Abstract

Saat ini terjadi perkembangan populasi lansia di Indonesia sehingga isu kesehatan lansia merupakan sesuatu yangesensial. Lansia adalah individu yang telah mengalami proses menua sehingga terjadi berbagai perubahan biologispada tubuhnya, sehingga berpengaruh terhadap penurunan fungsi organ. Kehilangan gigi adalah salah satu masalahyang umum pada lansia sehingga kebutuhan untuk pemasangan gigi tiruan merupakan hal penting yang perludiperhatikan masyarakat kelompok lansia. Implan gigi tiruan merupakan salah satu alternatif pengganti kehilangangigi yang memiliki banyak keuntungan dibandingkan gigi tiruan lain. Namun perlu disadari bahwa penggunaan implangigi memiliki pertimbangan-pertimbangan kondisi rongga mulut dan sistemik tertentu untuk menunjang keberhasilanperawatan. Pemahaman mengenai terjadinya proses menua dan hubungannya dengan pertimbangan penggunaanimplan sangat penting untuk diperhatikan oleh klinisi sebelum merencanakan perawatan, terutama berkaitan denganperubahan pada sistem pertahanan tubuh yang terjadi seiring proses menua.

Abstract

ABSTRAKPendahuluan: Penggunaan plat rekonstruksi setelah tindakan reseksi mandibula adalah hal yang umum digunakan pada kasus tumor rongga mulut. Komplikasi ekspos plat pada pasien dengan penggunaan plat rekonstruksi tanpa adanya bone graft cukup tinggi sekitar 20%. Fasciocutaneous advancement flap adalah teknik yang dapat digunakan untuk merekonstruksi dan memperbaiki defek jaringan lunak pada rekonstruksi plat. Teknik ini sederhana, singkat dalam proses pembedahan dan perawatan pasca operasi yang mudah untuk dokter dan pasien. Tujuan laporan kasus ini adalah melaporkan rekonstruksi defek plat mandibula dengan fasciocutaneous advancement flap. Laporan kasus: Pasien laki-laki 44 tahun datang ke rumah sakit dengan ekspos plat sejak 1 bulan sebelum masuk rumah sakit. Pasien riwayat dilakukan hemimandibulektomi 1 tahun yang lalu dengan diagnosis calcifying epithelial odontogenic tumor dan direkonstruksi menggunakan protesa condyle dan rekonstruksi plat. Perawatan yang dilakukan meliputi debridemen dan fasciocutaneous advancement flap untuk menutup defek ekspos plat rekonstruksi. Simpulan: Rekonstruksi defek plat mandibula dengan fasciocutaneous advancement flap dapat memberikan hasil yang baik, proses pembedahan yang singkat dan sederhana.Kata kunci: ekspos plat; fasciocutaneous advancement flap; rekonstruksi mandibula ABSTRACTIntroduction: The use of a reconstruction plate after mandibular resection is common in cases of oral tumours. Complications of plate exposure in patients using a reconstructed plate without a bone graft are quite high, around 20%. Fasciocutaneous advancement flap is a technique used to reconstruct and repair soft tissue defects in plate reconstruction. This technique is simple, short in surgery, and easy for postoperative care for doctors and patients. This case report aims to present the reconstruction of the mandibular plate defect with a fasciocutaneous advancement flap. Case report: A 44-year-old male patient came to the hospital with plate exposure one month before hospital admission. The patient had a history of hemimandibulectomy one year prior with a calcifying epithelial odontogenic tumour diagnosis and was reconstructed using a condyle prosthesis and plate reconstruction. Treatment includes debridement and a fasciocutaneous advancement flap to cover the exposed defect of the reconstruction plate. Conclusion: Reconstruction of the mandibular plate defect with a fasciocutaneous advancement flap can give good results with a brief and simple surgical process.Keywords: plate exposure; fasciocutaneous advancement flap; mandibular reconstruction

