Abstract

Infeksi kecacingan yang disebabkan oleh soil transmitted helminth (STH) dapat mengakibatkan gangguan gizi dan menurunkan kualitas sumber daya manusia. Imunitas terhadap infeksi cacing mengaktifkan respons Th2 yang ditandai oleh peningkatan kadar interleukin-4. Penelitian ini bertujuan mengetahui kadar IL-4 pada anak kecacingan dan yang tidak kecacingan serta menganalisis validitas pemeriksaan IL-4 sebagai indikator infeksi kecacingan. Sebanyak 74 sampel plasma EDTA diambil dari anak-anak kelas 1–3 SD di Kecamatan Jatinangor kemudian dilakukan pemeriksaan kadar IL-4 dengan metode ELISA. Hasil penelitian didapatkan kadar IL-4 pada subjek yang terinfeksi kecacingan lebih tinggi secara bermakna dibanding dengan subjek yang tidak terinfeksi (3,001 pg/mL berbanding 1,406 pg/mL ; p<0,001). Pada cut-off point 1,585 pg/mL, kadar IL-4 memiliki sensitivitas 66,7%, spesifisitas 65,9%, dan akurasi 66,2%. validitas instrumen pemeriksaan telur cacing berdasarkan pemeriksaan kadar Interleukin-4 termasuk kategori cukup dengan koefisien Κ (Kappa) sebesar 0,635. Kadar IL-4 pada subjek yang terinfeksi kecacingan meningkat 1,6 kali lebih tinggi dibanding dengan subjek yang tidak terinfeksi kecacingan. Simpulan, IL-4 merupakan pemeriksaan yang valid digunakan sebagai diagnostik infeksi kecacingan. Helminth infections are potent inducers of Th2 type responses as well as increased secretion of interleukin-4. The purpose of this study was to determine the interleukin-4 concentration in subjects with helminth infection and without helminth infection and to assess the validity of the plasma Interleukin-4 concentration assessment in helminth infection patients. This study examined 74 plasma samples from 1st-3rd grade elementary students in Jatinangor district using ELISA method. The results indicated that IL-4 concentration of helminth infection subjects was significantly higher compared to that of non-infected subjects (3.001 pg/mL vs 1.406 pg/mL, p<0.001). A cut-off point of 1.585 pg/mL for IL-4 concentration means 66.7% sensitivity, 65.9% specificity, and 66.2% accuracy . The validity of the helminth ova assessment instrument based on Interleukin-4 level testing is fair with a K (Kappa) coefficient of 0.635. The IL-4 level in helminth infection patient increased 1.6 times compared to non-infected subjects. In conclusion, IL-4 is a valid marker for diagnosing helminthiasis. Abstract

Metatarsus aduktus adalah salah satu kelainan bentuk kaki yang paling umum dengan insidensi sekitar 3%. Pola cedera pada kelainan ini belum dijelaskan sebelumnya. Penelitian ini akan mengamati pola fraktur pada kaki dengan metatarsus aduktus. Metatarsus aduktus ditentukan secara radiologis menggunakan sudut metatarsus aduktus pada kelompok trauma kaki yang disebabkan kecelakaan sepeda motor di UGD rumah sakit Hasan Sadikin Januari 2014 - Desember 2018. Dari 135 kasus trauma kaki, metatarsus aduktus ditemukan pada 14 pasien (10,7%). Dominan terjadi pada pria (70,1%). Fraktur soliter terjadi pada 8 pasien, fraktur multipel pada 6 pasien, dengan sebagian besar tulang yang terlibat adalah metatarsal keempat (n = 9). Fraktur berhubungan dengan peningkatan konsentrasi beban pada suatu tempat. Metatarsus aduktus menyebabkan beban yang lebih tinggi di lateral metatarsal yang meningkatkan risiko terjadinya fraktur. Perubahan anatomis pada metatarsus aduktus berhubungan dengan perubahan biomekanik kaki, menunjukan kesesuaian pola fraktur di area lateral metatarsal pada kasus trauma kaki.

