Abstract

A harbor is a place where ships stop. In the port there are many activities carried out, such as loading and unloading goods and passengers from ships. The utilization of Kambuno Port, located on Kambuno Island, Sinjai Regency, is no longer effective. The port is small and can no longer serve the number of ships and passengers that increase every day. The purpose of this research is to plan a deck on a piling type harbor located in Sinjai Regency. The analysis includes workload, design water level, and harbor dimensions. harbor dimensions. The data consists of primary and secondary data. After obtaining all the required data, it will be analyzed through several stages of ship design, presenting wind data, presenting topographic data, presenting bathymetry data, and presenting hydroboceanographic data (tides, wave currents, wave generating, design water level, and return period. water level, and return period. The workload consists of live load and death load. Based on the analysis, the total live load is 33.37tons and the total mortality load is 3,726.44 tons, where the water level is 1.07m. The dimensions of the harbor are length 68 m, and 8 m wide. Based on the analysis results, the dimensions of the slab are 68m x 8m x 0.30m, block 0.50mx 0.70m, pile cap is 1.00 x 1.00 x 1.00 m, and the pile is 0.4572 m in diameter and 0.012 m thick.

Abstract

Avascular Necrosis (ANV) is a pathological condition caused by impaired blood supply to the subchondral femur bone, causing necrosis and deformity of bone tissue. This condition causes severe pain, joint stiffness, and impaired physical mobility. Chronic avascular necrosis requires surgery to stabilize physical function. Total Hip Replacement surgery is one of the surgical options to relieve pain and restore joint function in patients with avascular necrosis. Nursing care for ANV patients is very important in managing pain, preventing complications, and promoting recovery. Through comprehensive nursing care, nurses have a significant impact on the physical and emotional health of patients, helping them live healthier and more functional lives. This article aims to implement nursing care for avascular necrosis patients undergoing surgery at Haji Adam Malik General Hospital, Medan. This study is a case report of a 63-year-old patient who had suffered from avascular necrosis for 10 years and underwent Total Hip Replacement surgery. Assessment data were collected through physical examination and supplemented with laboratory and radiology results. The postoperative diagnoses that are the priority in this patient are acute pain, impaired physical mobility, impaired skin integrity, and self-care deficit. Nurses focus on the criteria for treatment outcomes in reducing pain levels, increasing joint range of motion and physical mobility, improving skin integrity, and improving self-care. The intervention results are pain control that allows earlier mobilization and can be a reference for professional nurses in improving the quality of service, especially for avascular necrosis patients undergoing Total Hip Replacement (THR) surgery.

Abstract

Cancer is a health problem that is an important factor in global burden disease. Breast cancer is the most common cancer in women over the world and continues to increase. Quality of life for breast cancer patients becomes very important as the stage of cancer experienced by the patient increases. End stage breast cancer patients tend to experience readmission conditions after they return home from hospitalization. The aim of this study was to determine the quality of life in end stage breast cancer patients with the incidence of readmission. This study used a cross sectional method involving 110 stage 3 and 4 breast cancer patients who were undergoing treatment at the Adam Malik Hospital in Medan, who were sampled consecutively. The research was conducted from July to December 2023. The results of the chi-square analysis showed that the quality of life factors in end stage breast cancer patients that influenced the incidence of readmissions were physical problems with p<0.003 (CI 95%), social problems with p<0.007 (CI 95%), spiritual problems with p<0.030 (CI 95%) and medication problems with p<0.014 (CI 95%). Nursing interventions related to physical, social, spiritual conditions and treatment programs need to be carried out to improve the quality of life in end-stage breast cancer patients so that readmissions can be prevented or reduced

A total of 104 respondents with TB RO were being treated at the hospital. H. Adam Malik. Patient data came from the SITB of TB RO patients and was collected using TB MAS (TB-Medication Adherence Scale). The results of the study found that as many as 37 respondents (36.4%) were declared non-compliant and as many as 70 respondents (67.3%) were declared compliant. The purpose of this study is to see the relationship between adherence and BTA conversion in TB RO patients and to find out the factors that affect treatment adherence of TB RO patients. The results of this study found a relationship between patient compliance and BTA conversion with a p.value of 0.003. A bivariate analysis with chi-square was carried out and found that the factors that affect treatment compliance in TB RO patients are health service communication factors (0.000), personal traits (0.000), self-confidence (0.000), social support (0.000) and access to services (0.000).
