Abstract

Avascular Necrosis (ANV) is a pathological condition caused by impaired blood supply to the subchondral femur bone, causing necrosis and deformity of bone tissue. This condition causes severe pain, joint stiffness, and impaired physical mobility. Chronic avascular necrosis requires surgery to stabilize physical function. Total Hip Replacement surgery is one of the surgical options to relieve pain and restore joint function in patients with avascular necrosis. Nursing care for ANV patients is very important in managing pain, preventing complications, and promoting recovery. Through comprehensive nursing care, nurses have a significant impact on the physical and emotional health of patients, helping them live healthier and more functional lives. This article aims to implement nursing care for avascular necrosis patients undergoing surgery at Haji Adam Malik General Hospital, Medan. This study is a case report of a 63-year-old patient who had suffered from avascular necrosis for 10 years and underwent Total Hip Replacement surgery. Assessment data were collected through physical examination and supplemented with laboratory and radiology results. The postoperative diagnoses that are the priority in this patient are acute pain, impaired physical mobility, impaired skin integrity, and self-care deficit. Nurses focus on the criteria for treatment outcomes in reducing pain levels, increasing joint range of motion and physical mobility, improving skin integrity, and improving self-care. The intervention results are pain control that allows earlier mobilization and can be a reference for professional nurses in improving the quality of service, especially for avascular necrosis patients undergoing Total Hip Replacement (THR) surgery.

Abstract

Osteoarthritis (OA) of the knee is one of the most common musculoskeletal disorders and a leading cause of disability in the elderly. The severity of osteoarthritis can be assessed using the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), which is a widely used tool to measure pain, stiffness, and physical dysfunction in patients with OA. This study aimed to analyze the relationship between the demographic and clinical characteristics of patients with knee osteoarthritis and their WOMAC scores. The study used a quantitative, cross-sectional observational design. The relationship between functional disability (measured by the WOMAC score) and various factors was analyzed. The independent variables included age, sex, occupation, body mass index (BMI), OA grade, and duration of OA to the WOMAC score. Significant correlations were found between the WOMAC score and both OA grade (p=0.049) and OA duration (p=0.030). Furthermore, the multiple linear regression analysis revealed that OA duration (p=0.038) and OA grade (p=0.036) were significant predictors of the WOMAC score, collectively explaining 13.0% of its variance (R² value = 0.130).  The findings of this study indicate that OA grade and duration of illness are significant predictors of the level of disability as measured by the WOMAC scores. OA levels and disease duration were the primary predictors of functional disability, explaining 13.0% of the WOMAC score variance. Nursing interventions should focus on risk stratification based on these clinical markers, prioritizing early intervention for newly diagnosed patients, irrespective of their age or BMI.

Abstract

Nausea and vomiting is a complaint that often occurs in cancer patients undergoing chemotherapy. Even though he had received antiemetic therapy before giving chemotherapy, this complaint still appeared and even dominated the other complaints. So complementary therapies are needed to accompany the medical therapy provided to achieve holistic care in patient care. One complementary therapy that is very effective in reducing nausea and vomiting is acupressure. This study aims to conclude the importance of acupressure intervention in reducing complaints of nausea and vomiting in breast cancer patients. Searching for articles in this systematic literature review used the PRISMA method, which focuses on acupressure interventions in reducing nausea and vomiting. The search strategy used Boolean Search with keywords acupressure AND breast cancer OR breast cancer AND nausea and vomiting. The author selected the articles collected according to the determination of keywords in the inclusion and exclusion criteria. Six databases were searched from Proquest, ScienceDirect, CINAHL, Pubmed, Google Scholar, and SpringerLink sites from 2019-2023, and all articles were evaluated according to relevance and research quality. 15 articles have been analyzed according to characteristics, interventions, nausea and vomiting instruments used, and research objectives from 7 articles of international and 8 national reputations. This systematic literature review study significantly resulted in the conclusion that complementary acupressure therapy can reduce complaints of nausea and vomiting in breast cancer patients. Acupressure intervention can be used as evidence-based practice in improving holistic patient care