Abstract

Trauma adalah penyebab kematian utama pada usia di bawah 44 tahun di Amerika Serikat (AS). Di Indonesia, trauma menjadi penyebab kematian utama pada kelompok umur 15 – 24 tahun, dan nomor 2 pada kelompok usia 25 – 34 tahun. Penyebab umumnya ialah kecelakaan lalulintas, diikuti jatuh dari ketinggian, luka bakar dan karena kesengajaan (usaha pembunuhan atau kekerasan lain dan bunuh diri). Salah satu perintis pelayanan kedaruratan medik termasuk kasus trauma adalah Dr. Adams R. Cowley, dari beliau muncul konsep “The golden hour”. Pelatihan Advanced Trauma Life Support(ATLS) dimulai pada tahun 1980 di Alabama, AS, dan atas prakarsa Dr. Aryono D. Pusponegoro, Ketua Komisi Trauma IKABI pusat, mulai 1995 kursus ATLS terselenggara di Indonesia. Penelitian ini dilakukan secara retrospektif dalam kurun waktu Januari sampai Juli 2014 dengan jumlah pasien meninggal di instalasi gawat darurat bedah Rumah Sakit Hasan Sadikin Bandung sebanyak 58 pasien. Melalui penelitian ini akan ditelusuri penyebab kematian dilihat dari segi pertolongan pertama ketika pasien datang ke instalasi gawat darurat, dengan mengacu kepada prosedur Advanced Trauma Life Support (ATLS) yang biasa diterapkan. Hasilnya, pasien meninggal di instalasi gawat darurat bedah Rumah Sakit Hasan Sadikin Bandung dari Januari sampai Juli 2014 sebanyak 58 pasien, sebanyak 6 pasien (10,34%) meninggal pada satu jam pertama, 12 pasien (20,68%) meninggal pada satu sampai enam jam pertama. Dinilai dari segi prosedur Advanced Trauma Life Support (ATLS), mayoritas mengalami kegagalan pada tahap disability (D), yaitu sebanyak 41 pasien meninggal (70,06%), pada tahap circulation (C) sebanyak 10 pasien (17,24%), pada tahap breathing (B) sebanyak 6 pasien (10,34%) dan tahap airway (A) sebanyak 1 pasien (1,72%).

Abstract

The adolescent’s reproductive health is the major problem that should take into consideration. To solve this problem, it needs an alternative strategy by using a media, reproductive health/kesehatan reproduksi (KEPO) game, in Android smartphone. The aim is analyzing the effect of KEPO game on the five dimensions of female adolescent’s satisfaction. The research method was quasi-experiment, and the research design used pre-test post-test with control group design. The sampling technique used simple random sampling. The subject was 64 respondents of female student age of 12−15 years old in public junior high school in Bandung city. The respondents divided into two groups, the first one treated by KEPO game is 32 students of Public Junior High School 50 Bandung, while the control group was 32 students of Public Junior High School 8 Bandung, get counseling from Ujungberung Indah Public Health Center; the entire research conducted in April−May 2017. Research result shows the percentage differential of satisfaction average score improvement on both groups. The treatment group produces percentage of content 14.6%, display 23.6%, accuracy 11.4%, easiness 12.4%, and correctness 17%. In the other hand, control group produces content 5%, display 3%, accuracy 4.3%, easiness 2.8%, and correctness 4.7% with a p value for each indicator was p<0.05 on the intervention group and p<0.05 on the control one. Entirely, it known that adolescent’s satisfaction from those five aspects in using KEPO game has a matter differential p<0.05, with satisfied percentage, was 84%. In conclusion, there was an effect of KEPO game on the five dimensions of female adolescent’s satisfaction. PENGARUH PENGGUNAAN GIM KESEHATAN REPRODUKSI (KEPO) TERHADAP LIMA DIMENSI KEPUASAN REMAJA PEREMPUANKesehatan reproduksi remaja merupakan masalah utama yang harus diperhatikan sehingga untuk mengatasi ketidakpuasan remaja, diperlukan strategi alternatif, yaitu gim kesehatan reproduksi (KEPO) menggunakan smartphone Android. Tujuan penelitian ini adalah menganalisis pengaruh gim KEPO terhadap lima dimensi kepuasan remaja perempuan. Metode penelitian adalah quasi-experiment dan desain penelitian menggunakan pre-test post-test with control group design. Teknik pengambilan sampel menggunakan simple random sampling. Subjek penelitian adalah 64 responden siswa perempuan usia 12–15 tahun SMP Negeri di Kota Bandung. Responden dibagi menjadi dua kelompok, yaitu kelompok perlakuan menggunakan gim KEPO sebanyak 32 siswa SMP Negeri 50 Bandung, sedangkan kelompok kontrol menerima penyuluhan dari Puskesmas Ujungberung Indah sebanyak 32 siswa SMP Negeri 8 Bandung. Penelitian dilaksanakan pada bulan April–Mei 2017. Hasil penelitian menunjukkan perbedaan persentase peningkatan nilai rerata kepuasan pada kedua kelompok. Kelompok perlakuan menghasilkan persentase konten 14,6%, tampilan 23,6%, akurasi 11,4%, kemudahan 12,4%, dan ketepatan 17%. Di sisi lain, kelompok kontrol menghasilkan konten 5%, tampilan 3%, akurasi 4,3%, kemudahan 2,8%, dan ketepatan 4,7% dengan nilai p untuk setiap indikator sebesar p<0,05 pada kelompok intervensi dan p>0,05 kelompok kontrol. Secara keseluruhan, diketahui bahwa kepuasan remaja dari kelima aspek kepuasan dalam menggunakan gim KEPO memiliki perbedaan yang bermakna p<0,05 dengan persentase puas 84%. Simpulan, terdapat pengaruh penggunaan gim KEPO terhadap lima dimensi kepuasan remaja perempuan.

