Abstract

Tumors located in the maxillofacial part of the body were classified by WHO in 2017 and among these are intraosseous form of fibromatosis known as Desmoplastic and Cemento-ossifying fibromas. These tumors usually occur in the head and neck region, especially in the mandible and are relatively rare in the maxilla. Meanwhile, this study aims to discribe a rare case of the cemento ossifying fibroma that had been previously diagnosed as desmoplastic fibroma with a mass tumor in the anterior of the maxilla. A 22 years old female reported to the Cipto Mangkusumo Hospital with the main complaint of a lump in the right side of the upper jaw which appeared 2 years prior to the operation. In April 2017, the patient had a biopsy in Tarakan Hospital and the result was a desmoplastic fibroma. Due to the lump enlargement, the patient was admitted to RSCM in July 2019 and had biopsy incision with a diagnosis of cemento ossifying fibroma which was confirmed by the histopathological examination and histology report. The resection of the right part of maxilla was conducted alongside with reconstruction using the free fibular flap. Moreover, cemento ossifying fibroma and desmoplastic fibroma shared similar features, namely, clinical, histological and radiological features which are important in establishing the diagnosis and treatment of patient. Hence, extensive enucleation or resection is required due to the progressive nature of the tumor to prevent the potential for further recurrences.

Abstract

Pendahuluan: Ameloblastoma adalah tumor invasif lokal dengan insidensi kekambuhan yang tinggi jika tidak dieksisi sepenuhnya. Mandibulektomi segmental sebagai pilihan tatalaksana yang dapat menyebabkan deformitas dan asimetri pada wajah, serta masalah pengunyahan. Pemulihan defek setelah reseksi mandibula menimbulkan masalah yang cukup berat. Tujuan laporan kasus ini untuk melaporkan hasil rekontruksi menggunakan cangkok tulang non-vaskularisasi mandibulektomi segmental pada pasien dengan ameloblastoma mandibula. Laporan kasus: Pria berusia 33 tahun datang ke Poliklinik Rawat Jalan Bagian Bedah Mulut dan Maksilofasial RS Persahabatan dengan keluhan utama pembengkakan mandibula yang perlahan bertambah dan tidak nyeri sejak enam tahun lalu. Pemeriksaan ekstra oral tampak asimetri wajah, bukaan mulut tidak ada limitasi, tidak teraba pembesaran kelenjar getah bening regio leher. Pemeriksaan intraoral didapatkan adanya benjolan regio mandibula kiri dengan batas tegas, konsistensi padat, permukaan tampak trauma oklusi gigi antagonis, immobile, dan terdapat nyeri tekan. Gambaran radiologis menunjukan lesi radiolusen multilokuler menyerupai honeycomb pada corpus mandibula kiri meluas ke regio ramus mandibula kiri. Pemeriksaan biopsi menunjukkan hasil ameloblastoma tipe folikuler pada mandibula kiri dan dilakukan mandibulektomi segmental dengan rekonstruksi cangkok tulang fibula non-vaskularisasi. Sembilan bulan setelah operasi, cangkok tulang non-vaskularisasi tidak menunjukkan komplikasi intraoral dengan bekas luka submandibular ekstraoral baik. Simpulan: Cangkok tulang fibula non-vaskular dapat dipertimbangkan sebagai salah satu pilihan untuk rekonstruksi setelah mandibulektomi segmental pada pasien ameloblastoma.Kata kunci: Mandibulektomi segmental, cangkok fibula non-vaskularisasi, ameloblastoma folikuler. ABSTRACTIntroduction: Ameloblastoma is a locally invasive tumour with a high incidence of recurrence if not completely excised. Segmental mandibulectomy as a treatment option that can cause facial deformities and asymmetry and mastication problem. Recovery of the defect after mandibular resection presents a severe problem. This case report was aimed to report the results of reconstruction using a segmental non-vascularised mandibulectomy bone graft in a patient with mandibular ameloblastoma. Case report: A 33-year-old male came to the Outpatient Polyclinic of the Oral and Maxillofacial Surgery Division of Persahabatan General Hospital with the chief complaint of mandibular inflammation, which was slowly increasing yet painless since six years prior. Extraoral examination showed facial asymmetry, no limitation in mouth opening, no palpable enlargement of lymph nodes in the neck region. Intraoral examination revealed a lump in the left mandibular region with firm borders, solid consistency. The surface appears to be traumatic occlusion of the antagonist tooth, immobile, and tenderness was found. Radiological features showed a multilocular, honeycomb-like radiolucent lesion on the left mandibular body extending into the left mandibular ramus region. A biopsy showed follicular ameloblastoma in the left mandible, and a segmental mandibulectomy was performed with non-vascular fibular bone graft reconstruction. Nine months after surgery, non-vascular bone grafts showed no intraoral complications with either extraoral submandibular scar. Conclusion: Non-vascular fibular bone graft can be considered an option for reconstruction after segmental mandibulectomy in ameloblastoma patients. Abstract

