Abstract

Latar belakang : Human Immunodeficiency Virus (HIV) merupakan salah satu penyakit infeksi yang masih menjadi masalah kesehatan dunia yang utama hingga saat ini dan Indonesia merupakan negara endemitas tinggi. Peningkatan prevalensi infeksi HIV baik secara nasional maupun regional menggambarkan penularan HIV yang cepat dan progresif. Penyakit ini dapat menimbulkan berbagai manifestasi klinis. Pada umumnya infeksi HIV tidak menimbulkan gejala spesifik, hal ini berarti terdapat kemungkinan risiko orang yang menularkan tetapi tidak menyadarinya. Pencegahan adalah satu-satunya perlindungan terhadap epidemi HIV dan pelajar adalah salah satu kelompok terbaik yang untuk memulai pendidikan kesehatan mengenai infeksi HIV. Metode : Penelitian ini menggunakan metode kuantitatif dengan metode pengambilan data secara cross-sectional menggunakan kuesioner. Responden pada penelitian ini dipilih melalui tekhnik cluster sampling. Analisis statistik yang digunakan adalah analisa univariat serta analisis bivariat menggunakan metode korelasi Spearman. Hasil : Sebanyak 70 responden yang berpartisipasi dalam penelitian ini. Pada variabel pengetahuan responden, didapatkan responden sebagian besar memiliki pengetahuan yang baik yaitu sebanyak 34 orang (48,6%). Pada variabel tingkat perilaku pencegahan responden, mayoritas didapatkan responden yang memiliki tingkat perilaku baik yaitu sebanyak 51 orang (72,9%). Nilai korelasi menunjukkan terdapat hubungan bermakna (p<0,05) dengan arah hubungan yang positif dan sedang antara pengetahuan responden dengan perilaku pencegahan responden (r = 0,424). Kesimpulan : Terdapat hubungan yang positif antara pengetahuan tentang infeksi HIV terhadap perilaku pencegahan HIV pranikah pada santri di Pondok Pesantren Abu Hurairah Mataram.

Abstract

Right Bundle Branch Block (RBBB) is an abnormality in the physiological electrical conduction system of the heart, resulting from a blockade of the right bundle branch. The global incidence of RBBB is estimated to be approximately 0.8% annually. Typically, RBBB is considered a mild finding that does not increase the risk in otherwise healthy individuals without structural heart disease. However, RBBB may also be indicative of myocardial disease and has been identified as a predictor of mortality in certain populations. This literature review aims to summarize the pathophysiology and clinical approach to RBBB, with a particular focus on echocardiography (ECG) examinations, based on literature sourced from PubMed, Google Scholar, and additional manual searches conducted between 2016 and 2021. Additionally, the author examines the management, prognosis, and complications associated with RBBB.

Abstract

Introduction : The head and neck cancer are sixth most common cancers worldwide with cancer of oral cavity. The basic treatment modality for the carcinoma of oral cavity has been surgery and the large defects resulting from the ablation of the tumors require reconstruction. Reconstruction after wide excision of oral cancer challenge in itself, because it requires an appropriate method of reconstruction, in accordance with oncology rules and while maintaining the function . In this case serial, several methods of reconstruction from oral cancer are discussed. Case Report : We report 3 cases of patients female aged 48-71 years with complaints of lumps on the tongue. Patients with risk factors poor oral hygiene. On physical examination, cervical lymph node enlargement was found at levels I-IV. All patients underwent a CT scan of the head and neck. On histopathological examination, squamous cell carcinoma was found, the stage of oral cancer in this case series was stage I-III. The surgery was performed with a combined mandibular and neck dissection operation (Commando) with LD flap, and SCM flap. The patient is then treated in the intensive care unit for 1-2 days, then treated in the usual room and discharged on the 5-7th day of treatment without complications Discussion : Although there is increasing use of microvascular free flap in this era, however, conventional flap like LD and SCM still remains very reliable and versatile myocutaneous flap for primary as well as secondary reconstruction, In head and neck reconstruction, LD and SCM flap is used extensively due to its proven reliability and multipurpose nature . The main advantage of LD and SCM flap is that it can be used in a very wide range of defects in the head and neck region including the maxilla, neck as well as tempero-orbital area. Like the other myocutaneous flap, LD and SCM is harvested by single stage reconstructive procedure and does not require any flap delay or release. The morbidity of the donor site is surprisingly low, however, few patients may complain pain. It is also very useful in the patients with poor clinical conditions as well as for elderly patients. This flap can be used in the cases where there is inadequate recipient vessels for microanastomosis in the neck of the patients who underwent high dose radiotherapy or when there is contraindication to free flap such as medical conditions that make the patients unable to tolerate long surgical procedures or can be used as a salvage procedure after necrosis of free flap. wound dehiscence, infection, vascular rapture with marked increase in the morbidity. There is less mentioning of the incidence of recurrence at flap harvesting site however, few cases are reported in the literature. Conclusion : Reconstruction of composite oro mandibular defects of oral malignancy is a challenging problem for oncology surgeons. LD and SCM myocutaneous flap was found to be a versatile flap for reconstruction of large defects in the head and neck region with minimal complication rate. In today’s world of micro vascular flaps, in spite of associated complications, loco regional flaps play a crucial role at oncology centers. LD and SCM offer a faster, easier and reliable option for reconstruction in post-ablative head and neck surgery.

