Abstract

Objective: To compare the sensitivity and specificity of both types of prostate biopsies, between 6 core biopsy/sextant biopsy with volume based biopsy prostate. Material & method: The subjects were patients that have been performed prostate biopsy in Hasan Sadikin Hospital in 2006 – 2010. The data was divided into two groups, the group A (before 2009) 327 patients had performed 6 cores biopsy, group B (after 2009), the biopsy was performed based on prostate volume (volume < 40 cc : 8 cores, volume 40-60 cc : 10 coresand volume > 60 cc : 12 cores). Biopsy results confirmed by definitive results from surgery, then analyzed specificity and sensitivity between two groups. The analytical statistic test using unpaired T test and Levene’s test. Results: There were 654 patients divided in two groups. The data in both groups were similar based on analytical statistic test using unpaired t test (p = 0.28) and the data was in normal distribution (Levene’s test = p > 0.05). The group A, mean age 67.4 years, mean prostate volume 32.53 cc and mean PSA levels 29.89 ng/dl and at group B, mean age 66.7 years, mean prostate volume 30.89 cc and mean PSA levels 16.92 ng/dl. Volume based cores method in prostate biopsy have higher sensitivity and specificity compared with 6 core biopsy (97.5% vs 94.0%) and (92.2% vs 77.8%). Conclusion: The sensitivity and specificity of prostate biopsy was increased in volume based cores compared to 6 cores biopsy. Keywords: Prostate biopsy, volume based.

Abstract

Objective: To determine the incidence of squamous cell carcinoma (SCC) in patients with large bladder stone (> 40 mm). Material & method: This is a descriptive retrospective cross-sectional study, data was collected from medical record on patients with a diagnosis of large bladder stone (> 40 mm) that have been performed surgical removal of stone (vesicolithotomy) and bladder mucosa biopsy in Hasan Sadikin Hospital Bandung during 2006-2010. Furthermore, the data are classified according to pathology. Results: This study found cases of large bladder stone (> 40 mm) of 47 cases during the period 2006-2010, and of these 47 cases, there were 76.7% and 23.3% non-malignancy of a malignancy in 2 cases (4.3%) with anatomical pathology picture of SCC. Based on the age group of patients aged 31-40 years obtained 25.5%, 41-50 years 21.3%, 51-60 years 21.3% and > 60 years 23.4%, whereas for ages below 30 years 8.5%. Based on sex, male 95.7% and 4.3% of women. Based on the size, found a stone the size of 40 mm (48.9%), stone size of 50 mm (10.6%), stone size 60 mm and 70 mm respectively (8.5%), stone size 80 mm (12.8%), stone size > 80 mm (4.3%) and 6.4% with multiple bladder stones. Conclusion: There were only two cases of bladder SCC from 47 cases patients with large bladder stone (> 40 mm) in Hasan Sadikin Hospital Bandung during 2006-2010. Abstract

Objective: To assess the sensitivity and specificity of ultrasound guided fine needle aspiration biopsy (FNAB) in renal tumors.Methods: A diagnostic study was performed on 23 patients with renal tumors who visited the Urology Division, Department of Surgery, Dr. Hasan Sadikin General Hospital from January 2011 to August 2012. First time ultrasound guided was performed to the patients before nephrectomy. Renal tissues that were obtained from both procedures were examined histopathologically, and the result from nephrectomy was used as the gold standard. Analysis was conducted by measuring the sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of the ultrasound guided FNAB. Results: The results showed that the sensitivity value of ultrasound guided FNAB was 85.71%, with 50% specificity, 94.74% PPV and 25% NPV. From 23 patients, only 2 patients had hematoma. Conclusions: Ultrasound guided FNAB is sufficiently sensitive with a fair specificity for diagnosing renal tumors and is safe for patients.Keywords: Renal tumor, sensitivity, specificity, ultasound guided FNAB DOI: 10.15850/ijihs.v3n1.401

Abstract

The goal of Benign Prostate Hyperplasia (BPH) therapy is to relieve symptoms and maintain disease progression as well as the side effects of drugs and their benefits for improving the quality of life and sexual quality. This study aimed to assess the effect of tamsulosin therapy with combination therapy of tamsulosin and dutasteride on the erection hardness score (EHS) in men with LUTS due to BPH. This study is a cross-sectional study which assesses and compares differences in erection hardness scores (EHS) of patients with benign prostatic hyperplasia (BPH) before and after tamsulosin therapy and the combination of tamsulosin + dutasteride. After three months of therapy, the group of subjects given tamsulosin had an IPSS score of 12.87 (p=0.00), IIEF-5 15.43 (p=0.51), EHS 2.6 (p=0.08), while the tamsulosin + dutasteride group had an IPSS score 12.53(p=0.00), IIEF-5 15.83 (p=0.02), and EHS 2.6 (p=0.01). The decrease in EHS was more statistically significant in treatment with tamsulosin + dutasteride combination therapy compared with tamsulosin monotherapy, this was due to the effect of each drug combined.

Abstract

Cases of hypospadias were found to vary in specific periods, but There is currently no data on hypospadias cases handled at the Arifin Achmad Riau general hospital. Therefore, the researchers conducted this study which is a descriptive retrospective study, by taking data from the medical records of hypospadias patients at Arifin Achmad Hospital, Riau Province, for four years from 2017 to 2021. The data taken included the patient's age, type of hypospadias, whether there is a penis chordee, and the surgical technique performed. From the results of this study, there were 60 cases of hypospadias for four years (2017-2021) with a mean age of 11.73 (plus minus 11). Of the 60 hypospadias cases, 8 cases of anterior hypospadias (13.3%) were found. There were 26 cases (43.33%) of middle hypospadias. There were 24 cases (40%) of posterior hypospadias. There were 2 cases with cripples hypospadias (3.34%). The surgical technique performed is also different for each type. The surgical methods performed include Urethroplasty with Mathieu technique in 8 cases (13.33%), Urethroplasty with TIP technique in 9 (15%), Urethroplasty with SLAM technique in 16 (26,66%), Stage urethroplasty in 17 (28,33%), other procedure in 10 (16,67%). Prospective studies need to be carried out to compare the success rates of each surgical procedure related to the type of hypospadias and the degree of chordee.
