Abstract

Introduction: Condylomata acuminata (CA) is human papillomavirus (HPV) infection, and one of the most common sexually transmitted disease, characterized with papul or papillomatous nodule in genital, perineum and anal, this disease can be asymptomatic as well. Some study proved sexually transmitted infection increases the incidence of Human immunodeficiency virus (HIV)/ acquired immunodeficiency syndrome (AIDS), and vice versa. Management of CA is Trichloroacetic acid (TCA), Tincture Podophyllin, and Electrocautery. This study aims to determine profile of condyloma acuminata in Dermatovenereology Outpatient Polyclinic Sanglah General Hospital Denpasar Bali period 2015-2017. A retrospective study during three years (2015-2017).Method: Data was obtained from medical record in Dermatovenereology Outpatient Polyclinic Sanglah General Hospital Denpasar Bali period 2015- 2017.Result: Total patient condyloma acuminata is 260 patients (5.48%), male is dominant 70 patients (67.31%), range age 12-35 years old (123%), there was 59 patient who infected with HIV (22.31%), with five pregnancy (1.92%). Most therapy is given trichloroacetic acid (TCA) (79.62%).Conclusion: Men suffered from condyloma acuminata more commonly than women, the most common treatment for this condition is trichloroacetic acid (TCA)

Abstract

circumscribed, highly pruritic, hyperkeratotic, and hyperpigmented papules occurring typically over the shins, outer aspects of upper arms, and on the upper back with amyloid deposits in the papillary dermis. Several therapeutic strategies, including topical steroids, oral antihistamines, cyclosporine, retinoids, laser, phototherapy, cryosurgery, and surgical interventions, have been reported as treatment options for patients with LA, but no standardized treatment has been established.Case report: A 58-year-old man came to the Dermatovenereology Outpatient Department complaints of itchy blackish-brown papules on the shins. Dermatology examination found discrete multiple hyperpigmentation papules and plaque covered with white scale. A scar-like center surrounded by brownish circles or white edges was found from the dermoscopic examination. The histopathological examination found thickened keratin with compact orthokeratosis and hyaline materials in the papillary dermis with dendritic melanophages. The patient diagnosed with LA and treated by combining desoximetasone cream 0.25% with 3% salicylic acid. The papules on the legs had flattened in the patient, with a significant improvement in the severe itching after three weeks.Conclusion: Combination therapy of potent corticosteroids and keratolytic seems to be an appropriate modality and well-tolerated by LA patients. Skin lesion becomes thinner, and pruritus is reduced.

Abstract

Introduction: Syphilis is an infection caused by Treponema pallidum of the spirochaete family.Case: A 19-year-old Balinese came with a complaint of red spots on the palms of the hands and feet one month ago. The patient has a hearing disability and is currently enrolled in a special-needs school in Denpasar. VDRL examination with reactive results with titer 1:256 and TPHA reactive with TPHA titer 1:5120. The patient received benzathine penicillin G 2.4 million units intramuscularly. Significant improvement was obtained, and the serological test was decreased.Discussion: Syphilis transmits through sexual contact, vertically from pregnant women to their fetuses, through needle sharing. People with hearing disabilities and adolescents are considered vulnerable as objects of harassment and sexual violence.Conclusion: People with disabilities and adolescents are vulnerable to sexually transmitted diseases. Information on reproductive health and sexually transmitted diseases need to be more readily available for people with disabilities. Secondary syphilis therapy with a single dose of benzathine penicillin is effective from clinical or serological observations.

Abstract

ABSTRAK Insiden kanker kulit mengalami peningkatan selama sepuluh tahun terakhir. Terdapat dua hingga tiga juta kasus kanker non melanoma dan 132 ribu kasus kanker melanoma di dunia setiap tahunnya. Sepertiga dari total kasus kanker merupakan kasus kanker kulit. Berdasarkan Skin Cancer Foundation Statistic, seperlima dari warga amerika akan mengalami kanker kulit semasa hidupnya. Di Indonesia, kejadian kanker kulit memiliki persentase sebesar 7%. Selain kanker kulit, angka insiden dari pra kanker juga mengalami peningkatan tiap tahunnya. Tujuan penelitian ini ialah untuk mengetahui profil dari pra kanker maupun kanker kulit di RSUP Sanglah dalam periode 2015-2018 berdasarkan jenis tumor kulit, usia, jenis kelamin, lokasi lesi tumor kulit, pekerjaan dan terapi pada pasien pra kanker dan kanker kulit di RSUP Sanglah. Jenis penelitian ini adalah deskriptif retrospektif yaitu dengan menggunakan data sekunder berupa rekam medis pasien. Pengambilan sampel dengan menggunakan total sampling. Variabel yang akan diukur meliputi: jenis kanker kulit, usia, jenis kelamin, lokasi lesi kanker kulit, pekerjaan. Dari penelitian ini, ditemukan sebanyak 21 kasus tumor kulit. 8 merupakan kasus dengan kanker kulit dan 3 merupakan kasus pra kanker. Pasien berjenis kelamin wanita, rentangan usia 50-69, bagian wajah, bekerja di luar ruangan dan terapi dengan bedah eksisi merupakan yang umum pada kasus kanker kulit. Pada kasus pra kanker, pasien berjenis kelamin pria, rentangan usia 70-89, bagian lengan, bekerja di luar ruangan dan terapi dengan bedah eksisi merupakan yang umum pada kasus tersebut. Abstract

