Abstract

with sedation and mainly used in elderly patients or those with medical problems, including those who had ovarian mass. Epidural administration of amide local anesthetics in combination with opioids is widely used for pain relief because of the dose minimizing and side effects reducing benefits. Case: Patient was diagnosed with ovarian mass. The patient also had pleural effusion, ascites, and hypoalbuminemia. Epidural anesthesia used in this patient as a resource for perioperative and postoperative pain management. Discussion: Patient underwent unilateral salpingo-oophorectomy for her ovarian mass. Anesthetic doses were given carefully due to patient conditions with pleural effusion and ascites. Opioid and Neuromuscular Blocking Agents (NMBAs) effects were absolutely decreased due to hypoalbuminemia, but were overcome with Naloxone and Sugammadex. Epidural anesthesia used to stabilize the drugs and pain management for this patient. Conclusion: Epidural anesthesia is an excellent choice for unilateral salpingo-oophorectomy in patients with ovarian mass. It has been proven to provide excellent pain control after major surgeries and may be associated with a lower incidence of postoperative complications.

Abstract

Background: Primary tumor are rarely found in pregnancy. Treatment for intracranial tumors in pregnant woman is a challenge for anesthesiologists, obstetricians and neurosurgeons. A large size cerebellopontine angle (CPA) tumor can suppress the brain stem structure and cause death if not treated immediately. This makes the way we treat patients, when the delivery time and the surgical intervention we choose to be quite difficult.Case: A 23 years-old female patient using general anesthesia (GA) to accommodate profunda trans-peritoneal cesarean section (SCTP) in a 36 weeks pregnancy age, G1P0A0, ASA IIIE with CPA tumor and oligohydramnions.Disscussion: Brain tumor usually grow rapidly during pregnancy due to fluid retention, increased blood volume, and hormonal changes. There is no specific guidelines that explain the treatment of intracranial tumor in pregnancy. This patient suffers from CPA tumor and oligohydramnions so SCTP is required. We use GA for SCTP because it is safe for patients with intracranial tumor.Conclusion: Female G1P0A0, 23 years old, 36 weeks pregnancy age with CPA tumor was consulted by Obstetricians to Anesthesiologists to do SCTP with GA.

Abstract

Desa Pondokrejo memiliki aset pertanian dan peternakan skala kecil oleh warganya. Disamping produk utama berupa hasil pertanian dan peternakan, sektor ini juga menghasilkan limbah. Limbah yang tidak ditangani dengan tepat dapat menimbulkan masalah. Oleh karena itu, dilakukan Pengabdian kepada Masyarakat untuk menangani masalah limbah dengan pendekatan Asset Based Community Development (ABCD). Langkah pertama yaitu mengidentifikasi aset desa. Dari kegiatan ini diketahui bahwa limbah dapat dipandang sebagai aset (discovery). Selanjutnya dilakukan perumusan cita-cita bersama warga desa yaitu pemanfaatan limbah untuk mendukung hasil pertanian (dream). Pada tahap berikutnya dirancang kegiatan pelatihan pembuatan pupuk kompos dari limbah (design). Realisasi rancangan kegiatan pada Tanggal 18 Agustus 2024 di Dusun Pondokmiri, Desa Pondokrejo (define). Setelah kegiatan, diketahui bahwa warga merasa senang dan terbantu atas kegiatan ini. Mereka juga berencana untuk mengelola limbah pertanian dan peternakan untuk dijadikan pupuk kompos (destiny).
