Abstract

Background: Piriformis syndrome is a neuromuscular condition characterized by hip and gluteal pain caused by abnormality of piriformis muscle. In clinical practice, this syndrome is usually misdiagnosed with other spinal disorders. There is no specific test in diagnosing piriformis syndrome, therefore by understanding the characteristics and predisposing factors, clinicians may be more aware in diagnosing this syndrome.
Objective: To investigate the characteristics and predisposing factors of piriformis syndrome.Methods: A descriptive study was conducted on patients with gluteal pain in a referral hospital. Diagnosis was established by block injection on piriformis muscle and showed clinical improvement afterwards. Demographic characteristics, predisposing factors, and physical examination were analyzed. The data is collected and processed by SPSS 20.0.Results: The total of 65 patients were evaluated, with a male to female ratio 1:3. As much as 47.7% of patients were > 60 years old. Average weight was 60 kg and BMI (Body Mass Index) was within normal range (58.9%). Most subjects were housewives (55.4%) and the majority of subjects had history of microtrauma such as sitting on rigid surfaces (36.9%), walking or running long distances (18.5%), and cross-legged sitting (10.8%). Average sitting per day was approximately 5 hours. In 56.9% of subjects, exercises were conducted. There was no history of back pain in 81.5% of subjects.Conclusion: Our study showed age and housewives characteristics differ with other studies. The most predisposing factors were history of microtrauma, female, age, and normal BMI.

Abstract

Background Piriformis syndrome (PS) is one causes of buttock and hip pain which is caused by pressure of sciatic nerves on piriformis muscle. PS is sometime misdiagnosed because of similar clinical signs and symptoms with other lower back pain diagnosis such as Hernia Nucleus Pulposus (HNP), Sacroiliac joint pain, facet joint pain. Gold standard diagnosis of PS is diagnostic block injection. However, provocative test and prolonged H reflex on EMG might be also standard diagnostic of PS.Aim Understand the sensitivity and specificity of provocative test and prolonged H reflex to diagnose PS.Method 72 patients with suspected PS who came to Neurology OPD in period of August- December 2017 were testing with physical examination (provocative test) and Electromyography test of H reflex. PS is diagnosed by positive diagnostic block injection. Data was proceeded with SPSS 20 version.Result 72 patients with buttock pain which diagnosed with PS were dominated by female gender than male with ratio 1:3 with housewives as majority of work. Most age groups were 48.6% elderly (age more than 60 years old). Provocative tests result of Freiberg, FAIR, Beatty, Pace Sign, Hip Abduction sensitivity were 52.30; 66.15; 53.84; 46.15; 55.32 and specificity were 100; 42.85; 71.42; 71.42; 57.14. While sensitivity and specificity of prolonged H reflex more than 1.86 msec were 69.23 and 28.59. Combination of 3 provocative tests (FAIR, Freiberg and Beatty) resulted highest sensitivity and specificity in this study as 71.42 and 100.Conclusion Provocative test and prolong H reflex can be supported diagnosis of PS. However both tests might not be comparable yet than diagnostic block injection as gold standard of PS. Combination of provocative test increased the sensitivity and specificity of provocative test than single test only.

