The management of personnel administration in primary healthcare facilities such as Community Health Centers (Puskesmas) is still largely carried out manually, leading to various obstacles such as unstructured recording, difficulties in data retrieval, and the risk of document loss. This study aims to design and develop a web-based employee data recording information system using the prototype method at the Kubang Jaya Community Health Center (UPT Puskesmas). The background of this study is that employee data recording and personnel document management are still carried out manually, which creates obstacles in data retrieval, information updates, archive storage, and the preparation of personnel administration reports. This study employed the Research and Development (R&D) method, utilizing data collection techniques such as observation, interviews, and documentation. The development stages included needs analysis, prototype creation, prototype evaluation and refinement, system implementation, and system testing. Testing was conducted through functional testing on both the administrator and employee sides, as well as a feasibility test using a Guttman scale questionnaire administered to five respondents. The test results showed that all system functions operated as expected, and the feasibility test yielded a score of 96.67%, falling into the “highly feasible” category. Thus, the developed information system is deemed capable of supporting the management of employee data and personnel documents in a more effective, efficient, and integrated manner at the Kubang Jaya Community Health Center (UPT Puskesmas).
