Abstract

Structural Equation Model (SEM) is a statistical technique with simultaneous processing involves measurement errors, indicator variables, and latent variables. SEM is used to test hypotheses that state the relationships between latent variables when latent variables have been assessed through each of the indicator variables. Multiple Group SEM is a basic model analysis that uses more than one sample. This analysis aims to determine whether the components or models of measurement and structural models are invariant for the two sample groups. In this study, the data generated by some requirements. First, the data generated with sample size n = 250. The first generated data is homogeneous data where the measurement model is the same as the structural model in group 1 and group 2, while the second data is non-homogeneous data where the measurement model and the structural model in group 1 and group 2 is not the same. The data was analyzed using the help of the lavaan package available in R to obtain SEM estimation results and Goodness of Fit Model from some data that was formed. From the results of the merger of the two groups, it shows that the invariant of the two models with the largest df (63) which is Fit Mean model states the simplest model. However, the smallest df (48) with Fit.configural model states the most complex model. Keywords: SEM, Multiple Group, R Program

Abstract

Length of stay (LOS) of a patient in an Emergency Room (ER) is the interval between the time the patient arrives and the time the patient physically leaves the ER. Prolonged LOS has potential to cause crowding. Meanwhile, from a clinical point of view, the factor of LOS has an impact on service suspension, which can reduce patient safety, even to death. Covid-19 pandemic has made hospitals establish a strict patient screening system and has the potential to increase the LOS of patients in the ER. The purpose of this study was to identify the factors that contribute to the LOS of patients. This study is an observational study of 38 respondents who came to the Emergency. LOS was compared between before and during Covid-19 pandemic condition. Observations were carried out by the Time Motion Study method to determine the patient's average time at each stage of the ER. The results showed the average LOS for emergency room patients with inpatient plans was 2 hours 7 minutes, while the LOS during pandemic was 4 hours 46 minutes 52 seconds. There were 2 hours 39 minutes 52 seconds time extension. The time extension is due to several factors: results of laboratory tests SARS COV-2; approval process for the Covid-19 Screening Team; Emergency Room and Laboratory were located in separate buildings.

Abstract

Undang-Undang Nomor 6 tahun 2014 tentang Desa memberikan tanggungjawab yang besar bagi pemerintah desa, namun pemberian tanggungjawab yang besar tersebut tidak dibarengi dengan penyiapan sumber daya manusia yang baik, sehingga tata kelola pemerintahan yang baik dilingkungan pemerintahan desa masih jauh dari harapan. Hal ini terlihat dari masih adanya aparat desa yang kurang disiplin dalam menjalankan tugas dan tanggungjawabnya dan di sisi lain, pemberian otonomi desa melahirkan praktik korupsi baru di tingkat desa. Hasil penelitian menunjukkan bahwa prinsip good governance telah diterapkan dalam penyelenggaraan pemerintahan desa, sedangkan faktor yang menghambat pelaksanaan good governance dalam penyelenggaraan pemerintahan desa adalah partisipasi masyarakat, sumber daya manusia, dan disiplin aparat desa. Adapun faktor pendukung penerapan good governance dalam penyelenggaraan pemerintahan desa adalah adanya kerjasama dan komunikasi. Kata Kunci: pemerintah desa, tata kelola, good governance

Abstract

Extended length of stay (LOS) in the Emergency Department (ED) results in crowding, potentially causing adverse events and patient dissatisfaction. The Covid-19 pandemic has forced hospitals to implement screening procedures that can extend the LOS of patients in the ED. This study aimed to provide an overview of changes in ED LOS during the pandemic and its contributing factors. This study used a descriptive approach through observation and interviews. LOS observations were conducted using the time & motion study method to determine the average time in each ED stage. The observation results identified the differences in the average LOS during the Covid-19 pandemic, which was 2 hours 39 minutes 52 seconds, and the longest time was on laboratory examination (2 hours 59 minutes 27 seconds). A lengthy laboratory examination duration is due to the additional laboratory examination procedure (serology) for SARS COV-2, suboptimal screening team, and the absence of notification in the hospital information system.

Abstract

Medication error becomes one of the main concerns in ensuring patient safety. In 2020, X Hospital reported medication error incidents. Although categorized near miss, the incidents should have been prevented. One of the methods to prevent medication errors is by applying the seven rights of medication administration. This study aimed to identify the inhibiting factors in the implementation of the seven rights of medication administration in X Hospital and obtain alternative solutions to those obstacles. This study used an analytic observational design with a cross-sectional design. Data were collected from observations and a survey. Observations and a survey through questionnaires to 20 nurses at X Hospital were made to see the implementation of the procedure. At the end of the data collection, interviews were conducted to find out the obstacles in the inpatient installation in applying the seven rights of medication administration. The results show that the main root causes of medication administration errors were the standard operating procedure (SOP) that has not been standardized and nurses' non-compliance behavior on the seven rights procedure. To resolve medication administration errors, the hospital manager should revise the SOP, conduct internal workshops, and perform scheduled supervision to the nurses.

