Abstract

Background:  Visitors take part in the occurrence of nosocomial infections. Their hands are a reservoir for microorganisms that can be transmitted to patients as individuals susceptible for secondary infections. Hand washing before and after visiting patients is the easiest way to remove colonies of microorganisms in the hands. The alcohol-based hand rub is available for visitors in the hospital’s rooms but there are no evaluation by hospital managemen.     Objective: This study aims to evaluate hand-washing behavior of visitors at a private hospital in Yogyakarta. Methods: Research method used descriptive quantitative survey. Sample was taken by accidental sampling from 2015 - 2017 and got 229 respondents. Data collection by observation in the room includes ICU, ICCU, surgical ward and post partum ward. Results: The result showed that percentage visitors who did not wash their hands before and after entering in the ICU was  50% and 83.33%, in the medical ward was 80% and 81.82%, in the ICCU was 81,73% and 70,19% and in the post partum ward was 57,5% and 30%. Conclusion: The high percentage of respondents who did not wash their hands before and after visiting patients is an important concern because they will carry microorganisms that can be transmitted to others. Suggestions for hospital are to campaign, make banners and leaflets for health promotion of the importance washing hands before and after visiting patients in the hospital for public. Abstract

Background: Selective reduction for multi-fetal pregnancies is a part of reproductive technologywhich developed to help infertile couples have healthy babies. That treatment is controversialbecause the purpose of the action is to improve quality of life but it is contrary with sanctity of life.The point about sanctity of life from various religions to be a moral and it is believed of everyone.Objective: This paper aims to explore the associated ethical issues of quality of life versus sanctity oflife as well as some benefits and harms of selective reduction for the mother, fetuses and community.Method: Literature search of articles between 1996-2016. Results: In vitro fertilization is a commonresult of multiple gestations. In order to get healthy baby/babies, in the United Stated selectivereduction for fetus/fetuses who have bad condition should be done, even though it is a risky treatment.On the other hand, it is contrary with sanctity of life views from four religions including Catholic,Islam, Buddhism and Hinduism. Conclusion: The procedure of selective reduction has benefits andharms to both mother and fetus/fetuses. It is related to morbidity and mortality. The treatment alsobrings the impact for community. This is a difficult decision for couples because their desire is incontrast to moral principle views.

Abstract

Backgrounds: One significant problem in hospital is Health Care Associated Infection (HAI’s)  and it gives many effects on  patients health status.Purpose: This study was aimed to examine the incidence rate of phlebitis, the procedures of IV insertion and the factors that influence the phlebitis case in-patient department at the hospital.Methods: The main method used in this study is descriptive explorative. Started by giving training education for 15 nursing staff , medical doctor and pharmacists. Then they were obseved in doing the procedures of  IV incertion, care and documentation by Using Visual Infusion Phlebitis ( VIP) scale then followed by case studies to have clear results in describing the contribution factors which influence the phlebitis incidents.Result: 35 subjects in the experimental group and 35 subjects in the control group completed The result represents that 7 from 72 patients (9,7%) were suffer from phlebitis, 25 patients (34,7%) with IV insertion were not monitored well,  5% nurses did not do the hand washing, 78% did not use the skin perlak for preventing blood to the bed linen. The mode of desinfection direction were 44% circular, 55 % from distal to proximal part, and 11% did not do the desinfectant. 100% the nurses did not documentate the IV monitoring such as the swelling, redness, the size of IV canule and any other skin symptoms of phlebitis. Some medication, insertion side of IV canule on the metacarpal area and flushing procedures were also determinant factor that lead to phlebitis cases.Conclusion: This study conclude that the treatment, care and monitoring IV insertion is a significant and important procedures to prevent phlebitis and automatically becomes an important aspect in preventing Health Care Associated Infection (HAI’s)  rate in hospital.