Abstract

ABSTRAKPendahuluan: Kista dentigerous adalah kista odontogenik terbanyak kedua dan ditemukan sebanyak 49 persen lesi kistik intraoseus pada rahang manusia. Pemahaman tentang pertumbuhan dan perkembangan kista dentigerous menjadi ameloblastoma hingga saat ini masih belum terjawab. Tujuan laporan kasus ini melaporkan transformasi kista dentigerous menjadi ameloblastoma dari hasil evaluasi serial radiologi 4 tahun. Laporan kasus: Kista dentigerous yang pada awalnya dan selama kurun waktu 4 tahun perawatan tumbuh dan berkembang menjadi ameloblastoma, dijumpai disekitar mahkota gigi molar kedua mandibula kanan pada pasien berusia 17 tahun.Secara radiologi dengan panoramik lesi tampak multilokuler, dengan berbatas tegas. Gigi molar kedua kanan bawah yang terkena juga diangkat setelah prosedur enukleasi dilakukan. Uji histopatologi pertama menunjukan diagnosisnya kista dentigerous. Radiologi panoramik menunjukkan penyembuhan sempurna dari tulang mandibula setelah satu tahun kemudian. Kista dentigerous dengan ukuran yang besar terkadang dapat mengindikasikan patologi lain yang lebih agresif. Empat tahun kemudian terjadi kekambuhan lagi dan dilakukan pembedahan kuretase radikal dengan pemberian larutan Carnoy, dan hasil pemeriksaan patologis dari pembedahan kedua adalah ameloblastoma subtipe folikuler padat. Simpulan: Evaluasi perkembangan kasus kista dentigerous dari radigrafi radiologi selama 4 tahun menunjukkan kista dentigerous dengan gambaran radiografi multilokuler, berpotensi terjadi rekurensi dan berkembang menjadi ameloblastoma bila tidak dilakukan tindakan pembedahan secara adekuat dapat mendeteksi secara dini terdapatnya kista dentigerous dan ameloblastoma. Evaluasi dilakukan secara berkala dengan mengunakan rontgen panoramik pada pasien yang telah dilakukan tindakan operasi baik pada kista dentigerous maupun ameloblastoma. Kata kunci: kista dentigerous; ameloblastoma; radiologi ABSTRACT Introduction: Dentigerous cysts are the second most common odontogenic cyst and found as many as 49 percent of intraosseous cystic lesions in the human jaw. The understanding of the growth and development of dentigerous cysts into ameloblastoma is still unanswered. This case report aims to report the transformation of a dentigerous cyst into ameloblastoma observed from 4-year radiological evaluation series. Case report: A dentigerous cyst that initially grew and developed into ameloblastoma over four years of treatment was found around the crown of the right mandibular second molar in a 17-year-old patient. Radiologically, the lesion appeared to be multilocular, with well-defined borders. The affected lower right second molar was also removed after the enucleation procedure was performed. The first histopathological examination confirmed the diagnosis of a dentigerous cyst. The panoramic radiograph showed complete healing of the mandibular bone after one year. Large dentigerous cysts can sometimes indicate other, more aggressive pathologies. Four years later there was recurrence and radical curettage surgery was performed with Carnoy's solution, and the pathological examination result from the second surgery was solid follicular subtype ameloblastoma. Conclusion: Evaluation of the development of dentigerous cysts from the 4-years radiological evaluation showed dentigerous cysts with multilocular radiographs. The potential for recurrence and development of ameloblastoma if surgery is not performed adequately can detect early dentigerous cysts and ameloblastomas. Evaluation is carried out periodically using panoramic x-rays in patients who have undergone surgery for dentigerous cysts and ameloblastomas. Keywords: dentigerous cyst; ameloblastoma; radiology

Abstract

ABSTRACTIntroduction: The occurrence of the COVID-19 pandemic may affect the prevalence of patients with complaints of impacted teeth in dental hospital Faculty of Dentistry University of Indonesia (FKG UI), and there is no recent research on the distribution and frequency of impacted teeth based on age, gender, and teeth type in this hospital during the COVID-19 pandemic. This study aims to determine the distribution and frequency of impacted teeth during the COVID-19 pandemic in dental hospitals. Methods: A descriptive study uses the secondary data on impacted teeth patient's medical records. Utilizing purposive sampling, we selected participants for the research by carefully examining medical records to identify individuals that had undergone impacted tooth removal. The study used secondary data from the medical records of oral surgery patients at dental hospital FKG UI from June 2019 to June 2021, including complete patient identity, clinical diagnosis, and panoramic radiograph of the impacted teeth in June 2019 to June 2021. Diagnosis was determined through screening of medical records and panoramic radiographs. Data were analyzed using the statistical analysis software (SPSS version 21, IBM Corp.) Results: From 3,225 medical records of oral surgery patients at dental hospital FKG UI before and during COVID-19 pandemic, A total of 1292 patients were included in the study and 1496 impacted teeth cases were found. The frequency of impacted teeth cases before the pandemic was 825 (55.1%) cases; there were 671 (44.9%) cases during the pandemic. Conclusion: COVID-19 has caused a decrease in the distribution and frequency of impacted teeth cases, mostly in the middle aged patients, while more female as compared to male. The most common occurrence was in the third molars and the lowest was in the second molar tooth. Keyword : tooth, impacted, COVID-19, retrospective studies, wisdom tooth

