Abstract

Hypertension is a chronic disease requiring long-term treatment, making adherence to antihypertensive medication essential for blood pressure control and the prevention of complications. Pharmacist counseling, as part of clinical pharmacy services, is one approach that may support patients' adherence to medication therapy. This study aimed to analyze changes in medication adherence among patients with hypertension following pharmacist counseling at Apotek Nusantara, Palembang, Indonesia. A quasi-experimental study using a one-group pretest–posttest design was conducted from April to June 2025. A total of 73 patients with hypertension who met the inclusion criteria were enrolled. Medication adherence was assessed before and after pharmacist counseling using an educational booklet and the Morisky Medication Adherence Scale-4 (MMAS-4). Data were analyzed using the Wilcoxon Signed-Rank test with a 95% confidence level. At baseline, all participants (100%) demonstrated low medication adherence. Following pharmacist counseling, 45 participants (62%) achieved moderate medication adherence, while 28 participants (38%) remained in the low-adherence category. The Wilcoxon Signed-Rank test demonstrated a statistically significant difference in medication adherence before and after counseling (p < 0.001). These findings indicate a change in medication adherence following pharmacist counseling among patients with hypertension. Further studies employing controlled study designs are needed to provide stronger evidence regarding the effect of pharmacist counseling on medication adherence.
