Abstract

This study employs the Failure Mode and Effects Analysis (FMEA) method to examine the reasons behind the decrease in Crude Palm Oil (CPO) productivity and quality at the Adolina Plantation of PTPN IV. Plantation production data for the previous four years and CPO quality data based on impurity content metrics are among the data used. The study's findings show that, with a drop of 0.95 tons/ha/year, or 3.97%, the average realized productivity of 22.96 tons/ha/year is still below the RKAP target of 23.90 tons/ha/year. Several divisions, particularly Division 5, saw the most decline. With a standard deviation of 0.0107, the average CPO impurity level was 0.023% with a range of 0.01%–0.04%, which still satisfies quality standards but exhibits variability. According to the FMEA analysis, delays in processing Fresh Fruit Bunches (FFB), with a Risk Priority Number (RPN) of 315, and inefficient fruit sorting, with an RPN of 240, were the primary causes of the drop in CPO output and quality. The findings demonstrated that initial processing and raw material management were more of the primary issues than machine technical issues. It is anticipated that this research would give businesses a foundation for deciding on corrective actions to maximize CPO productivity and quality.
