Abstract

ABSTRAKPendahuluan: Kista dentigerous adalah kista odontogenik terbanyak kedua dan ditemukan sebanyak 49 persen lesi kistik intraoseus pada rahang manusia. Pemahaman tentang pertumbuhan dan perkembangan kista dentigerous menjadi ameloblastoma hingga saat ini masih belum terjawab. Tujuan laporan kasus ini melaporkan transformasi kista dentigerous menjadi ameloblastoma dari hasil evaluasi serial radiologi 4 tahun. Laporan kasus: Kista dentigerous yang pada awalnya dan selama kurun waktu 4 tahun perawatan tumbuh dan berkembang menjadi ameloblastoma, dijumpai disekitar mahkota gigi molar kedua mandibula kanan pada pasien berusia 17 tahun.Secara radiologi dengan panoramik lesi tampak multilokuler, dengan berbatas tegas. Gigi molar kedua kanan bawah yang terkena juga diangkat setelah prosedur enukleasi dilakukan. Uji histopatologi pertama menunjukan diagnosisnya kista dentigerous. Radiologi panoramik menunjukkan penyembuhan sempurna dari tulang mandibula setelah satu tahun kemudian. Kista dentigerous dengan ukuran yang besar terkadang dapat mengindikasikan patologi lain yang lebih agresif. Empat tahun kemudian terjadi kekambuhan lagi dan dilakukan pembedahan kuretase radikal dengan pemberian larutan Carnoy, dan hasil pemeriksaan patologis dari pembedahan kedua adalah ameloblastoma subtipe folikuler padat. Simpulan: Evaluasi perkembangan kasus kista dentigerous dari radigrafi radiologi selama 4 tahun menunjukkan kista dentigerous dengan gambaran radiografi multilokuler, berpotensi terjadi rekurensi dan berkembang menjadi ameloblastoma bila tidak dilakukan tindakan pembedahan secara adekuat dapat mendeteksi secara dini terdapatnya kista dentigerous dan ameloblastoma. Evaluasi dilakukan secara berkala dengan mengunakan rontgen panoramik pada pasien yang telah dilakukan tindakan operasi baik pada kista dentigerous maupun ameloblastoma. Kata kunci: kista dentigerous; ameloblastoma; radiologi ABSTRACT Introduction: Dentigerous cysts are the second most common odontogenic cyst and found as many as 49 percent of intraosseous cystic lesions in the human jaw. The understanding of the growth and development of dentigerous cysts into ameloblastoma is still unanswered. This case report aims to report the transformation of a dentigerous cyst into ameloblastoma observed from 4-year radiological evaluation series. Case report: A dentigerous cyst that initially grew and developed into ameloblastoma over four years of treatment was found around the crown of the right mandibular second molar in a 17-year-old patient. Radiologically, the lesion appeared to be multilocular, with well-defined borders. The affected lower right second molar was also removed after the enucleation procedure was performed. The first histopathological examination confirmed the diagnosis of a dentigerous cyst. The panoramic radiograph showed complete healing of the mandibular bone after one year. Large dentigerous cysts can sometimes indicate other, more aggressive pathologies. Four years later there was recurrence and radical curettage surgery was performed with Carnoy's solution, and the pathological examination result from the second surgery was solid follicular subtype ameloblastoma. Conclusion: Evaluation of the development of dentigerous cysts from the 4-years radiological evaluation showed dentigerous cysts with multilocular radiographs. The potential for recurrence and development of ameloblastoma if surgery is not performed adequately can detect early dentigerous cysts and ameloblastomas. Evaluation is carried out periodically using panoramic x-rays in patients who have undergone surgery for dentigerous cysts and ameloblastomas. Keywords: dentigerous cyst; ameloblastoma; radiology

