Abstract

Nephrolithiasis is a urinary tract disorder with increasing prevalence, driving the utilization of natural ingredients such as meniran herb (Phyllanthus niruri L.) as a complementary therapy. This study aims to examine and map the pharmacological potential and main secondary metabolite profile of meniran herb as a complementary therapy for nephrolithiasis based on pharmaceutical e-journals. The research method used was a retrospective descriptive study employing a secondary data approach on a sample of 60 articles from pharmaceutical e-journals publication period April 2021 and May 2026.The results showed that the pharmacological activity profile was dominated by antilithiatic activity reported in 37 articles (61.67%), followed by antioxidant activities in 31 articles (51.67%), and diuretic activity in 15 articles (25.00%). The primary secondary metabolites identified were flavonoids in 32 articles (53.33%), tannins and phenolics in 28 articles (46.67%), alkaloids in 24 articles (40.00%), and lignans (phyllanthin and hypophyllanthin) in 21 articles (35.00%). In conclusion, Meniran Herb exhibits promising pharmacological potential as a complementary therapy for nephrolithiasis through synergistic, multifaceted mechanisms (whole-herb approach) driven by its diverse secondary metabolite constituents.

Abstract

Type 2 diabetes mellitus is a chronic disease that requires long-term treatment; therefore, the rational use of oral antidiabetic drugs is important to achieve therapeutic outcomes and prevent complications. This study aimed to evaluate the rationality of oral antidiabetic drug use in patients with type 2 diabetes mellitus at Bane Community Health Center, Pematang Siantar, in 2025 based on the indicators of right patient, right indication, right drug, right dose, and right duration of administration. This study was a non-experimental study using a descriptive-evaluative method with a retrospective approach. The sample consisted of 70 medical records of patients with type 2 diabetes mellitus who received oral antidiabetic therapy during 2025. Data were obtained from medical records and compared with the therapeutic guidelines used in the study. Data were analyzed descriptively using frequency distributions and percentages. The results showed that glimepiride was the most frequently used drug, prescribed to 57 patients (81%), while metformin was used by 37 patients (53%). A total of 46 patients (66%) received monotherapy and 24 patients (34%) received combination therapy. The evaluation showed that the right patient, right indication, right drug, right dose, and right duration of administration were each achieved by 70 patients (100%). Therefore, the use of oral antidiabetic drugs in patients with type 2 diabetes mellitus based on the five evaluated indicators was considered rational.
