Abstract

Pregnancy with anemia is a condition in which a pregnant woman experiences low hemoglobin levels (generally <11 g/dL), reducing the blood’s ability to carry oxygen and potentially affecting maternal and fetal health. Moreover, maternal age >35 years is classified as a high risk group, making appropriate care essential to prevent complications and maintain health during pregnancy. This case report aims to describe midwifery care for a multigravida woman in the second trimester, aged over 35 years, with moderate anemia, conducted at the Independent Midwifery Practice U Kamal, Bangkalan, between March–June 2026, and at the home of Mrs. M. At the first visit, the mother presented with moderate anemia symptoms including pallor, weakness, easy fatigue, and shortness of breath. Risk factors included advanced maternal age, noncompliance with iron tablet intake, and limited nutritional intake. At the second visit, the mother reported back pain, which is a physiological condition in pregnancy. By the third visit, the mother was no longer anemia, supported by general examination findings of pink conjunctiva. Thus, the remaining risk factor was maternal age, as anemia was no longer present. Management consisted of nutritional counseling, administration of iron tablets, and recommendations to consume iron rich foods along with vitamin C. Comprehensive midwifery care combining supplementation, education, and family support is crucial to reduce the risk of complications and to minimize moderate anemia in multigravida pregnant women aged >35 years.

Abstract

Low back pain is one of the most common complaints experienced during the third trimester of pregnancy as a result of anatomical, hormonal, and biomechanical changes that occur throughout pregnancy. Increased maternal body weight and uterine enlargement place greater pressure on the lumbar region, which may reduce comfort, interfere with daily activities, and affect the mother's quality of rest. One of the non-pharmacological interventions that can be applied to manage this complaint is prenatal yoga. This final project aimed to provide midwifery care for a third-trimester pregnant woman experiencing low back pain through the implementation of prenatal yoga. This report employed a case study design using a midwifery care management approach, including assessment, diagnosis, planning, implementation, and evaluation. The subject of the case study was Mrs. L, a 26-year-old G2P1A0 woman at 33 weeks of gestation, whose primary complaint was low back pain. Midwifery care was provided over three visits from June to July 2026. The interventions included health education, prenatal yoga performed 2–3 times per week for 30–60 minutes per session, warm compresses using a hot water bag, the use of a maternity support belt, advice to sleep in the left lateral position, postural correction, restriction of strenuous activities, adequate nutritional intake, sufficient rest, and continuous monitoring of both maternal and fetal conditions. Following the interventions, both the mother and the fetus remained in good condition. Assessment of pain intensity using the Numeric Rating Scale (NRS) demonstrated a decrease from a score of 5 at the first visit to 3 at the second visit and 2 at the third visit. In addition, the mother reported greater comfort in performing daily activities and was compliant with all recommendations provided. Regular prenatal yoga combined with education on daily activity modification, proper body posture, adequate rest, and a healthy lifestyle was shown to reduce the intensity of low back pain in third-trimester pregnant women. Therefore, prenatal yoga can be considered a safe and effective non-pharmacological intervention in midwifery care for pregnant women experiencing low back pain.