Abstract

Komplikasi yang terjadi bersamaan dengan tindakan pencabutan gigi molar ketiga impaksi meningkat seiring meningkatnya frekuensi terjadinya gigi molar tiga impaksi. Fraktur angulus mandibula, bergesernya gigi yang bersebelahan, atau terdorongnya fragmen gigi masuk ke dalam struktur anatomi di sekitarnya merupakan beberapa contoh komplikasi yang dapat terjadi selama pencabutan gigi impaksi. Posisi gigi impaksi, struktur anatomi, kemampuan dan kewenangan operator, memegang peranan penting dalam keberhasilan tindakan pencabutan molar ketiga impaksi. Seorang wanita, berusia 34 tahun, dikonsulkan ke klinik kami dengan diagnosis tulang mandibula yang bergeser. Pasien mengeluh kesulitan untuk membuka mulut, setelah dilakukan pencabutan molar ketiga rahang bawah impaksi dengan lokal anestesi oleh seorang dokter Spesialis yang bukan Spesialis Bedah Mulut dan Maksilofasial satu hari sebelumnya. Dari pemeriksaan klinis, didapatkan adanya rasa nyeri dan parastesia pada sisi mandibula kiri. Gambaran panoramik menunjukkan adanya garis fraktur pada regio angulus mandibula kiri. Setelah dilakukan reposisi terbuka dan fiksasi menggunakan plat rekonstruksi, pasca operasi pasien sudah dapat menggunakan rahang dengan baik.

Abstract

Objective: Marsupialization is therapy for ranulas but sometimes in determining the diagnosis with epidermoid cysts like to be wrong. Epidermoid cysts in the mouth, especially at the base of the tongue, have clinical features similar to those of ranula. In the operating durante we can find the difference. For that we need a more careful examination and in determining the diagnosis of epidermoid cysts with ranula. Methods: a 16-year-old male has a lump under the tongue since the age of 2.5 years as big as a marble. Since 1 month ago the lump was getting bigger and causing the tongue to be bitten frequently, difficulty speaking and pain relapsing in lumps. We perform therapy with biopsy excision where previously we will marsupialization. Results: Epidermoid cyst found during surgery previously diagnosed with ranula Conclusion: Precisely distinguishing epidermoid and ranular cysts requires a complete clinical examination and interview with the patient. So that surgical therapy such as marsupialization and biopsy excision can be selected

Abstract

Objective: This paper aimed to report the management of neglected bilateral mandibular corpus fracture with open reduction and fixation technique. Method: A 38-year-old male patient was referred to Dr. Cipto Mangunkusumo Hospital with chief complaints can not close the mouth and chewing food properly. The patient had a history of injured in motor vehicle accidents, 3 months ago. Extra oral examination showed facial asymmetry, while in ntra oral examination revealed anterior open bite malocclusion. In orthopantomogram (OPG) image, fracture lines in bilateral mandibular corpus were seen. Results: The fractures were managed with Open Reduction Internal Fixation (ORIF) using reconstruction plates to reestablished the occlusion and facial asymmetry. Postoperative evaluation showed good occlusion and facial symmetrical without injured the mental nerve. Conclusion: Early and proper management of bilateral mandible corpus fracture will provide the optimum result. Refracturing the neglected bilateral mandibular corpus is treatment choice in order to restore the mastication and aesthetic function.

Abstract

Objective: The purpose of this case report is to evaluate the treatment plan for the following patients. The definite diagnosis of these lesions requires the integration of their clinical, radiological and histological characteristics. Methods: For 8 years ago, a 52-year-old female patient initially felt an increase in the mouth of the left lower jaw. The lumps are initially the size of a marble and then expand like a ping-pong ball. The lumps are painless. Before the mass, there was no history of trauma in this area. There is no history of fever and no history of weight loss. The patient underwent a biopsy of the left mandible at Dr. Cipto Mangunkusumo National Central Public Hospital in September 2019. Results: Cemento ossifying fibroma. The lumps in the cheek gums extend to the lingual area of ​​teeth 35-37, the size is 6x5x4cm, the texture is hard, the surface is smooth, the boundary is not clear, not fixed, there is no tenderness, and the color and temperature are the same as the surrounding tissues. The lump does not bleed easily, and there is no discharge of pus or blood. The patient's treatment plan was to perform a segmental mandibular resection with a 2.4 mm reconstruction plate. Conclusion: The treatment planning for segmental mandibulectomy in general anesthesia is one of the right treatment options. Choosing a submental submandibular approach to the surgical procedure can provide easy access and visualization during surgery.