Abstract

Background: Ventilator-Associated Pneumonia (VAP) is one kind of nosocomial infection that happens to patients who use mechanical ventilator and tracheostomy. Patients who are hospitalized in Intensive Care Unit (ICU) would have high risk onsuffering from Pneumonia.Objective: This paper aims to describe the interventions of nurses and to give recommendations for managing the VAP prevention using case study method.Methods: The data were collected from a patient who used mechanical ventilator and continued by observing and interviewing the nurses who took care of the patient.Results: The development of late onset of VAP was noticed after 96 hours. The observation result showed that there were fungi appeared after conducting a sputum culture from the patient. There was an increase of White Blood Cells on 72 hours after the intubation and the temperature increased until 37o C while the patient was using ventilator. Unfortunately, the nurses were not aware of the tendency of late onset VAP development in the patient.Discussion: Fungi are microorganism which causes VAP. The fungi were not proliferating because the patient had received early antibiotics for treatment. Early administration of antibiotics will prevent the development of VAP, but late VAP is associated with antibiotic-resistant organism. The significant increase of WBC count and the tendency of temperature while the patient was using ventilator were signs for developing infection. VAP can be prevented by conducting good oral hygiene every 2 hours using chlorhexidine 0.12% and using orogastric tube and ETT with sub glotticport to patients who use mechanical ventilator. Furthermore, positioning the patient’s head with 35o - 45o bed elevation is intended to prevent aspiration as well as checkingadequate endotracheal tube cuff of at least 20 cm H2.Conclusion: Awareness, responsibility, knowledge, skills of nurses are basis forinfection control especially for preventing VAP incidents.

Abstract

Introduction: Chronic disease often cause patients feeling anxious in which it give negative consequences on their physical, emotional, social, intelectual and spiritual condition. Nurse can applay complementary therapy such as healing gardens to reduce patient anxiety level. Healing gardens can give comfortable situation, therfore it can improve relaxtation and reduce stress that can make hospitalization shorter. Some researres, results show that healing gardens can improve relaxtation and reduce patients anxiety who are sick.Purpose: To identify the relationship between healing gardens toward reducing chronic patients anxiety level in one of Private Hospital in Yogyakarta.Methode: Research design which was done was observation. Population in this research were all chronical hospitalized patient in in one of Private Hospital in Yogyakartaa from August to October 2015. Total sample in this research were 30 respondens who are choosed randomly.Result: The test result of data analysis on the difference of before and after experiencing healing gardens showed p value 0.000 (p value < 0.05), means that healing gardens can reduce chronical panients anxiety level.Conculsion and suggestion: Healing gardens can reduce anxiety level in chronic illnes patients. Hospital should applay healing gardens toward chronical patients in order to reduce patient anxiety level. For nurse, they also should give and touch to patients who were doing healing gardens where these can psychologically can improve their safety and comfortable so it can reduce patients anxiety level. Key words: healing gardens, anxiety, chronic disease.

Abstract

ABSTRAK Covid-19 menjadi permasalahan pandemic global, tingkat penyebaran dan keparahan menjadi menghawatirkan dari virus tersebut. Kabupaten Sleman merupakan salah satu wilayah yang mengalami transmisi lokal. Penularan virus Corona dapat terjadi dengan mudah dari satu orang ke orang lain. Tujuan kegiatan ini adalah untuk meningkatkan pengetahuan warga masyarakat Padukuhan Pringwulung tentang pencegahan penyebaran Covid-19. Metode yang digunakan adalah penyuluhan, pelatihan dan diskusi. Kegiatan ini menghasilkan peningkatan pengetahuan dan kemampuan untuk membuat face shield. Kesimpulan: Kader mampu memahami tentang pencegahan penyebarab Covid-19 dan mampu membuat face shield secara mandiri.Kata Kunci: pencegahan penyebaran, Covid-19, pengetahuan   ABSTRACT Covid-19 is a global pandemic problem, the level of spread and severity of the virus is worrying. Sleman Regency is one of the areas experiencing local transmission. Corona virus transmission can occur easily from person to person. The purpose of this activity is to increase the knowledge of the residents of the Padukuhan Pringwulung community about preventing the spread of Covid-19. The methods used are health education, training and discussion. This activity resulted in increased knowledge and the ability to make face shields. Conclusion: Cadres are able to understand about preventing the spread of Covid-19 and are able to make face shields independently.Keywords: prevention of spread, Covid-19, knowledge
