Digital transformation at the village level is not limited to population administration; it can also improve services directly related to community safety. In Bangbayang Village, Cipaku District, Ciamis Regency, referral recording and village ambulance requests still rely on personal communication by telephone or instant messages. This approach makes it difficult for village officers to consistently monitor queues, service histories, driver assignments, and completion status. This study aims to analyze requirements and design SIMANGPRANG as a mobile-based village health service application. A descriptive approach was employed through observation, interviews, and documentation during field practice. The system design followed a prototyping method consisting of requirements communication, quick planning, quick design, prototype construction, and design delivery for feedback. The system was modeled using Unified Modeling Language, including use case, activity, sequence, and class diagrams. The study produced a design involving three primary roles: residents or patients, ambulance drivers, and village administrators. The proposed features include registration, ambulance requests, referral-document upload, status monitoring, task notifications, driver-patient communication, completion reports, account administration, and report export. The SIMANGPRANG design clarifies the service workflow, establishes a more orderly data trail, and provides a technical foundation for application development. This research is limited to the analysis and design stages; therefore, system effectiveness must be validated through implementation and field testing.
