Abstract

Chronic diseases such as type 2 diabetes mellitus, hypertension, asthma, and chronic kidney disease require long-term therapy that often involves multiple medications, increasing the risk of Drug Related Problems (DRPs). This review article summarizes patterns of DRPs and the contributing factors based on recent studies. The findings indicate that common DRPs include inappropriate dosing, drug interactions, drug use without indication, untreated indications, and ineffective therapy. In chronic kidney disease, DRPs frequently arise from inadequate dose adjustment in response to declining renal function, while in asthma, suboptimal medication use and incorrect inhaler technique are dominant causes. Overall, DRPs negatively affect therapeutic outcomes, increase the incidence of adverse effects, and elevate the risk of complications. Therefore, pharmacist-led interventions such as regular medication reviews, counseling, dose adjustment, and patient education are essential to prevent DRPs and improve treatment safety and effectiveness in chronic disease management.