Abstract

Osteoarthritis (OA) merupakan penyakit degeneratif sendi yang paling umum terjadi di negara berkembang, termasuk Indonesia. Angka kejadiannya mencapai 15.5% pada laki-laki dan 12.7% pada wanita. Walaupun OA dapat menyerang seluruh sendi, namun sendi lutut merupakan sendi yang paling sering terkena. Tingginya angka prevalensi OA disebabkan karena peningkatan angka harapan hidup dan prevalensi dari obesitas, yang memicu disabilitas kronik serta peningkatan beban sosial dan ekonomi pada negara berkembang. Penanganan OA lutut merupakan suatu tantangan karena sendi lutut memiliki kemampuan regeneratif dan pemulihan yang rendah. Terapi konservatif seperti asetaminofen, obat anti inflamasi non-steroid, dan opioid hanya efektif dalam menangani nyeri dan mengurangi inflamasi. Salah satu terapi regeneratif yang sedang berkembang dalam menangani OA lutut adalah injeksi platelet-rich plasma (PRP), dimana merupakan produk darah autolog dengan konsentrasi platelet, faktor pertumbuhan, dan sitokin yang tinggi. Penggunaan PRP dapat menjadi alternatif karena tidak hanya mengurangi inflamasi, namun memiliki kemampuan untuk memulai dan meregenerasi jaringan yang rusak. Namun, walau PRP dapat dipertimbangakan sebagai terapi yang aman untuk penanganan OA lutut, masih terdapat pro dan kontra terkait teknik yang digunakan serta variasi komposisi dari PRP yang digunakan. Artikel ini akan memaparkan informasi terkini mengenai mekanisme dan efektifitas dari PRP dalam penanganan OA lutut.Kata kunci: Osteoartritis lutut; platelet-rich plasma; terapi regeneratif

Abstract

Background: Memory is a place where information is stored from the learning process or experience. There are several types of memory , one of them is short term memory. Declining sleep quality is directly proportional to the decrease in short-term memory. Poor sleep quality is often associated with medical student due to exams or vast amount of tasks. Therefore, researcher wants to see whether there is significant correlation between sleep quality and short-term memory function in students.Aim: To assess the association of the quality of sleep towards short term memory function of medical student of Pelita Harapan University.Methods: This study was conducted with a cross-sectional method, with taking sample using the method of a simple random sample. A total of 90 respondents at University of Pelita Harapan were taken. Data collected sorted out according to the inclusion and exclusion criteria. Quality of sleep assessed with PSQI questionnaire while short-term memory assessed by Digit span backward test. Results processed with SPSS version 24 and tested with Chi Square.Results: Data analyzed by Chi square test showed there are 33 students (58.9%) have poor sleep quality and short term memory function. There are also significant association between the quality of sleep and short term memory function (p value = 0.026)Conclusion: There is significant association between the quality of sleep and short term memory function of medical students of Pelita Harapan University.

Abstract

Background : Plantar fascia plays significant role in supporting the height and structure of Medial Longitudinal Arch (MLA). In flat foot, the MLA is depressed. There is thickening of plantar fascia reported in cases of plantar fasciitis in subjects with flat foot. Therefore, research needs to be done to investigate the relation between flat foot and plantar fascia thickness.Aim:  To understand the relationship between flat foot and plantar fascia thickness among medical students of Pelita Harapan University.Methods : This study was conducted using cross-sectional method with sampling method of non-random consecutive sampling. Data was collected through examinations performed and adjustments are made using inclusion and exclusion criteria. Flat foot was determined using navicular drop test and plantar fascia thickness was assessed using ultrasonography measurement. Results were analyzed using SPSS 22.0 and statistically tested using Spearman’s rho test.Result : The analysis of the relationship between flat foot and plantar fascia thickness showed positive correlation with correlation coefficient of 0.634 on the right foot and 0.443 on the left.Conclusion : There is moderate and strong positive relationship between flat foot and plantar fascia thickness among medical students of Pelita Harapan University.

Abstract

Background: Plantar fasciitis is a common problem caused by thickening of the plantar fascia. The normal plantar fascia thickness ranged between 2-3 mm and it was generally accepted that value more than 4mm was considered pathologic.Objective: to identify normal plantar fascia thickness in adults using ultrasonography.Methods: This is a cross sectional study measuring the thickness of plantar fascia in 145 subjects with no history of heel pain. Plantar fascia thickness was measured in both feet using an ultrasound. Age, height and weight were recorded and analysed.Results: As much as 145 subjects were included in this study. Male to female ratio was 0.7. Mean age was 44 and body mass index (BMI) was mostly within normal range. Plantar fascia thickness in male was 2.71 ± 0.48 mm in right foot, and 2.74 ± 0.47 mm in left foot. Fascia thickness in female was 2.55 ± 0.50 mm in right foot, and 2.57 ± 0.45 mm in left foot. There was a significant plantar fascia thickness difference between male and female (p = 0.035 in right foot, and p=0.04 in left foot). Age, weight and BMI had a significant correlation towards plantar fascia thickness. In multivariate analysis, age and BMI revealed to have a linear correlation to plantar fascia thicknessConclusion: Age and BMI were found to be the best predictive factor of plantar fascia thickness.

