Abstract

This study aimed to analyze the effectiveness of antihypertensive drugs from the Calcium Channel Blocker (CCB) and Angiotensin Receptor Blocker (ARB) classes in reducing blood pressure among hypertensive patients. The study employed a retrospective observational design using medical record data collected from November 2024 to February 2025 and was conducted in four primary healthcare centers in Banyumas Regency. A total of 70 patients enrolled in the PROLANIS program were selected using purposive sampling. Data were analyzed based on changes in blood pressure and statistically tested using the One-Way ANOVA test. The results showed that the ARB + CCB combination therapy was the most frequently used regimen (50%) and achieved the lowest mean blood pressure value (129.94/77.25 mmHg) among the therapy groups. Statistical analysis indicated a significant difference in systolic and diastolic blood pressure between patients receiving monotherapy and combination therapy (p < 0.05). These findings suggest that the ARB + CCB combination therapy was associated with greater blood pressure reduction compared to monotherapy among hypertensive patients in primary healthcare settings.

Abstract

TB (Tuberculosis) patients in Indonesia are the 2nd highest in the world after India. The large number of TB patients in Indonesia is due to the lack of success in TB treatment due to patient non-compliance with medication regularly. Factors that affect non-compliance are low knowledge of TB patients and lack of family support. This study aims to determine the level of family knowledge and support, medication adherence, and determine the relationship between the level of knowledge and family support to medication adherence in TB patients at the Kroya Health Center, Cilacap Regency in 2024. This study uses a non-experimental method with cross-sectional data collection conducted in April-May 2024. The study population is all TB patients totaling 130 respondents who underwent treatment at the Kroya Health Center, Cilacap Regency from November 2023 to May 2024, and the research sample was 56 respondents. The conclusion of this study is that the knowledge level of TB respondents is the highest at 37.5% and 55.4%, and the adherence to taking medication at the Kroya Health Center is 30.4% and 57.1%. In this study, the highest TB patients were at a moderate level of knowledge (55.4%); family support is not supportive (96.4%) and medication adherence is low (30.4%) to moderate (57.1%). Testing with the chi-square method there was a relationship between the level of knowledge and medication adherence in TB patients (p-value 0.000), but there was no relationship between family support and medication adherence (p-value 0.268) in patients at the Kroya Health Center, Cilacap Regency with a r value of 0.541. The limitation of this study is that there is no follow-up on the respondents such as monitoring TB drugs so that the existing data is only data taken when filling out the questionnaire.

Abstract

Medication adherence is a crucial factor in the successful management of type 2 diabetes mellitus (T2DM), particularly among older adults who are more vulnerable to treatment failure due to age-related physiological decline and multiple comorbidities. This study aimed to determine the level of medication adherence and analyze the factors associated with medication adherence behavior among older adults with type 2 diabetes mellitus at Kembaran 1 Primary Health Center. This quantitative study employed a cross-sectional design involving 62 older adults with type 2 diabetes mellitus selected using a total sampling technique. Medication adherence was assessed using the Medication Adherence Rating Scale-10 (MARS-10), while the associations between variables were analyzed using the Chi-square test. The results showed that 64.5% of the respondents had a high level of medication adherence, whereas 35.5% demonstrated low adherence. Bivariate analysis revealed that age (p = 0.008), educational level (p = 0.021), employment status (p = 0.013), body mass index (BMI) (p = 0.015), and type of antidiabetic therapy (p = 0.001) were significantly associated with medication adherence behavior. In contrast, sex (p = 0.675), duration of diabetes mellitus (p = 0.510), and the presence of comorbidities (p = 0.519) were not significantly associated with medication adherence. These findings indicate that medication adherence among older adults with type 2 diabetes mellitus is influenced by sociodemographic factors, clinical characteristics, and treatment-related factors. Identifying these determinants may serve as a basis for developing intervention strategies to improve medication adherence and optimize therapeutic outcomes.
