Abstract

One of the important aspects of development in children is cognitive development which strengthens children's reasoning, how children manage information, how children overcome curiosity, children's self-management of difficulties encountered when they want to explore their high curiosity, apart from providing knowledge about things. -things around can also help children in terms of exporting their surroundings. The aim of this research is that children can sharpen their memory, intuition, focus and be able to solve problems independently using the monoplay box game media. The qualitative method is the method used to observe the subjects studied, namely 21 Kindergarten B1 children at Wisma Sari Kindergarten Denpasar. In accumulating the data used, namely observation, interviews and checklists. The gains obtained from this research can be seen from the Pre- Simulation period, 1st period, 2nd period and 3rd period after using the Monoplay Box game in the observations that have been made, it can increase the cognitive development of the aud through the Monoplay Box game with the results experiencing a significant increase.

Abstract

Diare merupakan suatu kondisi dimana seseorang buang air besar dengan konsistensi tinja yang lembek atau mencair bahkan dapat berupa air saja dan frekuensinya lebih sering (3 kali atau lebih) dalam sehari. Diare merupakan penyakit yang berbasis lingkungan yang faktor utama dari kontaminasi air atau tinja berakumulasi dengan perilaku manusia yang tidak sehat. Penelitian ini menggunakan penelitian kualitatif yang bersifat deskriptif. Penelitian ini dilakukan di pukesmas ofa padang mahondang kec. Pulau rakyat kab. Asahan. Hasil penelitian ini menunjukkan ada 5 desa yaitu desa ofa padang mahondang, padang mahondang, persatuan, bangun, dan tunggul 45 yang penderita penyakit diare tercatat paling banyak, yang di laporkan pada tahun 2021 sebanyak 253 orang lebih sedikit di banding tahun 2017 sebanyak 480 orang, di pukesmas ofa padang mahondang. Berdasarkan hasil penelitian dapat di simpulkan, Peran dari tenaga kesehatan sangat dibutuhkan agar program diare dapat berjalan secara optimal. Di Puskesmas ofa padang mahondang, tenaga kesehatan yang terlibat dalam mendukung pelaksanaan program diare tidak hanya petugas diare, promkes dan tenaga kesehatan lingkungan selain itu pemerintah juga berperan dalam pelaksanaan program

Abstract

Recording of medical record files must meet the minimum standards of hospital services. The purpose of this study was to analyze the completeness of medical record documents for inpatients at RSUD DR. Pirngadi, Medan City. This study used a quantitative method with a descriptive approach, the data analysis used was a univariate test. Of the 540 medical record files, there were 131 incomplete medical record files which were used as the population and used a sample of 99 medical record files. The research was conducted on January 9-February 17, 2023. The results showed that of the 99 medical record files on the patient's medical resume items, the majority 75 (75.8%) were complete, the minority 24 (24.2%) were incomplete, assessment initial medical majority 88 (88.9%) complete files, minority 11 (11.1%) incomplete, majority nursing assessment complete 99 (100%), cppt majority 83 (83.8%) complete files, minority 16 (16.2%) were incomplete, the majority of hand over forms were 99 (100%) complete, the majority of medical cards were 87 (87.9%) complete, the minority were 12 (12.1%) incomplete and the majority of nutritional care forms are complete 99 (100%). Based on the results of the study, it can be concluded that the incomplete inpatient medical record files at RSUD Dr. Pirngadi Medan City were affected by PPA's lack of compliance and discipline in filling out medical record files. It is expected that all PPAs can write down all examination actions in the patient's medical record file completely.