Abstract

The combination of online and offline learning had an impact on students' affective knowledge and skills. This study investigated the relationship between combined offline-online learning method and students’ competency, academic grades, and satisfaction in the Childbirth and Newborn Care Course. This study employed a retrospective cohort design and involved 175 students who completed the Childbirth and Newborn Care Course (attending more than 14 sessions) and the final exam. The data collection utilized the lecturer’s grade records and for the evaluation of students’ satisfaction evaluations the existing academic system was used. Chi-square analysis revealed a significant relationship between the combined offline-online method and students’ skill competency (r = 0.000) and academic grades in the Childbirth and Newborn Care Course. However, no significant relationship was observed between the combined offline and online method and student satisfaction. These findings suggested a need for developing teaching method from conventional to combined offline and online learning. The combined offline-online learning must be well-designed and aligned with learning objectives so that students can be engaged in the learning process properly.

Abstract

Background: Incompetent and inadequate human health resources lead to a declining quality of health services. A quality assurance education system is needed to provide good quality training for healthcare workers. National competence tests are part of the guaranteed measurement of quality education. This study aimed to explore individual student factors that influence the failure to pass the competence test on the nursing competency test.Methods: This research is qualitative research with a phenomenological approach. Data collection using snowball sampling techniques by conducting in-depth interviews in an unstructured manner. The subject of the research was 7 alumni of the DIII Study Program Poltekkes Kemenkes Bengkulu who never passed the competence test. The analysis used is thematic analysis.Results: Individual factors influencing the failure to pass the competence test are low learning interest, low motivation, lack of self-confidence, anxiety, and inability to work on competence tests. This lack of self-confidence involves not understanding how to answer questions, not taking studying seriously, asking challenging questions, and not studying enough. Informants' lack of enthusiasm in studying was caused by them taking things too lightly, believing they would undoubtedly pass, there was a GPA point evaluation, they were lethargic, spent too much time on their cell phones, and were preoccupied with final assignment reports. The majority of respondents stated that they did not feel motivated to study to pass the competency test. Anxiety is frequently felt during tests since students do not comprehend the strategies for answering questions and the time is too short. Aside from that, they are not meticulous in answering the questions because they are in a rush.Conclusion: There are 5 individual factors influencing the failure of students to pass the competence test. Knowing the individual factors that can affect the lack of competence test, the educational institution can optimize the student's self factor following the competency test.
