Abstract

Operational safety of ships is a crucial aspect in shipping to prevent accidents. One of the early indicators of potential hazards is near miss, an incident that almost causes an accident but does not result in any loss. This study aims to analyze the operational safety risks of the KM. Meratus Kahayan based on near miss data using the Formal Safety Assessment (FSA) approach. The FSA method was chosen because it is able to systematically identify hazards, assess risks, and formulate control recommendations in accordance with IMO standards. This study uses a qualitative approach with data collection techniques through observation, interviews, and documentation of near miss reports on KM. Meratus Kahayan. The data obtained were then analyzed through the 5 stages of FSA: hazard identification, risk assessment, identification of risk control options, cost-benefit analysis, and decision recommendations. The results showed that the main causes of near miss were human factors, non-optimal work procedures, and the working environment conditions on board. The highest risks were found in cargo handling and navigation activities. Based on the analysis, it is recommended to enhance safety training for the crew, evaluate and improve standard operating procedures, and strengthen the near miss reporting culture as an effort to prevent accidents. The conclusion of this study is that the implementation of FSA based on near miss data is effective in identifying and controlling ship operational safety risks. The recommendations are expected to improve shipping safety and support the establishment of a better safety management system on KM. Meratus Kahayan.
