Abstract

Kidney failure in children is a serious condition requiring renal replacement therapy such as hemodialysis and is associated with high mortality rates. Several clinical factors, including age, nutritional status, anemia, hypertension, and hyperkalemia, are suspected to contribute to mortality. However, data regarding mortality-related factors among pediatric patients undergoing hemodialysis in Indonesia remain limited. This study aimed to identify factors associated with mortality in pediatric patients undergoing hemodialysis. This research used an analytic observational design with a cross-sectional approach. Data were obtained from medical records of pediatric patients aged 0–18 years who underwent hemodialysis from January 2022 to December 2025. A total of 66 patients met the inclusion and exclusion criteria. Data were analyzed using univariate and bivariate analysis with chi-square/Fisher exact tests and odds ratio (OR) analysis. Most patients were male (54.5%) and aged 13–18 years (57.6%). Hypertension (p = 0.012), hyperkalemia (p = 0.002), and nutritional status based on height-for-age (p = 0.040) were significantly associated with mortality. Hyperkalemia was the most dominant factor, increasing mortality risk by 10.2 times. Early detection and optimal management are important to reduce mortality risk in children with kidney failure.
