Abstract

Pregnancy at the age of ≤16 years is considered a high-risk pregnancy because the reproductive organs and psychological condition of the mother have not yet developed optimally. This condition increases the risk of various complications, one of which is anemia during pregnancy. Anemia can cause health problems for both the mother and fetus if not properly managed. Methods: This study used a single case report design. The subject was Mrs. I, a 15-year-old primigravida (G1P0A0) with a high-risk pregnancy due to maternal age ≤16 years and moderate anemia. The study employed a midwifery management approach consisting of subjective and objective data assessment, data analysis, and implementation of midwifery care. The care was provided from April to June 2026 through three home visits. At the first visit, Mrs. I was diagnosed with G1P0A0 at 26–27 weeks of gestation. Based on the Poedji Rochjati Pregnancy Risk Score (KSPR), she obtained a score of 6, indicating a high-risk pregnancy. Laboratory examination at the second visit showed a hemoglobin level of 7.8 g/dL, indicating moderate anemia. The midwifery care provided included education regarding high-risk pregnancy and anemia, balanced nutrition counseling, recommendations to take iron (Fe) tablets twice daily with proper administration, health education regarding danger signs during pregnancy, monitoring of treatment adherence, and referral planning to a hospital for further management. Throughout the care process, the mother was cooperative, understood all the education provided, and was willing to follow the recommendations of the healthcare provider. This case study demonstrates that the midwifery care provided was appropriate to the needs of a pregnant woman aged ≤16 years with moderate anemia. Early detection, health education, adequate nutritional intake, adherence to iron tablet consumption, regular antenatal monitoring, and appropriate referral are important efforts to prevent complications and improve maternal and fetal health.

Abstract

Pregnancy with anemia is a condition in which a pregnant woman experiences low hemoglobin levels (generally <11 g/dL), reducing the blood’s ability to carry oxygen and potentially affecting maternal and fetal health. Moreover, maternal age >35 years is classified as a high risk group, making appropriate care essential to prevent complications and maintain health during pregnancy. This case report aims to describe midwifery care for a multigravida woman in the second trimester, aged over 35 years, with moderate anemia, conducted at the Independent Midwifery Practice U Kamal, Bangkalan, between March–June 2026, and at the home of Mrs. M. At the first visit, the mother presented with moderate anemia symptoms including pallor, weakness, easy fatigue, and shortness of breath. Risk factors included advanced maternal age, noncompliance with iron tablet intake, and limited nutritional intake. At the second visit, the mother reported back pain, which is a physiological condition in pregnancy. By the third visit, the mother was no longer anemia, supported by general examination findings of pink conjunctiva. Thus, the remaining risk factor was maternal age, as anemia was no longer present. Management consisted of nutritional counseling, administration of iron tablets, and recommendations to consume iron rich foods along with vitamin C. Comprehensive midwifery care combining supplementation, education, and family support is crucial to reduce the risk of complications and to minimize moderate anemia in multigravida pregnant women aged >35 years.
