Abstract

Objective: To evaluate the stone free rates of kidney and ureter stone patients managed by Extracorporeal Shockwave Lithotripsy (ESWL), and the ureter stone free rate managed by ureterolithotripsy. Material & method: We reviewed the medical records of kidney and ureter stone patients managed by ESWL and ureter stone patients managed by ureterolithotripsy in Arifin Achmad Regional General Hospital Pekanbaru Riau Province, Indonesia, from January 2010 - December 2016. ESWL and ureterolithotripsy stone free rates were examined by the control of KUB röngents. Results: There were 891 kidney and ureter stone patients consisting of 325 (36.5%) were kidney stone patients and 566 (63.5%) were ureter stone patients. The pyelum stones were the most (78.2%) in kidney stones and the lower ureter stones were the most (57.2%) in ureter stones. There were more male patients than the female ones in which most of the patients were in the group age of 49-59 years. The amount of patients increased each year. The kidney stone free rate managed by ESWL was lower (71.7%) than ureter stone free rate (84.1%) by ESWL, while the stone free rate of ureter stone patients managed by ureterolithotripsy was 100%. Conclusion: The ESWL stone free rate of the kidney stone patients was lower than the one in ureter stone patients while the ureterolithotripsy stone free rate was 100%.

Abstract

Objective: To evaluate the profiles of urinalysis in kidney, ureter, and bladder stone patients. Material & Methods: We reviewed the medical records of kidney, ureter and bladder stone patients proved underwent operative managements and urinalysis in Arifin Achmad Regional General Hospital Pekanbaru Riau Province, Indonesia in 2011-2015. The sampling technique in this study was minimum sampling that meets the inclusion criteria. Univariate test was used for statistical analysis. Approval on the study was obtained from the Ethical Review Board for Medicine and Health Research, Medical Faculty, University of Riau. Results: There were 226 patients fulfilled the inclusion criteria. This study showed that in macroscopic urinalysis the mostly (94.7%) urine density was 1.003-1.030, urine color mostly (46%) was turbid yellow, all (100%) urine acidity showed normal results with mean acidity was 6, most (95.1%) erythrocytes >1 in urine sediment, most (66.2%) leukocytes was >5 hpf, mostly (99.1%) crystal was absent and most urinary epithelium <15 (63.1%). In microscopic urinalysis showed mostly  protein in urine was absent (56.4%), and mostly ketone (98.7%) was absent in urine and mostly (56.2%) was absent protein in urine. Conclusion: Urinalysis findings could be normal or abnormal in kidney, ureter and bladder stone patients.

Abstract

Objective: To evaluate the success rates of tamsulosin and dutasteride as medical therapy based on international prostate symptom score (IPSS) in symptomatic benign prostatic hyperplasia (BPH) patients. Materials & Methods: We reviewed medical records of the symptomatic BPH patients in Arifin Achmad Regional General Hospital, Pekanbaru, Riau Province, Indonesia in 2011-2016. The previous and after IPSS of administering tamsulosin 0.2 mg or 0.4 mg or dutasteride 0.5 mg as a single therapy and tamsulosin 0.4 mg with dutasteride 0.5 mg as a combination therapy were assessed. Wilcoxon test was used for statistical analysis. Approval on the study was obtained from the Ethical Review Board for Medicine and Health Research, Medical Faculty, University of Riau. Results: 100 patients fulfilled the inclusion criteria. The results showed 50% patients with mild IPSS and 50% patients with severe IPSS before the medication. 70% patients administered tamsulosin 0.2 mg as first-line therapy, 35% patients administered medication for 6 months-1 year, and 3% patients with mild IPSS, 44% with moderate IPSS and 53% with severe IPSS after the medication. The Wilcoxon test showed there was a significant different (p<0.005) between IPSS levels before and after the medication with success rate 18%. Conclusion: There were good success rate of tamsulosin 0.2 mg, tamsulosin 0.4 mg, dutasteride 0.5 mg as single therapy and tamsulosin 0.4 mg with dutasteride 0.5 mg as combination therapy in symptomatic BPH patients based on IPSS, and the medical therapy are still recommended.

