with type 2 diabetes mellitus is influenced by sociodemographic factors, clinical characteristics, and treatment-related factors. Identifying these determinants may serve as a basis for developing intervention strategies to improve medication adherence and optimize therapeutic outcomes.
