Abstract

Abstract: Stroke is a global health problem. Ensuring the arrival of patients to the hospital to get medical treatment in a timely manner is vital in a stroke event. The late arrival of stroke patients at the hospital is the main reason for delayed medical treatment. Age, gender, education level, site of residence, distance of residence, and ambulance usage are some of the factors associated with the late arrival of stroke patients to the hospital. This study was aimed to determine the relationship between age, gender, educational status, distance of residence, residence site, as well as ambulance usage and the late arrival of stroke patients at Prof. Dr. R. D. Kandou Hospital Manado. This was an analytical retrospective study using patients’ medical record data. Samples were obtained by using purposive sampling technique with a minimum number of 226 samples. Data were analyzed by using the chi-square test which showed that of the 231 samples the P values were, as follows: age (P=0.711), gender (P=0.879), education level (P=0.010), residence site (P=0.303), distance of residence (P=0.458), and ambulance usage (P=0.469). Conclusion: There was a significant association between education level and the late arrival of stroke patients at Prof. R. D. Kandou Hospital Manado.Keywords: stroke, late arrivalAbstrak: Stroke merupakan masalah kesehatan global. Memastikan kedatangan pasien ke rumah sakit untuk mendapatkan penanganan medis secara tepat waktu merupakan hal yang vital pada suatu kejadian stroke. Keterlambatan kedatangan pada pasien stroke sebelum tiba di rumah sakit merupakan alasan utama terjadinya keterlambatan penanganan medis pada kasus stroke. Usia, jenis kelamin, tingkat pendidikan, jarak tempat tinggal, letak tempat tinggal dan pengguanaan ambulans merupakan beberapa faktor yang berhubungan dengan keterlambatan kedatangan pasien stroke ke rumah sakit. Penelitian ini bertujuan untuk mengetahui hubungan antara usia, jenis kelamin, tingkat pendidikan, letak tempat tinggal, jarak tempat tinggal dan penggunaan ambulans dengan keterlambatan kedatangan pasien stroke di RSUP Prof. dr. R. D. Kandou Manado. Jenis penelitian ialah analitik retrospektif menggunakan data rekam medik pasien. Metode pengambilan sampel menggunakan teknik purposive sampling dengan jumlah minimal 226 sampel yang memenuhi kriteria penelitian. Hasil uji chi-square dari 231 sampel mendapatkan nilai P sebagai berikut: usia (P=0,711), jenis kelamin (P=0,879), tingkat pendidikan (P=0,010), letak tempat tinggal (P=0,303), jarak tempat tinggal (P=0,458) dan penggunaan ambulans (P=0,469). Simpulan: Terdapat hubungan bermakna antara tingkat pendidikan dengan keterlambatan kedatangan pasien stroke di RSUP Prof. R. D. Kandou ManadoKata kunci: stroke, keterlambatan kedatangan

Abstract

Heart failure (HF) with reduced left ventricular ejection fraction (LVEF) is frequently associated with cognitive decline, including memory impairment. This study investigates the relationship between carotid intima-media thickness (CIMT), a marker of subclinical atherosclerosis, and specific memory domains immediate, recall, and recognition in patients with reduced LVEF. A cross-sectional study was conducted on 52 HF patients at Prof. Dr. R.D. Kandou Hospital. Memory function was assessed using the Word List Memory Test, and CIMT was measured via carotid ultrasound. Regression analysis showed that increased CIMT in the left common carotid artery (CCA Sinistra) was significantly associated with lower immediate and recognition memory performance. Furthermore, elevated CIMT in both carotid arteries was negatively correlated with recall memory. These findings highlight a potential vascular contribution to cognitive impairment in HF patients, suggesting that subclinical carotid atherosclerosis may impair cerebral perfusion. The study provides clinical insight into the importance of vascular assessment in managing cognitive health in HF populations. Early detection of carotid pathology may offer a strategy to prevent or reduce cognitive decline in these patients.

Abstract

Background: Visuospatial disorders refer to deficits in attention, exploration, and interaction with the contralateral side of space that cannot be attributed to primary sensory or motor dysfunction. These disorders are usually caused by lesions in the right hemisphere due to cerebral circulation disorders, such as stroke. Transcranial Magnetic Stimulation (TMS) has emerged as a noninvasive treatment option for addressing visuospatial deficits. Purpose: To evaluate the effectiveness of TMS as a therapy for visuospatial disorders in post-stroke patients. Method: This study was conducted using randomized controlled trials (RCTs) comparing TMS with sham treatment for post-stroke visuospatial impairment identified through searches in Google Scholar, PubMed/Medline, Clinical Key, Wiley Online, Science Direct, ResearchGate, and Neurona. A meta-analysis was conducted to assess the efficacy of TMS therapy for improving visuospatial deficits in post-stroke patients. Results: A total of 189 studies on TMS were identified with four homogeneous articles finally included in the meta-analysis. The analysis combined visuospatial assessments using the Motor-Free Visual Perception Test (MVPT), ​​Star Cancellation Test (SCT), Albert Test (AT), and Line Bisection Test (LBT). The meta-analysis revealed significant findings for the MVPT, with a weighted mean difference (WMD) of 2.43 (95% CI: 1.86 to 3.01, p < 0.001). For LBT, WMD was 2.69 (95% CI: 2.04 to 3.34, p = 0.020). Conclusion: The improvements in visuospatial function observed in participants undergoing TMS suggest that TMS may serve as a potential therapeutic option for post-stroke patients with visuospatial impairment.

Abstract

ABSTRACT Introduction: Stroke is one of the causes of mortality and long-term disability in the world. Triglyceride ratio-high-density lipoprotein is an objective parameter that has been validated as a prognostic tool for outcome 3 months after ischemic stroke. Aim: To determine the relationship between triglyceride- high-density lipoprotein with ischemic stroke outcome. Methods: This study was a retrospective observational cross-sectional study in acute ischemic stroke patients treated at Prof dr. R. D Kandou Manado hospital taken by consecutive sampling. The bivariate test was analyzed using a simple linear regression method. Results: 150 acute ischemic stroke patients were included in this study, with an average age of 60.00±10.10 years, compromising 57.3% men, and average TG/HDL ratio of 3.51. Barthel's outcomes with mild dependence were 62%, while the good mRS outcomes were 52.67%. Linear regression test of the TG/HDL ratio on the Barthel scale, obtained the value of r=0.19 and the value of R square=0.038, with a value of p=0.018, and the regression line equation of the Barthel value=65.31+(-1.27) TG/HDL ratio. Whereas to the mRS scale, the value of r=0.18 and the value of R square=0.032, with a value of p=0.028, and the regression line equation of the mRS value=3.058+0.60 TG/HDL ratio. Discussion: : TG/HDL ratio has a negative relationship on the Barthel scale and a positive relationship on the mRS scale, with a weak correlation value. TG/HDL ratio is considered as an independent parameter that predict ischemic stroke outcome. Keywords: Triglycerides, high-density lipoprotein , Ischemic Stroke, Outcome