Background: Coral is an osteo-conductive biomaterial that can act as an alternative scaffold for osteogenesis. In this in vitro study we analyzed the activity of osteoblast-like cells after treatment with the coral Goniopora. Methods: Human osteoblast-like MG-63 cells were incubated in α-minimal essential medium supplemented with 10% fetal bovine serum and 300 ng/mL amphotericin B plus 1% penicillin-streptomycin and stored in a 5% CO2 incubator at 37°C. The Goniopora were smashed into size A (20 mesh), B (1–2 mm), and C (200 mesh) particles, sterilized using gamma radiation and applied to cells. Protein and alkaline phosphatase (ALP) concentrations were evaluated after incubation for 24 and 48 h. Results: The protein assay of 24 h and 48 h cultured osteoblasts illustrated that treated cells, whether with coral size A, B and C exhibited a lower mean value compared to the untreated cells. For ALP levels there were statistically significant differences at 48 h between B and C (p = 0.004), and A and C (p = 0.09). Conclusions: No significant differences in total protein concentrations were found among all groups after 24 and 48 h. Smaller coral size and longer incubation time tended to facilitate osteogenesis. These results require further empirical validation.

Abstract

Ossifying fibroma (OF) merupakan suatu neoplasma jinak berkapsul yang dianggap sebagai jenis lesi fibroosseous (FOL) yang terdiri dari berbagai jumlah tulang yang terkalsifikasi dan menyerupai sementum dalam stroma fibrosa. Lesi ini biasanya ditemukan pada wanita dengan rentan segala usia namun paling sering terjadi pada decade ketiga dan ke empat kehidupan. Secara radiografik gambaran lesi OF yang bervariasi tergantung pada tingkat mineralisasi dan waktu perkembangan lesi. Lesi radiolusen unilocular dengan densitas campuran atau lesi radiopak dengan batas yang jelas atau radiolusen tipis seperti kapsul fibrosa yang dapat mengakibatkan ekspansi. Seorang pasien wanita berusia 20 tahun, datang dengan keluhan utama gigi bungsu atas dan bawah tidak tumbuh sempurna. Gusi kanan bawah kadang dirasakan sakit dan ada keterbatasan membuka mulut. Pasien menyadari ada asimetri wajah ± 3 bulan yang lalu. Pada pemeriksaan klinis menunjukkan adanya benjolan di corpus mandibula kanan yang minimal, tidak terasa sakit dan tidak tampak tanda-tanda peradangan. Pada pemeriksaan intraoral didapatkan #47 erupsi sebagian. Dilakukan eksisi biopsi dan ekstraksi #47. Diagnosis pasien ini adalah ossifying fibroma jenis juvenile yang terjadi pada usia dewasa muda dengan rencana perawatan eksisi biopsy dan ekstraksi #47. Pemeriksaan CBCT dapat memperlihatkan secara komprehensif gambaran lesi dan pembahasan diagnosis banding lesi.

Abstract

Background: The COVID-19 pandemic spread rapidly worldwide, including in Indonesia. As a result, people developed a fear of visiting hospitals and a tendency to ignore symptoms unrelated to COVID-19. A zygomaticomaxillary complex bone fracture is one of the most frequent midface fractures and is often accompanied by infraorbital nerve paresthesia. Purpose: This article aims to report two cases of neglected zygomaticomaxillary complex bone fractures with persistent infraorbital nerve paresthesia caused by the patient’s fear of seeking medical care. Cases: The first case is a 19-year-old male with a history of trauma two weeks prior to hospital admission. He was diagnosed with a neglected zygomaticomaxillary complex bone fracture, which resulted in persistent infraorbital nerve paresthesia. This condition was due to the patient’s fear of visiting the hospital. The second case is a 21-year-old female who presented with a history of persistent numbness on the left cheek and upper lip for one month prior to hospital admission. The symptoms originated from an accident where the patient fell from a ladder measuring two meters in height. Case management: Surgical intervention—open reduction and internal fixation (ORIF)—was performed on both patients to promote nerve recovery. However, there is ongoing debate regarding the safety of elective oral maxillofacial surgery procedures during the COVID-19 pandemic. After a follow-up period, both patients experienced a recovery of sensory function in the infraorbital nerve. Conclusion: Infraorbital nerve sensory disturbance along with zygomaticomaxillary complex bone fracture found during the COVID-19 pandemic should be managed with ORIF to improve nerve recovery.