Abstract

REFERRAL PAIN DISTRIBUTION PATTERN AND TRIGGERING POSITION OF SACROILIAC JOINT PAINABSTRACTIntroduction: sacroiliac joint (sIJ) contribution to cause low back pain is still widely debated. sIJ is hardly distinguished from facet joint pain or other musculoskeletal disorder causing low back pain and leads to diagnostic difficulties. There are several factors that contribute to SIJ pain diagnosis, which are referral pain area and triggering position.Aim: To investigate the distribution pattern of referral pain and position that could trigger the sacroiliac joint pain.Method: Patients with complaints of pain in buttock area and diagnosed as sIJ pain by intra articular block diagnostic test at Neurology Clinic of of siloam Hospitals Lippo Village between August to december 2017 were interviewed to record the area of referral pain and position which triggered the pain.Results: 114 subjects with complaint of buttock pain, ratio between men and women was 1:2.35 (n=34 and 80), were included in this study. Posterior compartment of the thigh (19.3%, n=22) is the most common referral pain area followed by lateral part of the thigh (11.4%, n=13). Getting up from sitting position and long period of sitting were the two most dominant factors that could trigger pain (61.4%, n=70 and 49.1%, n=56).Discussion: The referral pain distribution pattern of sacroiliac joint pain was dominated by the posterior part and lateral part of thigh, while getting up from sitting and long period of sitting is found in majority of the subjects as the most dominant triggering position.Keywords: Pain pattern, sacroiliac joint pain, triggering positionABSTRAKPendahuluan: Kontribusi nyeri sendi sakroiliaka sebagai penyebab nyeri pinggang masih menjadi perdebatan. Secara klinis, nyeri sendi sakroiliaka sulit dibedakan dengan nyeri sendi faset lumbal maupun gangguan muskuloskeletal lainnya sehingga menyulitkan dalam diagnosis. Terdapat faktor yang memengaruhi terjadinya nyeri sendi sakroiliaka antara lain posisi tubuh yang mencetuskan nyeri dan area nyeri alih yang menyertainya.Tujuan: Untuk mengetahui pola distribusi nyeri alih dan faktor posisi tubuh yang mencetuskan atau memperberat nyeri sendi sakroiliaka.Metode: Pasien yang datang dengan keluhan nyeri bokong yang terdiagnosis mengalami nyeri sendi sakroiliaka berdasarkan tes diagnostik injeksi blok intraartikular di poliklinik saraf RS Siloam Lippo Village dalam rentang waktu Agustus–Desember 2017 dilakukan wawancara untuk mengetahui area nyeri alih dan posisi pencetus nyeri.Hasil: Sebanyak 114 pasien dengan keluhan pada area bokong, rasio antara laki-laki dan perempuan dengan perbandingan 1:2,35 (n=34 dan 80) diikutsertakan dalam studi ini. Paha bagian posterior merupakan area nyeri alih terbanyak (19,3%, n=22) diikuti area paha lateralis (11,4%, n=13). Sedangkan posisi tubuh yang paling dominan memicu timbulnya nyeri sendi sakroiliaka adalah posisi bangun dari duduk (61,4%, n=70) dan duduk lama (49,1%, n=56).Diskusi: Pola distribusi nyeri alih pada pasien nyeri sendi sakroiliaka yang paling banyak ditemukan adalah nyeri pada daerah paha sisi posterior dan paha sisi lateral sedangkan faktor yang paling dominan mencetuskan atau memperberat nyeri sendi sakroiliaka adalah posisi bangun dari duduk dan posisi duduk yang lama.Kata kunci: Nyeri sendi sakroiliaka, pola nyeri, posisi tubuh pencetus