Abstract

The Covid-19 pandemic has brought negative consequences on hospital finances, the mental health of health care workers (HCWs), and even resulted in the deaths of several health workers at Hasta Brata Hospital (HBH). This study aimed to explore the experiences of HBH's staff in dealing with the pandemic in the hospital. This research is qualitative research with a phenomenological approach. Data were collected using in-depth interviews with two of HBH's staff and then followed by the theme analysis. Hospital staff experienced job demands including emotional and physical stress at work, fear of infection, repeated conflicts with patient visitors, different mindsets among staff, increased workload, and feelings of frustration towards people who do not comply with prevention protocols. Fortunately, they had adequate job resources, ranging from the provision of personal protective equipment, nutrition improvement, and regular Covid-19 screening; update knowledge and methods of handling Covid-19; to motivational and spiritual support from HBH leaders. The staff also had strong personal resources so that they could cope with stressful work situations, feel optimistic that they could win against Covid-19, and hoped that the pandemic would end. Spiritual support embodies tawakal, an Islamic concept that gives complete surrender to God after employing maximum effort. The presence of strong job and personal resources has moderated job demands and resulted in work engagement. Tawakal is an added value for personal resources staff who finally realize that Covid-19 cannot be avoided and must be fought before surrendering entirely to God.

Abstract

Hospitals have an important role in improving health status by providing quality services, including complete medical record data and information. Complete and accurate medical records can be used for health care and patient treatment, evidence in law enforcement, medical discipline and medical ethics, educational and research purposes, basis of payment for health care costs, and health statistical data. This study aimed to identify the factors causing the incomplete filling of inpatient medical records at Dr. H. Slamet Martodirdjo Regional General Hospital Pamekasan. This research applied the descriptive observational method, with data collected through secondary data analysis using document studies and problem exploration using focus group discussions (FGD). The document study was conducted on 80 samples of inpatient medical records, while the FGD was conducted by inviting the head of the planning department, medical records officer, and 8 heads of inpatient wards to find out the causes and solutions for incomplete medical record files. The study results found that the number of incomplete medical records was 76.25%, with the highest percentage of incompleteness due to doctors, pharmacists, and nurses. The main problem causing the incompleteness of medical records was professional caregivers (PPA) who lacked discipline. The solutions obtained were management meetings and evaluations to professional caregivers, constructing policies that regulate reward and punishment systems, monitoring the checklists of medical record completeness, and routine evaluation of medical record files.

Abstract

All medical procedures on patients must be initiated by completing the informed consent. According to the hospital's minimum service standard, the completeness of informed consent filling is 100%, but the achievement of informed consent completeness at Bangil Regional General Hospital is still 37%. This study analyzed the causative factors of incomplete informed consent at Bangil General Hospital. The data analysis method used was descriptive. Data collection techniques were document studies and focus group discussions (FGD). Document studies were conducted on 100 informed consent documents, and FGD was done with 17 people consisting of 7 physicians-in-charge and 10 installation heads. The results of the FGD were analyzed using a fishbone diagram and revealed a list of problems that caused incomplete informed consent at Bangil General Hospital; then, scoring was carried out to determine the three main factors driving the problem of incomplete informed consent at the hospital. The three main factors for incomplete informed consent were the absence of a flow or policy guide for filling out, the unavailability of informed consent documents in outpatient installations, and the absence of a person in charge of the room for filling out informed consent.

Abstract

This studies audit modified into carried out to find out the general safety, manage, and conservation efforts that had been carried out to provide criminal guarantees, preserve sustainability, and increase the outcomes of the participatory trends that have been set up via PNPM and facilitated thru the guarantees). The trouble confronted thru BKAD is that it is a sustainable arrears from the SPP group in Banyuasin. This studies takes up the issue via the use of investigating the location of BKAD in defensive, coping with, and keeping the consequences of the implementation of PNPM in Banyuasin. The qualitative qualitative technique have become selected to study the trouble this is being investigated. The research statistics had been amassed through in-intensity interviews, observations, and documentation. The self-discipline of informants was done with the aid of way of purposive sampling and snowball sampling. Records is verified via regular participation, declaration, member checking, and triangulation. The information had been analyzed qualitatively, which consisted of facts collection, facts reduction, data presentation, and statistics verification. The research findings cowl three problems. First, BKAD takes safeguards aimed at inventorying SPP assets and properties through discussions. 2nd, BKAD carries out a manage software by using manner of restructuring consumer loans, taking element with all the corporations worried in PNPM. 0.33, the conservation efforts completed with the aid of using BKAD have resulted in sustainable SPP.

Abstract

Di Provinsi Sumatera Selatan, tepatnya di OKU Timur, hanya tersedia fasilitas pendidikan tingkat PAUD dan Sekolah Dasar. Tidak ada sekolah menengah pertama atau sekolah menengah atas; Meningkatnya jenjang pendidikan setelah SD membuat banyak siswa SD di OKU Timur tidak berminat untuk melanjutkan ke jenjang berikutnya karena jarak dan biaya yang harus ditempuh untuk sampai ke sekolah sasarannya. Oleh karena itu, minat membaca siswa menjadi berkurang, terutama karena sarana dan prasarana perpustakaan sekolah belum memadai. Metode yang digunakan adalah pendekatan individual yang dilakukan secara berkelompok menjadi tiga bagian (timedu-support, tim motivasi, dan tim bimbingan konseling). Beberapa hasil ditemukan dari tiga aspek: (1) Tim Motivasi, ditemukan perbedaan tingkat minat belajar siswa; (2) Tim Penyuluhan, membentuk organisasi kepemudaan untuk menggerakkan kegiatan masyarakat OKU Timur, seperti lomba, pengajian rutin, gotong royong, dan sebagainya; (3) Tim Pendukung Edu, memperbaiki 3 sarana pendidikan yaitu perpustakaan sekolah, taman baca desa dan lapangan serba guna.
