Abstract

Rekam medis elektronik (RME) merupakan terobosan teknologi informasi dan komunikasi yang mempermudah penyimpanan rekam medis dalam bentuk data elektronik, sehingga tenaga kesehatan dapat memperoleh informasi pasien dengan mudah, cepat, tanpa terbatas jarak dan waktu, disertai berbagai kemudahan dan manfaat lainnya. RME juga mempermudah pasien untuk mengakses data medisnya sendiri. Walau demikian, terobosan ini juga membuka kemungkinan dilema etik baru, antara lain konfidensialitas informasi, peretasan, potensi ancaman terhadap rapport dokter-pasien, dan gangguan sistem di tengah pelayanan. Suatu RME minimal memiliki dua bentuk pengaman yaitu otentikasi dan otorisasi, di samping enkripsi dan penggunaan penyimpanan cloud. Indonesia perlu membuat panduan keamanan dan standardisasi RME, baik pada fasilitas pelayanan tingkat pertama ataupun di rumah sakit, dan melakukan audit sistem keamanan RME secara rutin. Sebagai dokter dan tenaga kesehatan terdapat beberapa hal yang harus diperhatikan dalam penyelenggaraan RME sehingga manfaat yang diharapkan bagi peningkatan layanan terhadap pasien dapat terwujud.

Abstract

Proper death certification has broad medical and public health benefits. Therefore, death certificates should be completed accurately according to the medical certification of cause of death (MCCD) scheme. To improve the data quality of MCCD, Rumah Sakit Persahabatan (RSP) has implemented an electronic form (e-SMPK) since 2023. Although equipped with a user manual and coordination support system, incomplete and inaccurate e-SMPKs are still found. This study aims to identify interventions that can improve the quality (completeness and accuracy) of e-SMPK in RSP using one action research cycle consisting of 4 stages, i.e., diagnosing, planning, taking, and evaluating action. Qualitative data was collected through focused group discussions and field observations. Quantitative data was collected by comparing the completeness and accuracy of e-SMPKs before and after the intervention agreed upon with the participants. Triangulation of the data was carried out to maintain the credibility of the research. This study found that obstacles to e-SMPK consist of system and regulatory factors, skills, practical tools, and processes. Interventions are implemented by adjusting regulations, providing guidance and training, revising electronic forms, improving coordination, and facilitating consultation and verification to overcome obstacles. An increase in the completeness and accuracy of filling in the e-SMPK elements was obtained, most of which were statistically significant.

Abstract

Forensic and mortuary personnel face significant occupational hazards, yet comprehensive health risk assessments in these settings remain limited. This study assessed workplace risks at the Forensic Medicine and Mortuary Unit of Persahabatan General Hospital using the Health Risk Assessment (HRA) framework to recommend effective control measures. Seventeen personnel participated, with data collected through interviews, questionnaires, walk-through surveys, and internal records. Environmental measurements included noise levels (sound level meters), lighting (lux meters), and air quality parameters such as PM2.5, PM10, formaldehyde, and total volatile organic compounds (TVOCs). Ergonomic risks were assessed using REBA and ROSA, while psychosocial risks were analyzed using the SV-NBJSQ. The key risks identified were low back pain, nasopharyngeal carcinoma, leukemia, computer vision syndrome, and irritant contact dermatitis. Contributing factors included manual handling, awkward postures, insufficient lighting, and exposure to formalin. Proposed solutions encompassed introducing ergonomic tools, implementing educational programs, and revising operational guidelines. These interventions aim to reduce workplace hazards, enhance worker safety, and improve overall productivity. This study provides a structured approach for implementing workplace safety interventions, offering a model that can be applied in forensic and mortuary settings globally.

Abstract

Introduction: The lack of experts in forensic medicine will become a serious problem globally in the future. In Indonesia, the unique position of forensic medicine at the intersection between medicine and law itself means that the role of forensic doctors can still expand widely. The future development of forensic medicine in Indonesia is determined mainly by forensic doctors and prospective forensic doctors themselves. Therefore, this study aims to determine the motivation of doctors in choosing the specialty of forensic medicine and their views on work as forensic doctors. Methods: This is a cross-sectional descriptive observational research using mixed methods, with forensic doctors and residents as respondents. Result: The results show that 17.8% chose forensic medicine because of their interests and 11.1% because they considered forensic medicine to be a challenging specialty. The absence of scholarships, the lack of institutional and family support, the lack of number and variety of cases in various educational centers, and the limited supporting facilities and financing of forensic medical services are the main obstacles for forensic doctors. Conclusion: Regeneration is needed to meet the need of forensic doctors in various regions in Indonesia by spreading information about the importance of forensic medicine. Good coordination between forensic experts, the Indonesian Forensic Doctors Association (PDFI), other professional organizations, and relevant stakeholders is needed to overcome various obstacles and ensure the growth of forensic medicine in Indonesia.