Sexually Transmitted Infections (STI) is one of the world’s major health problems. Bali, and in particular the town of ubud is a world famous tourist destination and often visited by foreigners. This makes Ubud very vulnerable to the spread of STIs. The purpose of this study was to determine the prevalence and characteristics of STI cases based on age, gender and risk factors at the Anggrek clinic UPT Ubud II from January to December 2016. This research is a descriptive research with secondary data gathered from the medical record. Sample of this research is medical record of STI patient who came to Anggrek clinic UPT Ubud II in the period of 1 January - 31 December 2016 with amount of 273 samples. Secondary data are further processed statistically by using statistical software.The results of the study showed the most frequent STIs were HIV, there were 134 cases (49.1%), followed by Syphilis with 69 cases (25.3%), Gonorrhea with 61 cases (22.3%) and Condyloma Akuminata with 9 cases (3.3%). Men had the higher incidence of STI with 251 cases (91.9%) whereas in women there were only 22 cases (8.1%). The highest STI population was in the age group of 25-49 years where there were 166 cases (60.8%). The highest risk factor for Condyloma acuminata, gonorrhea and syphilis was 79 people (56.8%), in HIV the highest risk factor was homosexual (MSM) of 112 cases (83.6%). It can be concluded that the characteristic of STI patients found is HIV as the most frequent type of STI, where the incidence of STIs is more common in males, in the 25-49 age group with homosexual (MSM) risk factors. Abstract

ABSTRAKDermatitis kontak akibat kerja (DKAK) adalah salah satu penyakit kulit yang timbul pada lingkungan kerja akibat adanya kontak langsung dari pekerja dengan bahan iritan maupunalergen. Tujuan dari penelitian ini adalah mengetahui prevalensi dan karakteristik kejadiandermatitis kontak akibat kerja pada nelayan di Desa Perancak, Jembrana tahun 2018. Penelitianini dirancang sebagai studi deskriptif dengan pendekatan cross sectional. Penelitian dilakukandengan teknik total sampling, dan didapatkan 46 orang yang sesuai kriteria. Hasil menunjukanbahwa dari 46 orang nelayan didapatkan kejadian tersering pada rentang usia 45-54 tahun(45,7%), dan didominasi oleh laki-laki (84,8%). Gejala tersering adalah likenifikasi (76%),sering terjadi di telapak tangan (87%), dan sebagian besar responden tidak melakukanpengobatan (60,9%). Sebagian besar responden tidak memiliki riwayat atopi diri (80,4%), danpada keluarga (84,8%). Sebagian besar responden sudah memiliki masa kerja ?4 tahun(97,8%).Seluruh responden menyatakan mengalami kontak langsung dengan peralatan kerja, danterkadang mengakibatkan luka. Penggunaan alat pelindung diri tidak dilakukan oleh 43 orang(93,5%). Lama kontak >4 jam/hari (93,5%), dan frekuensi paparan tersering >5 kali/hari (63%).Diperlukan tindakan pencegahan dengan menggunakan alat pelindung diri saat melakukanproses kerja. Kata kunci: dermatitis kontak akibat kerja, prevalensi, karakteristik, nelayan.

Abstract

Background: Lipschutz ulcer, also known as vulvae acutum ulcer, is an acute ulcer in the vulva. The aetiology and pathogenesis of Lipschutz ulcer are unclear, but it is known to be associated with infectious diseases such as Mycoplasma infection, paratyphoid fever, influenza A, and most infections with Epstein-Barr virus (EBV). This ulcer appears in adolescent females aged 14–20 years old, with 70% of cases occurring in virgin women. Purpose: To review the causes of non-Sexually Transmitted Diseases (STD) ulcers, so clinicians can establish an accurate diagnosis and rational therapy; therefore, minimizing the psychological impact on the patient due to possible misdiagnosis of STD-induced ulcer. Case: A painful wound in the genital area of a 20-year-old virgin woman. She was diagnosed with Lipshutz ulcer and vulvovaginal candidiasis based on the anamnesis, physical examination, and laboratory examination obtained from vaginal discharge using potassium hydroxide, Gram staining, and blood test to exclude genital ulcer caused by the sexually transmitted agent. A blood test was taken, including the serological tests for syphilis and genital herpes. Tests for EBV were also performed. The patient was treated only with single-dose fluconazole 150 mg orally and saline compress on the ulcer. Genital ulcer and vaginal discharge improved one week after treatment. Discussion:  Lipshutz ulcer management is symptomatic, usually self-limiting, and disappears spontaneously within 1–2 weeks without recurrences. It can also occur as a single lesion with possible coinfection of other agents, for example, candidiasis, which in this case, requires treatment of comorbidities. Hygiene factors play an essential role in preventing occurrence of the secondary infection and further development of the disease. Conclusion: The diagnosis of Lipshutz ulcer is mainly based on clinical manifestation after excluding several possible ulcers that can affect the genital area, including sexually transmitted infections.