Abstract

Objective: To evaluate the characteristic and the management of Fournier’s gangrene. Material & Methods: We reviewed medical records of Fournier’s gangrene patients including age, etiology, invasion of bacteria, underlying disorders, clinical symptom, radiology and laboratory findings, management, and complication in Arifin Achmad General Hospital, Pekanbaru, Riau Province, in January 2012–December 2017. Statistical analysis of univariate was used. Approval of the study was obtained from the Ethical Review Board for Medicine and Health Research, Medical Faculty, University of Riau. Results: There were 19 patients in the study. The result showed that Fournier’s gangrene cases occurred mostly in age 56-65 year old age group in 8 (42.1%) patients. The etiology was from urogenital cause in 15 (78.9%) patients. Most invasion bacteria was Acinetobacter baumannii in 5 (26.3% ) patients. The most underlying disorder was elderly age in 10 (5.6%) patients. The most frequent symptom was puss on the scrotal in 10 (52.6%)patients. The most laboratory examination was puss bacterial culture in 14 (73.7%) patients. The most frequent treatment for Fournier’s gangrene was initial and preoperative management in 19 (100%) patients. The acute complication of Fournier’s gangrene was sepsis in 5 (26.3%) patients. Conclusion: Fournier’s gangrene occurred mostly in elderly age in which Acinetobacter baumannii was the most invasion bacteria and sepsis was the most complication.

Abstract

Objective: This study aims to describe the risk factors of bladder cancer patients in Arifin Achmad Regional General Hospital, Riau Province, Indonesia. Material & Methods: We reviewed the medical records of bladder cancer patients managed in Arifin Achmad Regional General Hospital from January 2013 - December 2017. The data collected were sex, age, histopathology types, staging, smoking history, occupation, ethnic/tribes, and city/district origine. Statistical analysis of univariate was used. Approval on the study was obtained from the Ethical Review Board for Medicine and Health Research, Medical Faculty, University of Riau. Results: There were 101 bladder cancer patients consisting of 83.2% patients with transitional cell carcinoma and 65.3% with advanced stage. The result showed the most common (82.2%) patients were males and 35.6% of patients were in age 51-60 year old group, with the youngest was in 9 year old group and the oldest is 85 years. The most common risk factors of bladder cancer patient were farmers in 29.7% and the non-smoking patients were in 52.5%, Malay ethnic in 30.7% and Pekanbaru origin in 30.7% of patients. Conclusion: Bladder cancer risk factors in Arifin Achmad Regional General Hospital were males, farmers, Malay ethnic, and Pekanbaru origin.

Abstract

Objective: To find out the description of the characteristics of Priapism patients. Material & Methods: This research used a total priapism patients in Arifin Achmad Hospital, Riau Province from January 2023 to February 2023. Results: From 7 patients with priapism who met the inclusion and exclusion criteria, most age group were from the 36-45 year age group in 3 (42.9%) patients. History of disease in patients with most priapism was leukemia in 3(42.9%) patients. Conclusion: Classification of ischemic priapism were in 4 (57.1%) patients and non-ischemic priapism was 3 (42.9%) patients and all management of priapism patients was carried out by surgery (100%). Abstract

Objective: This study aims to identify the characteristics of GPPPD in urologic patients. Material & Methods: The research design used descriptive cross-sectional study conducted at urology clinic patients of Arifin Achmad Hospital, Pekanbaru, Riau Province in October 2021 to January 2022 The patients were interviewed based on validated questionaire. The data collected were urology diseases and disorders, age, symptoms, risk factors of biology, interpersonal, psychological, socio-cultural, sexual satisfaction. The study had been approved by ethical committee of Medical Faculty, Riau University. Univariate analysis were used. Results: There were 22 patients in the study with most urology disease were urinary tract infection (54.5%), while the most common age group was 41-60 years(77.3%) and the common risk factors were biological (41%), psychological (13.6%) and socio-cultural ones (100%). Conclusion: The characteristics of GPPPD in urologic patients were mostly with UTI, age group of 41-60 years and socio-cultural factors. Keywords: GPPPD, urology, urinary tract infection.