Abstract

Introduction: Orofacial cleft is one of the many congenital malformations that often occur in human, leaving it at the fourth level of the most common orofacial birth defect findings. The incident rate is one in 700-1000 deliveries, where without therapeutic and surgical interventions, children with an orofacial cleft may have problems with speech, nutrition intake, and growth. Bone morphogenetic protein 2 (BMP-2) gene play essential roles in the migration and proliferation of neural crest cell of the early head formation and regulate mineralised tissues such as maxillary, mandible, palate and teeth. This study was aimed to analyse the BMP-2 polymorphism and its potential association with orofacial cleft in an Indonesian population. Methods: Cross-sectional study was conducted towards 128 samples, 32 samples of orofacial cleft patients and 96 samples of control. Extracted genotype and allele was determined with PCR-RFLP method using stored DNA samples from 32 orofacial cleft patients, and 96 healthy control. Results: The TT genotype was showing the p-value = 0.001, OR = 2.43% in orofacial samples (71.4%), which was significantly higher than in control groups (28.6%). The allele distribution was also considered statistically significant (p = 0.036, OR =1.89%. Conclusion: There is a significant association of rs235768 A>T polymorphism of the BMP-2 gene on non-syndromic orofacial cleft patients in Indonesia.

Abstract

Background: Cleft lip and palate are the most common congenital orofacial anomalies. Dental students must have a basic knowledge, understanding, and awareness of comprehensive cleft management. The Faculty of Dentistry Universitas Indonesia undergraduate program uses the problem-based learning (PBL) method to teach dental students about cleft management. Purpose: This study aims to determine the level of knowledge and exposure to learning with the PBL method regarding comprehensive cleft management. Methods: A cross-sectional design was used, and data were collected from 253 dental students using an online questionnaire via Google Forms. Results: Ten preclinical students (8.5%) had a low level of knowledge, 56 (47.5%) had a sufficient level of knowledge, and 52 (44.1%) had a high level of knowledge of comprehensive cleft management. Among the clinical students, 6 (4.4%), 65 (48.1%), and 64 (47.4%) demonstrated low, sufficient, and high levels of knowledge, respectively. Furthermore, most students were content with the lectures on comprehensive cleft management (46.24% satisfied and 20.55% very satisfied) and the supervision received during the lectures (43.48% agreed and 23.71% strongly agreed). Most students believed the lectures on cleft management could be applied when they graduate as dentists. The Chi-square test showed no significant relationship between learning exposure and the student’s knowledge level. Conclusion: Most Faculty of Dentistry Universitas Indonesia students who had learned about cleft lip and palate using the PBL method had sufficient knowledge about comprehensive cleft management. However, no significant relationship was observed between learning exposure and knowledge level.

Abstract

Introduction: Mandibular canine impaction is one of the most complex dental anomalies. The low incidence rate of mandibular canines impaction resulted in the unavailability of guidelines for managing impacted mandibular canines, especially bilateral horizontal cases. This case report describes a surgical procedure to manage bilateral horizontal mandibular canines impaction. Case report: A 14-year-old girl with bilateral horizontal mandibular canines impaction was referred from the orthodontics department. The orthopantomograph (OPG) radiograph shows there was bilateral horizontal mandibular canines impaction near premolars root apices. The patient went for surgical removal in general anesthesia for the impacted canines. Odontectomy was performed by separating the tooth into several fragments. A three months follow-up showed there was no post-operative complaint. Conclusion: Odontectomy of bilaterally impacted mandibular canines can be done to avoid interference with orthodontic treatment. Keyword : Bilateral horizontal canine impaction, odontectomy, oral surgery Penatalaksanaan kasus impaksi kaninus bilateral horizontal mandibula secara bedah: laporan kasusABSTRAKPendahuluan: Impaksi kaninus bilateral mandibula adalah salah satu bentuk kompleksitas kelainan gigi. Insidensi yang rendah pada kasus impaksi kaninus bilateral mandibula menyebabkan ketiadaan petunjuk untuk penatalaksanaan bedah pada kasus yang serupa, terutama pada kasus bilateral. Laporan kasus ini bertujuan mendeskripsikan prosedur pembedahan pada kasus impaksi kaninus bilateral mandibula. Laporan kasus: Perempuan berusia 14 tahun dengan impaksi kaninus bilateral mandibula dirujuk dari departemen ortodonsia. Radiografi panoramik meununjukkan terdapat impaksi kaninus bilateral dengan tipe horizontal, berdekatan dengan ujung akar premolar. Pasien dilakukan ekstraksi gigi impaksi kaninus dalam anestesi umum dengan metode pembelahan gigi menjadi beberapa fragmen. Pada kontrol tiga bulan selanjutnya tidak terdapat keluhanan pascaoperasi. Simpulan: Odontektomi pada kasus impaksi kaninus bilateral mandibula dapat menjadi opsi untuk mencegah gangguan dalam perawatan ortodonti.Kata kunci: Impaksi kaninus bilateral horizontal, odontektomi, bedah mulut
