Abstract

Objective: To analyze the distribution features of Gleason score in the different prostate zones. Material & Method: Twenty paraffin block specimens of radical prostatectomy were analyzed looking for Gleason score distribution in each zone. Specimens were stained with Hematoxilin Eosin. Results: Among the 20 cancers, 16 (80%) specimens had more than one focus. Most foci were found in peripheral zone (95%), only one specimen contained foci solely found in transitional zone. More than half (55%) prostate specimens contain three different Gleason grades. Only one specimen contained a single grade. Conclusion: Our results confirm the heterogeneous, multifocal, and multizonal nature of prostate carcinomas. Most specimens had more than one tumor focus, with predominant location in the peripheral zone. Almost all transition zone foci were found concomitantly with peripheral zone foci. Abstract

Objective: To evaluate the characteristic and the management of Fournier’s gangrene. Material & Methods: We reviewed medical records of Fournier’s gangrene patients including age, etiology, invasion of bacteria, underlying disorders, clinical symptom, radiology and laboratory findings, management, and complication in Arifin Achmad General Hospital, Pekanbaru, Riau Province, in January 2012–December 2017. Statistical analysis of univariate was used. Approval of the study was obtained from the Ethical Review Board for Medicine and Health Research, Medical Faculty, University of Riau. Results: There were 19 patients in the study. The result showed that Fournier’s gangrene cases occurred mostly in age 56-65 year old age group in 8 (42.1%) patients. The etiology was from urogenital cause in 15 (78.9%) patients. Most invasion bacteria was Acinetobacter baumannii in 5 (26.3% ) patients. The most underlying disorder was elderly age in 10 (5.6%) patients. The most frequent symptom was puss on the scrotal in 10 (52.6%)patients. The most laboratory examination was puss bacterial culture in 14 (73.7%) patients. The most frequent treatment for Fournier’s gangrene was initial and preoperative management in 19 (100%) patients. The acute complication of Fournier’s gangrene was sepsis in 5 (26.3%) patients. Conclusion: Fournier’s gangrene occurred mostly in elderly age in which Acinetobacter baumannii was the most invasion bacteria and sepsis was the most complication.

Abstract

Urinary tract infection (UTI) and urinary tract calculi (UTC) still causes significant health problem and present as a dominant case in urology. Urine culture is a standard examination prior to UTC surgery which provide information on microorganism and its sensitivity to antibiotics. Despite of appropriate preoperative antibiotic therapy, we still found post operative infections among those patients. This study aims to determine if there any difference between urine and calculi culture in patients underwent UTC surgery. This is a comparative observational study in patients underwent urinary stone removal procedures from April 2022 to August 2022. Urinary culture attained 5-7 days prior to procedure and crushed calculi culture directly after procedure. The comparison between two examinations was performed. Total of 50 cases consisting of 33 (66%) males and 17 (34%) females was included. Patient with positive urine culture and calculi culture was 15 (30%) and 14 (28%) respectively. Only 3 (6%) patients positive for both, and only one patient with same pathogen. In 11 (22%) patients, we found positive calculi culture with previously sterile urine culture. Most common pathogen in urine culture was Escherichia coli, and Klebsiella pneumoniae found mostly in calculi culture. This study showed 21% accuracy, 66,67% specificity, 20% positive prediction value, 68,57% negative prediction value and 54% diagnostic accuracy. Low positive prediction value and diagnostic accuracy for microorganism of preoperative urine cultured was shown in this study. As a consequences, calculi culture is required  as a routine examination during stone removal procedures.

Abstract

Background: Gastric duplication (GD) is a rare congenital anomaly, representing 2–9% of all gastrointestinal duplications. These embryologic abnormalities are typically managed by complete surgical resection due to the risk of complications, including malignancy. However, this standard approach becomes perilous when the duplication cyst is intimately adherent to vital organs. We present a case where a large gastric duplication cyst was inseparable from the pancreas, necessitating a deviation from standard management. Case presentation: An 8-month-old female infant presented with a four-month history of non-bilious vomiting and progressive abdominal distension. A palpable, cystic, 8x5 cm mass was identified in the left upper abdomen. Abdominal ultrasound revealed a loculated, septated cystic lesion, and a barium study demonstrated a significant filling defect on the greater curvature of the stomach. Initial management was delayed as the family sought alternative medicine. Surgical exploration revealed a large gastric duplication cyst arising from the greater curvature, which was found to be densely adherent to the body and tail of the pancreas. To avoid catastrophic pancreatic injury, a complete resection was abandoned in favor of a pancreas-sparing mucosectomy. The entire mucosal lining of the duplication was excised, and the shared muscular wall was preserved and repaired. Postoperatively, the patient had a transient ileus but recovered well, with complete resolution of symptoms. At an 11-day follow-up, she was thriving, feeding well, and had gained significant weight. Histopathology confirmed a benign gastric duplication cyst. Conclusion: This case highlights that for complex gastric duplication cysts where resection would endanger vital structures, complete mucosal excision is a safe, effective, and organ-preserving surgical alternative. This technique successfully mitigates the risks of both the untreated anomaly and iatrogenic surgical complications, underscoring the importance of surgical judgment and adaptability in managing rare congenital anomalies.

