Abstract

Postoperative fever is a fever that occurs after surgery and might develop among children, adults, and the elderly indirectly related to the type of surgery. Surgical site infection (SSI) is the most common cause of postoperative fever. However, it is common that the specific causes of fever are not found or without any signs of infection. This study aimed to determine the cause of postoperative fever and its intervention plan. This research was a case study at X Hospital in September to October 2019 initiated by identifying the root of the problem, identifying the priority of the root problem, and planning the intervention. The results of the identification of the root of the problems found that fever occurred less than 24 hours postoperatively. The results of the root problem identification revealed no SOP that regulates the Professional Care Providers (PCP) to give instructions to patients if there is a delay in the operating schedule. The proposed intervention plan is to make regulations regarding service delay instructions. Postoperative fever less that is than 24 hours causes patient discomfort due to the absence of instructions from the PCP if there is a delay in the operating schedule. The establishment of the regulations is a solution to reduce the incidence of postoperative fever.Keywords: postoperative fever, operation delay, regulation

Abstract

The surgery rooms have an important role in the hospital, so it must be managed properly. The surgery rooms consume 9% of the annual hospital budget. High operational costs caused by the large cost of human resources, medical materials, and other supporting facilities. This study was conducted in a private hospital type C with 181 beds and has four surgery rooms. The number of patients which was operated in X Hospital was very high, and the highest number in July 2019 reached 515 patients. There were a large supply of drugs and medical devices in the surgery rooms.  The number of each drug reached hundreds ampoule, and medical devices reached tens and some in the hundreds. The management of pharmaceutical supplies in the surgery rooms was carried out by senior nurse head of surgery rooms. Monitoring logistic by pharmacy officer only done once a month. This made excessive of stock and become expired. The purpose of this study was to analyze the factors that cause the lack of pharmaceutical logistics monitoring in the surgery rooms. Determination the root of the problem was done by using a fishbone diagram through a Focus Group Discussion with the division heads and subdivision heads of X Hospital. Focus Group Discussion is done by asking the participants one by one to find the root of the problem based on five factors (man, material, method, machine, and environment). Obtained 8 root problems then analyzed with 5 why to get the main root problem. Furthermore, determine the biggest problem using the CARL (capability, accessibility, readiness, and leverage) method, with a weighting score between 1-5. The results found that the absence of pharmaceutical logistics monitoring SOP in the surgery rooms was the most contributing factor to the lack of pharmaceutical logistics monitoring in the surgery rooms.

Abstract

The first confirmed Corona Virus Disease 2019 (Covid-19) case in Indonesia was found in March 2020 and rapidly spread out across all provinces. Covid-19 hit unexpectedly that almost all hospitals around the world were unprepared for it. Several studies show that Covid-19 impacts negatively on hospital finance, the mental health of healthcare workers (HCWs), and the death of HCWs. Hasta Brata Hospital (HBH) is one of the hospitals in East Java affected by the pandemic. Therefore, this study aims to investigate the experience of HBH staff face the issues. This study was qualitative research with a phenomenological approach. The data were collected using in-depth interviews with two HBH’s staff. ACovid-19 pandemic causes problems to the hospital, especially the staff. They experienced deep emotional and physical pressure at work, such as fear of infection, repeated conflict with patient's visitor, having different mindsets among staff, and increasing workload. The situation in the community is also frustrating them as the Covid-19 cases keep rising, while people tend to disobey the Covid-19 prevention protocols. Fortunately, they have proper job resource, started from physical support such as the provision of personal protective equipment, nutritional enhancement, and periodic Covid-19 screening; cognitive support such as knowledge update about Covid-19 and its handling methods; to psychological support such as motivational and spiritual support from the HBH leader. Moreover, they also pose strong personal resources that help them to cope with stressful working situations. They could, then, bounce back, feel optimistic that they could fight against Covid-19, with the hope that the pandemic will have an end. Continuous spiritual support gives them another personal resource of tawakkal, an Islamic concept of giving entire resignation and reliance on God only after exerting maximum effort. It seems that job resources and personal resources have succeeded in reducing the highly demanding situation and transform it into positive outcomes. Given a challenging situation, the hospital staffs strive to perform well and bring themselves into their work when providing patient care. Covid-19 pandemic forced the hospital and staff to adapt to the condition quickly. The presence of job resources and the provision of strong personal resources have succeeded in moderating job demands and resulting in positive outcomes. Tawakkal becomes an adding point on staff's personal resources that made them realize that Covid-19 is inevitable, and they have to fight back before finally giving their full resignation to God.