Abstract

ABSTRAK Latar belakang. HIV (Human Immunodeficiency Virus) merupakan retrovirus yang menginfeksi limfosit T CD4 yang dapat menyebabkan AIDS (Acquired Immune Deficiency Syndrome) yang merupakan sindrom akibat menurunnya kekebalan tubuh yang menyebabkan kolapsnya sistem imun oleh infeksi HIV. Prevalensi HIV/AIDS diketahui terus meningkat yang dikaitkan dengan padatnya jumlah penduduk, mobilitas masyarakat, kurangnya edukasi, dan budaya di lingkungan yang berisiko. Penelitian ini bertujuan untuk mengetahui gambaran tingkat pengetahuan mahasiswa Fakultas Kedokteran Universitas Sumatera Utara tentang infeksi HIV terhadap perilaku pencegahan HIV pranikah. Metode. Penelitian ini merupakan penelitian yang bersifat deskriptif dengan menggunakan pendekatan cross-sectional study. Sampel dipilih menggunakan teknik consecutive sampling, dengan besar sampel sebanyak 170 orang dengan menggunakan kuesioner. Hasil. Sebanyak 170 responden terkumpul pada penelitian ini yang terdiri atas 35.3% Perempuan dan 64.7% laki-laki. Seluruh responden berada pada kisaran usia 18-21 tahun. Dijumpai seluruh mahasiswa Fakultas Kedokteran Universitas Sumatera Utara memiliki tingkat pengetahuan tentang infeksi HIV dan melakukan perilaku pencegahan HIV Pranikah yang baik. Kesimpulan. Tingkat pengetahuan dan perilaku mahasiswa Fakultas Kedokteran Universitas Sumatera Utara terhadap infeksi HIV dikategorikan baik. Kata kunci: AIDS, HIV, Pencegahan, Perilaku, Tingkat pengetahuan.   ABSTRACT Background. HIV (Human Immunodeficiency Virus) is a retrovirus infecting CD4 T lymphocytes that may cause AIDS (Acquired Immune Deficiency Syndrome) which is a collapse of immune system of patients infected by HIV. Prevalence of HIV/AIDS is known to be increasing by time and is related to population growth, urban mobilization, lack of education, and high-risk sociocultural behaviors. This study aimed to describe level of knowledge and attitudes on HIV and pre-marriage prevention among medical student in Faculty of Medicine, Universitas Sumatera Utara. Method. This study is a descriptive research using a cross- sectional study approach. The sample was selected using the consecutive sampling technique with a sample size of 170 people using a questionnaire. Result. 170 questionnaires were gathered in this study. 35.3% among respondents were female and 64.7% were males, with all at the range of 18-21 years old. All medical students in Faculty of Medicine Universitas Sumatera Utara had a good knowledge regarding HIV infection and applied good behavior on pre-marriage prevention of HIV infections. Conclusion. The level of knowledge and behavior of students of the Faculty of Medicine, University of North Sumatra, regarding HIV infection is generally   categorized well. Abstract

Tenosynovial giant cell tumors are a group of benign soft tissue tumors of synovial origin with characteristic histologic features. In general, based on the growth pattern, it can be divided into local and diffuse types, and based on the location of the growth, it can be divided into intra-articular and extra-articular. Local type tumors generally develop slowly, while diffuse type tumors develop more aggressively and often affect the patient's quality of life. Recent developments show that tenosynovial giant cell tumor is a clonal neoplastic tumor driven by overexpression of Colony Stimifying Factor 1 (CSF1) in the synovial area, thus forming a mass. In this case report, we report a case of localized tenosynovial giant cell tumor and recent developments regarding its pathogenesis.

Abstract

An iliac aneurysm is a bulging and weakness in the wall of the iliac artery, a group of arteries located in the pelvis. The development of the disease is not progressive and the diagnosis is often accidental. In some cases, the diagnosis is made after complications occur, the most serious of which is aneurysm rupture. Intervention for a ruptured aneurysm can cause total circulatory collapse and has a high risk of perioperative morbidity and mortality ≤50%. The incidence of this case is around 0.4% of all abdominal aneurysms. This case is a challenge to handle because the anatomical location and surgical procedure are considered difficult. Open surgery and endovascular intervention are the definitive therapies for iliac artery aneurysms.

Abstract

Gastric perforation is the leakage of stomach acid into the abdominal cavity which will cause peritonitis and is an emergency. The risk of mortality due to the disease is 10 times greater than other acute abdominal diseases. One of the efforts to prevent gastric perforation is to know the risk factors that can increase the possibility of this disease. This study uses a retrospective descriptive method. Samples were taken using the total sampling technique. The data used are secondary data using medical record data from patients diagnosed with gastric perforation with an age range of 18-33 years in the 2020 period at the Sumbawa Regency Regional General Hospital. A total of 13 people suffered from gastric perforation at the age of 18-33 years with an average age of respondents of 23 years. All respondents were of different genders (100%). A total of 12 people (92.3%) had a smoking habit and 8 people (61.5%) consumed alcohol. There was 1 person (7.6%) with a history of Gastroesophageal Reflux Disease (GERD) and 5 people (38.4%) suffered from peptic ulcers disease. 15.4% of respondents had a history of consuming Non-Steroidal Anti-Inflammatory Drugs (NSAIDs) and 92.3% with a history of consuming traditional herbal medicine containing Curcumae rhizoma, Zingiberis rhizoma, Zingeberis aromaticae, and caffeine. In this study, all respondents were male and the average age was 23 years in the age range of 18-33 years. Risk factors found in respondents were smoking habits, alcohol consumption, history of Gastroesophageal Reflux Disease (GERD) and peptic ulcers disease, consumption of NSAIDs and traditional herbal medicine containing Curcumae rhizoma, Zingiberis rhizoma, Zingeberis aromaticae, and caffeine which have relaxing side effects and become addictive after consuming them.