Abstract

Objective: To find out the description of the characteristics of Priapism patients. Material & Methods: This research used a total priapism patients in Arifin Achmad Hospital, Riau Province from January 2023 to February 2023. Results: From 7 patients with priapism who met the inclusion and exclusion criteria, most age group were from the 36-45 year age group in 3 (42.9%) patients. History of disease in patients with most priapism was leukemia in 3(42.9%) patients. Conclusion: Classification of ischemic priapism were in 4 (57.1%) patients and non-ischemic priapism was 3 (42.9%) patients and all management of priapism patients was carried out by surgery (100%). Abstract

Objective: This study aims to identify the characteristics of GPPPD in urologic patients. Material & Methods: The research design used descriptive cross-sectional study conducted at urology clinic patients of Arifin Achmad Hospital, Pekanbaru, Riau Province in October 2021 to January 2022 The patients were interviewed based on validated questionaire. The data collected were urology diseases and disorders, age, symptoms, risk factors of biology, interpersonal, psychological, socio-cultural, sexual satisfaction. The study had been approved by ethical committee of Medical Faculty, Riau University. Univariate analysis were used. Results: There were 22 patients in the study with most urology disease were urinary tract infection (54.5%), while the most common age group was 41-60 years(77.3%) and the common risk factors were biological (41%), psychological (13.6%) and socio-cultural ones (100%). Conclusion: The characteristics of GPPPD in urologic patients were mostly with UTI, age group of 41-60 years and socio-cultural factors. Keywords: GPPPD, urology, urinary tract infection.

Abstract

This study aimed to analyze the effect of age variation on testicular morphometry and spermatozoa motility in cats. A total of 21 male cats were grouped into three age categories (< 1 year, 1–3 years, and 3 years). The testes were collected through an open castration procedure, and their morphometric parameters (length, width, and circumference) were measured. Semen samples were obtained by incising the cauda epididymis of each testis to evaluate its motility level under a light microscope. Data were analyzed using one-way Analysis of Variance (ANOVA), followed by the Tukey test to identify differences between groups. The results showed that age variation did not cause a significant difference in the morphometric characteristics of the collected testes (P>0.05). However, a significant difference was found in spermatozoa motility levels among the different age groups. It is concluded that age does not affect the physical dimensions of the testis but has a significant influence on the functional quality of spermatozoa. The 1–3 year-old group showed the optimal level of spermatozoa motility compared to the other age groups in this study

Abstract

This community service activity aims to assess the feasibility and impact of the community-based HHUS tele-mentoring program for urological screening in rural primary healthcare centers in Indonesia. A prospective quasi-experimental study was conducted from August to October 2025 at Pangkalan Baru Primary Health Center, Riau, Indonesia. Four general practitioners (GPs) underwent a one-day HHUS training session, followed by one month of tele-mentoring using the UroSono Mentor app, a telemedicine platform that supports both real-time video consultation and asynchronous image review, and one additional month of independent practice. Confidence, usability, feasibility, mentor responsiveness, patient satisfaction, and behavioral intention were assessed using structured questionnaires. Data were analyzed using the Friedman test (p < 0.05). Confidence scores improved significantly across all phases (p = 0.018), increasing from 2.06 (pre-training) to 4.37 (tele-mentoring) and 3.81 (independent practice). Training satisfaction was high (3.81 ± 0.19), with positive ratings for mentor responsiveness (4.44 ± 0.24) and usability (4.13 ± 0.14). Patient satisfaction averaged 4.49 ± 0.54. Of the 26 patients examined, 69.2% had normal findings, while 30.8% required referral. Community-based tele-mentored HHUS effectively enhanced GPs’ competence and confidence, achieving strong acceptance among both healthcare providers and patients. The UroSono Mentor model shows promise for scalable integration into rural urological screening programs.