Abstract

The Covid-19 pandemic has brought negative consequences on hospital finances, the mental health of health care workers (HCWs), and even resulted in the deaths of several health workers at Hasta Brata Hospital (HBH). This study aimed to explore the experiences of HBH's staff in dealing with the pandemic in the hospital. This research is qualitative research with a phenomenological approach. Data were collected using in-depth interviews with two of HBH's staff and then followed by the theme analysis. Hospital staff experienced job demands including emotional and physical stress at work, fear of infection, repeated conflicts with patient visitors, different mindsets among staff, increased workload, and feelings of frustration towards people who do not comply with prevention protocols. Fortunately, they had adequate job resources, ranging from the provision of personal protective equipment, nutrition improvement, and regular Covid-19 screening; update knowledge and methods of handling Covid-19; to motivational and spiritual support from HBH leaders. The staff also had strong personal resources so that they could cope with stressful work situations, feel optimistic that they could win against Covid-19, and hoped that the pandemic would end. Spiritual support embodies tawakal, an Islamic concept that gives complete surrender to God after employing maximum effort. The presence of strong job and personal resources has moderated job demands and resulted in work engagement. Tawakal is an added value for personal resources staff who finally realize that Covid-19 cannot be avoided and must be fought before surrendering entirely to God.

Abstract

During the COVID-19 pandemic, up-to-date information becomes a necessity for the public, and social media, especially Instagram, play a significant role in digital marketing, including hospitals. The Instagram account of a private hospital in Batu City had a decreasing performance and has become inactive since the Covid-19 pandemic outbreak. This study aimed to find a solution to the decreasing performance of the hospital's Instagram account to increase visits, trust, and patient satisfaction. This research was a quasi-experimental study with a pre- and post-test design without control conducted from September to October 2021. The intervention carried out was conducting training on optimizing the Instagram features. The research sample was ten people obtained through the total sampling method. The data were analyzed using a descriptive analysis test. The training was carried out twice in a period of one week. The first training session discussed the basic features of Instagram and their use, and discussions on the technical aspects of content creation were done in the second session. Measurement of training understanding was carried out by pre-test and post-test. All points in the questionnaire showed positive points of increasing knowledge after the training. Instagram optimization training at the hospital in Batu City can improve staff understanding of digital marketing and is expected to expand the business and introduce the hospital to new patients.

Abstract

Recording and reporting are parts of health services aiming to create administrative discipline to improve health service quality. Covid-19 pandemic prevalence has impeded unit quality indicator recording and reporting, causing suboptimal quality measurements. This study aimed to see the implementation of Google Forms as a solution to overcome obstacles in unit quality indicator recording and reporting. This study was a one-group pre-experimental with a pre-and post-intervention design without control. The sample was seven people who were Laboratory Unit staff of a Private Hospital in Batu City. The results showed that applying Google Forms can provide staff's ability and timeliness in measuring unit quality indicators. A good quality measurement method can increase the effectiveness of monitoring and evaluation related to patient-centered quality improvement.

Abstract

Praktik outsourcing telah menjadi tren yang semakin umum terjadi di berbagai sektor industri. Alih daya (outsourching) adalah penggunaan tenaga kerja untuk menghasilkan atau melakukan pekerjaan oleh suatu perusahaan, melalui perusahaan yang menyediakan pekerja/buruh. Penelitian ini bertujuan untuk 1) Menilai keunggulan kompetitif lulusan SMK jurusan Akomodasi Perhotelan dalam menghadapi persaingan di pasar kerja yang dipengaruhi oleh praktik outsourcing, 2) Mengidentifikasi dan memanfaatkan peluang kerja alternatif bagi lulusan SMK jurusan Akomodasi Perhotelan di tengah tren outsourcing di industri perhotelan, 3) Menyusun strategi yang dapat diterapkan oleh lulusan SMK jurusan Akomodasi Perhotelan untuk meningkatkan daya saing mereka di pasar kerja yang dipengaruhi oleh praktik outsourcing.  Kajian ini diteliti menggunakan pendekatan kualitatif deskriftif melalui metode observasi, wawancara, dan studi dokumentasi. Adapun hasil dalam penelitian ini yakni  a) Lulusan SMK  jurusan Akomodasi Perhotelan telah belajar khusus dan mendalam dalam berbagai aspek operasional hotel, yaitu front office, housekeeping, pelayanan tamu, dan manajemen acara. Mereka memiliki keterampilan praktis yang sesuai dengan kebutuhan industri perhotelan, yang membuat mereka menjadi kandidat yang sangat diinginkan oleh perusahaan hotel maupun  perusahaan lain yang mencari spesialis dalam bidang akomodasi. b) Lulusan SMK juga memiliki potensi untuk menjadi pengusaha atau pemilik usaha dalam industri perhotelan atau sektor-sektor terkait. Mereka dapat memulai usaha sendiri seperti bisnis jasa pembersihan, agen perjalanan, layanan katering, atau usaha kecil lainnya yang berkaitan dengan akomodasi dan perhotelan. c) Lulusan SMK dapat meningkatkan daya saing mereka dengan memperdalam pengetahuan dan keterampilan dalam penggunaan perangkat lunak manajemen hotel, sistem reservasi online, atau aplikasi pelacakan inventaris. Kesimpulan dari penelitian ini adalah praktik outsourcing semakin umum terjadi di berbagai sektor industri, termasuk Bank, Rumah sakit, dan Kantor pelayanan publik. Lulusan SMK jurusan Akomodasi Perhotelan memiliki keunggulan kompetitif yang signifikan dalam memenuhi kebutuhan pasar kerja yang dipengaruhi oleh praktik outsourcing.
