Abstract

Disamping efek dari infeksi hepatitis C, pengobatan hepatitis C dengan menggunakan pegylated interferon alfa dikatakan memiliki efek samping berupa depresi. Untuk mengevaluasi gejala depresi pada pasien hepatitis C yang mendapatkan terapi pegylated interferon alfa di divisi Gastroentero-hepatologi RSUD Dr. Saiful Anwar Malang. Penilaian depresi dilakukan 1 kali, dengan menggunakan kuisioner Patient Health Questionnaire 9 (PHQ-9). Penelitian ini dilakukan antara tahun 2014-2015. Seluruh pasien hepatitis C yang mendapatkan terapi dengan pegylated interferon alfa, diberikan formulir kuisioner dari Patient Health Questionnaire (PHQ-9) untuk dievaluasi kondisi depresi yang terjadi. Dari keseluruhan pasien berjumlah 24 pasien, didapatkan hasil berupa depresi minimal sebanyak 9 pasien (37,5%), depresi ringan sebanyak 12 pasien (50%), depresi sedang sebanyak 2 pasien (8,3%) dan depresi sedang-berat sebanyak 1 pasien (4,2%). Rerata usia pasien adalah 53,55±12,13 (dalam tahun), dengan rentang usia 30-73 tahun. Jumlah pasien pria sama dengan jumlah pasien wanita (masing-masing 12 orang). Rerata lama terapi adalah 17,78±11,88 (dalam minggu). Genotipe terbanyak adalah genotipe 1 (16 pasien, 66,67%). Manifestasi depresi yang terbanyak adalah kelelahan yang terus menerus dan penurunan nafsu makan. Penelitian membuktikan adanya gangguan depresi pada pasien-pasien hepatitis C yang mendapat terapi pegylated interferon alfa (pegIFN?). Penilaian kondisi mental sebelum memulai terapi dan setelah selesai terapi, perlu dilakukan secara rutin untuk menilai adanya efek samping yang perlu ditangani secara lebih lanjut.Kata Kunci: Derajat depresi, hepatitis C kronis, pegIFN?

Abstract

Prevalensi wanita dengan diagnosis Diabetes Mellitus (DM) meningkat sepanjang tahunnya termasuk dalam kehamilan. Kami laporkan kasus seorang wanita 36 tahun dengan persalinan anak pertama yang dikonsultasikan dengan peningkatan gula darah sesudah persalinan. Dari hasil anamnesa dan pemeriksaan fisik tidak didapatkan riwayat DM pada pasien dan keluarga yang dapat menjadi penyebab keterlambatan diagnosis DM pada kehamilan. Anamnesis, observasi dan hasil pemeriksaan laboratorium mengindikasikan potensi DM yang dimulai sebelum kehamilan namun tidak dikonfirmasi melalui skrining pemeriksaan gula darah pada kehamilan. Bayi yang dilahirkan mempunyai berat badan normal namun sempat mengalami hipoglikemia. Pasien diberikan terapi injeksi insulin basal dimulai pada dosis 6 iu dan dinaikkan bertahap menjadi 8 iu dan 10 iu, sesuai dengan hasil evaluasi hingga tujuan terapi tercapai. Kasus ini menunjukkan pentingnya penggalian riwayat dengan teliti dan skrining DM dalam kehamilan pada mereka yang berisiko untuk mencegah komplikasi pada janin maupun ibu. Abstract

The use of convalescent plasma has been beneficial for more than a century to treat various infections.  The lack of effective treatment currently available for COVID-19 has reemerged the use of convalescent plasma as a potential therapeutic option. This is an observational descriptive study in 13 COVID-19 patient who got convalescent Plasma. Consist of 11 (84%) male patient, 2 (16%) female patient. Divided into two groups based on severity, and outcome. 38% patient included in severe disesase and 62% patient in critically Ill condition. 30% patient who got convalescent plasma deceased, and 70% had a resolve condition.

Abstract

Background: Colorectal cancer (CRC) is one of the most common types of cancer worldwide. CRC is most typical symptom is hematochezia while the risk factors encompass sex, age, genetic predispositions, history of colorectal polyps or cancer, chronic inflammatory bowel diseases, physical exercise, and specific dietary choices, including high red meat intake, fried food, and low fiber consumption. This study aims to screen the risk factors of colorectal cancer.Subjects and Method: This research was conducted on residents in the Sepanjang village, Malang Regency, involved in community service by the Internist Association of Malang. Community service was carried out in July 2023 and 148 residents were involved in collecting data on risk factors for colorectal carcinoma. Data collection was carried out using a questionnaire with a cross-sectional approach. The questionnaire assesses the presence of symptoms of bloody stools and several habits related to colorectal carcinoma risk factors, including age, gender, exercise routine, and dietary history. Data presentation and analysis were carried out as in the table.Results: Among the risk factors listed assessed using the questionnaire, Men (p=0.021), lack of exercise (p= 0.008), consumption of fried food (p= 0.021), and consumption of instant noodles (p= 0.013) significantly affect the prevalence of hematochezia.Conclusion: Men, lack of exercise, consumption of fried food, and consumption of instant noodles significantly affect the hematochezia indicating CRC.
