Abstract

Background: Pemphigus vulgaris (PV) is an autoimmune disease characterized by mucocutaneous blistering and erosion. This is rare, but greatly affects the patient’s life quality and often cause complication of disease and therapy. Hyperglycemia is a complication due to steroid use called glucocorticoid-induced hyperglycemia (GIH). This case report describes hyperglycemia in PV treatment, which later can be a consideration of PV management.Case: A 44-year-old male patient complained of painful lesions on almost the whole body with a form of bullae, erosion, crusting, brittle, the Nikolsky sign (+), and Asboe-Hansen sign (+). The patient was diagnosed with PV. After he had supportive therapy and high-doses of methylprednisolone, his blood sugar is increased. Patients diagnosed by hyperglycemia state due to steroid use, then given insulin as therapy. The patient diagnosed with PV based on history taking and physical examination, but the histopathologic examination wasn’t done due to lack of modality at the hospital. The steroid was given as an immunosuppressive. Be the main therapy for PV, steroids lead hyperglycemia due to disruption of glucose metabolism, thereby increasing insulin resistance in tissues. The diagnosis of hyperglycemia due to steroid use is made in a patient with a normal sugar level before PV therapy. It occurred within the first 1-2 days of therapy. In these patients, diagnosis confirmed by increasing pre-prandial, 2 h post-prandial, and any-time glucose level, after two days methylprednisolone administration. Collaboration with internal medicine colleagues is needed.Conclusion: PV treatment with steroids can induce hyperglycemia, which is dangerous. The understanding mechanism is needed to make early detection and provide therapy properly.

Abstract

Sifilis merupakan suatu infeksi menular seksual (IMS) yang disebabkan oleh Treponema pallidum. Kondisi ini dapat menimbulkan komplikasi apabila tidak mendapatkan penanganan yang cepat dan tepat. Salah satu komplikasi yang perlu diperhatikan adalah sifilis okuler. Minimnya studi epidemiologi sifilis okuler di Indonesia membuat kewaspadaan para klinisi tergolong kurang sehingga penelitian penting untuk dilakukan untuk menekan angka morbiditas. Tujuan: Penelitian ini dilakukan untuk mengetahui prevalensi dan karakteristik pasien sifilis okuler yang berkunjung ke Poliklinik Kulit dan Kelamin RSUP Prof. Dr. IGNG Ngoerah. Metode: Studi deskriptif obsevasional yang dilakukan ini memanfaatkan data sekunder dari rekam medis pasien sifilis okuler yang menjalani pengobatan di Poliklinik Kulit dan Kelamin RSUP Prof. Dr. IGNG Ngoerah Denpasar selama periode Januari 2018–Desember 2020. Seluruh pasien yang berkunjung saat periode penelitian dilibatkan dalam penelitian ini (total sampling) dan dilakukan eksklusi apabila data rekam medis tidak lengkap. Analisis data dilakukan dengan perangkat lunak SPSS versi 20. Hasil: Terdapat 595 pasien sifilis yang merupakan kasus baru dan hanya ditemukan lima kasus sifilis okuler. Kasus sifilis okuler didominasi oleh pasien laki-laki (8%) dan kelompok usia produktif (60%). Sebagian besar pasien merupakan pekerja swasta (80%) dan berasal dari luar Bali (60%). Seluruh pasien belum menikah dan 80% di antaranya adalah heteroseksual. Diketahui bahwa 40% pasien memiliki status HIV positif dan seluruh pasien sifilis okuler mengalami sifilis stadium sekunder. Kesimpulan: Terjadi peningkatan kasus sifilis yang sangat drastis bila dibandingkan dengan penelitian periode sebelumnya di RSUP Prof. Dr. IGNG Ngoerah Denpasar. Penelitian lebih lanjut masih dibutuhkan untuk mentelaah faktor risiko lain yang mungkin berperan dalam terjadinya sifilis okuler.

Abstract

The neutrophil-to-lymphocyte ratio (NLR) is the ratio between the number of neutrophils and lymphocytes in the peripheral blood. Neutrophils are responsible for an initial immune response to pathogens that enter the body through the mechanisms of chemotaxis, phagocytosis, the release of reactive oxygen species (ROS), granular proteins, production and release of cytokines. Neutrophils also have an important role in the occurrence of the systemic inflammatory response (SIRS). There are several types of diseases in the field of dermatology that are also related to NLR. It is associated with the presence of an inflammatory process. In addition, NLR is considered an easy, inexpensive, and reproducible parameter associated with clinical outcomes and disease severity. Several diseases in the field of dermatology found to be associated with NLR include leprosy, Stevens-Johnson syndrome/ toxic epidermal necrolysis (SJS/TEN), psoriasis, vitiligo, atopic dermatitis, systemic sclerosis, and skin cancer. In recent years, its role as an independent prognostic factor for neoplasms and as an inflammatory biomarker in various acute and chronic diseases has been increasingly established in the dermatology field. However, not all studies have found significant results. One of them is vitiligo which still finds controversial results. For this reason, this literature review will discuss specifics related to several diseases in dermatology associated with NLR.