Abstract

ABSTRACTIntroduction: The occurrence of the COVID-19 pandemic may affect the prevalence of patients with complaints of impacted teeth in dental hospital Faculty of Dentistry University of Indonesia (FKG UI), and there is no recent research on the distribution and frequency of impacted teeth based on age, gender, and teeth type in this hospital during the COVID-19 pandemic. This study aims to determine the distribution and frequency of impacted teeth during the COVID-19 pandemic in dental hospitals. Methods: A descriptive study uses the secondary data on impacted teeth patient's medical records. Utilizing purposive sampling, we selected participants for the research by carefully examining medical records to identify individuals that had undergone impacted tooth removal. The study used secondary data from the medical records of oral surgery patients at dental hospital FKG UI from June 2019 to June 2021, including complete patient identity, clinical diagnosis, and panoramic radiograph of the impacted teeth in June 2019 to June 2021. Diagnosis was determined through screening of medical records and panoramic radiographs. Data were analyzed using the statistical analysis software (SPSS version 21, IBM Corp.) Results: From 3,225 medical records of oral surgery patients at dental hospital FKG UI before and during COVID-19 pandemic, A total of 1292 patients were included in the study and 1496 impacted teeth cases were found. The frequency of impacted teeth cases before the pandemic was 825 (55.1%) cases; there were 671 (44.9%) cases during the pandemic. Conclusion: COVID-19 has caused a decrease in the distribution and frequency of impacted teeth cases, mostly in the middle aged patients, while more female as compared to male. The most common occurrence was in the third molars and the lowest was in the second molar tooth. Keyword : tooth, impacted, COVID-19, retrospective studies, wisdom tooth

Abstract

Background: Cleft lip and palate are the most common congenital orofacial anomalies. Dental students must have a basic knowledge, understanding, and awareness of comprehensive cleft management. The Faculty of Dentistry Universitas Indonesia undergraduate program uses the problem-based learning (PBL) method to teach dental students about cleft management. Purpose: This study aims to determine the level of knowledge and exposure to learning with the PBL method regarding comprehensive cleft management. Methods: A cross-sectional design was used, and data were collected from 253 dental students using an online questionnaire via Google Forms. Results: Ten preclinical students (8.5%) had a low level of knowledge, 56 (47.5%) had a sufficient level of knowledge, and 52 (44.1%) had a high level of knowledge of comprehensive cleft management. Among the clinical students, 6 (4.4%), 65 (48.1%), and 64 (47.4%) demonstrated low, sufficient, and high levels of knowledge, respectively. Furthermore, most students were content with the lectures on comprehensive cleft management (46.24% satisfied and 20.55% very satisfied) and the supervision received during the lectures (43.48% agreed and 23.71% strongly agreed). Most students believed the lectures on cleft management could be applied when they graduate as dentists. The Chi-square test showed no significant relationship between learning exposure and the student’s knowledge level. Conclusion: Most Faculty of Dentistry Universitas Indonesia students who had learned about cleft lip and palate using the PBL method had sufficient knowledge about comprehensive cleft management. However, no significant relationship was observed between learning exposure and knowledge level.

Abstract

Introduction: Mandibular canine impaction is one of the most complex dental anomalies. The low incidence rate of mandibular canines impaction resulted in the unavailability of guidelines for managing impacted mandibular canines, especially bilateral horizontal cases. This case report describes a surgical procedure to manage bilateral horizontal mandibular canines impaction. Case report: A 14-year-old girl with bilateral horizontal mandibular canines impaction was referred from the orthodontics department. The orthopantomograph (OPG) radiograph shows there was bilateral horizontal mandibular canines impaction near premolars root apices. The patient went for surgical removal in general anesthesia for the impacted canines. Odontectomy was performed by separating the tooth into several fragments. A three months follow-up showed there was no post-operative complaint. Conclusion: Odontectomy of bilaterally impacted mandibular canines can be done to avoid interference with orthodontic treatment. Keyword : Bilateral horizontal canine impaction, odontectomy, oral surgery Penatalaksanaan kasus impaksi kaninus bilateral horizontal mandibula secara bedah: laporan kasusABSTRAKPendahuluan: Impaksi kaninus bilateral mandibula adalah salah satu bentuk kompleksitas kelainan gigi. Insidensi yang rendah pada kasus impaksi kaninus bilateral mandibula menyebabkan ketiadaan petunjuk untuk penatalaksanaan bedah pada kasus yang serupa, terutama pada kasus bilateral. Laporan kasus ini bertujuan mendeskripsikan prosedur pembedahan pada kasus impaksi kaninus bilateral mandibula. Laporan kasus: Perempuan berusia 14 tahun dengan impaksi kaninus bilateral mandibula dirujuk dari departemen ortodonsia. Radiografi panoramik meununjukkan terdapat impaksi kaninus bilateral dengan tipe horizontal, berdekatan dengan ujung akar premolar. Pasien dilakukan ekstraksi gigi impaksi kaninus dalam anestesi umum dengan metode pembelahan gigi menjadi beberapa fragmen. Pada kontrol tiga bulan selanjutnya tidak terdapat keluhanan pascaoperasi. Simpulan: Odontektomi pada kasus impaksi kaninus bilateral mandibula dapat menjadi opsi untuk mencegah gangguan dalam perawatan ortodonti.Kata kunci: Impaksi kaninus bilateral horizontal, odontektomi, bedah mulut
