Abstract

Pada bulan Desember 2019, muncul kasus pneumonia di Wuhan, Cina yang disebabkan oleh virus SARS-CoV-2. Kasus ini menjadi pandemik multipel dikarenakan tingginya angka kematian. Oleh sebab itu, dibutuhkan parameter yang dapat memprediksi prognosis pasien COVID-19. Profil hematologi 24 jam pertama bisa digunakan sebagai prediktor untuk melihat tingkat mortalitas pasien. Tujuan penelitian ini adalah untuk mengetahui korelasi profil hematologi 24 jam pertama terhadap mortalitas pada pasien COVID-19 di RS Murni Teguh Memorial Medan pada tahun 2020. Penelitian merupakan penelitian analitik dengan desain cross sectional. Sampel penelitian 62 data rekam medik pasien COVID-19 di RS Murni Teguh Memorial Medan tahun 2020 dengan metode consecutive sampling. Analisa data dengan metode univariat untuk mendeskripsikan karakteristik sampel, serta analisa bivariat untuk menganalisis hubungan antara variable hemoglobin, trombosit, neutrofil, limfosit, dan NLR terhadap variabel mortalitas dengan metode spearman. Dari hasil penelitian terdapat korelasi dari profil hematologi berupa hemoglobin (p = 0,001), Trombosit (p = 0,001), Neutrofil (p= 0,002), Limfosit ( p = 0,002) dan NLR ( p = 0,001) terhadap mortalitas pada pasien COVID-19. Kesimpulan penelitian ini adalah terdapat korelasi yang signifikan antara profil hematologi 24 jam pertama terhadap mortalitas pada pasien COVID-19 di RS Murni Teguh Medan Tahun 2020.

Abstract

Human Immunodeficiency Virus (HIV) is a viral infectious disease that attacks the immune system or white blood cells (lymphocytes). HIV, which belongs to the Retroviridae family, is lymphatic, which can damage the specific immune system, namely T-helper lymphocytes or Cluster of Differentiation 4 (CD4). The CD4 count is normal when it is 500–1400 cells/mm3, which is usually the determinant of whether a patient is infected with HIV or not. This study aims to assess the comparison of CD4 counts in HIV-TB patients before and after ARV and OAT treatment at Dr. Pirngadi Medan City. Observational analysis research with a cross-sectional design was conducted at RSUD Dr. Pirngadi Medan City in September 2022. The sample for this study was 20 patient records from the medical records of RSUD Dr. Pirngadi, Medan City. The data were analyzed using an unpaired t-test. The results of the study showed that the most HIV-TB pulmonary  patients at RSUD Dr. Pirngadi Medan City were aged 36-45 years (50.0%). The majority of male patients (90.0%) and the domicile of most patients in Medan City are in the Medan Tembung sub-district (15.0%). The results of the unpaired t-test obtained a p-value of 0.022, meaning that there was a significant difference between the CD4 count before and after ARV and OAT treatment in HIV-TB patients at RSUD Dr. Pirngadi Medan City. There were significant differences in CD4 counts before and after ARV and OAT treatment in HIV patients with pulmonary TB co-infection at Dr. Pirngadi, Medan City.

Abstract

Dengue is a disease commonly found in tropical and subtropical regions, with its main vector of transmission being the Aedes aegypti mosquito. Common symptoms experienced by dengue patients include fever followed by severe headache, retro-orbital pain, nausea, vomiting, fatigue, and typically a rash due to minor bleeding. In acute infections, CRP is often used to assess disease activity causing inflammation and can be one of the inflammatory biomarkers in dengue patients. To examine the correlation between CRP levels, hemoconcentration, and platelet count in children diagnosed with Dengue. This study is an observational analytical study using a cross-sectional approach and data collection through consecutive sampling with a total sample size of 60 individuals. In this study, there were 60 pediatric patients diagnosed with Dengue Hemorrhagic Fever (DHF) and Dengue Fever. Most of 34 patients (56.7%) were male, while 26 patients (43.3%) were female. The majority of DHF and Dengue Fever patients were aged between 5 and 11 years (63.3%). The Chi-Square test analysis between CRP and hematocrit showed p=0.486. The Chi-Square test analysis between CRP and platelet count showed p=0.042 (p<0.005). CRP has no correlation with hemoconcentration, but correlate with trombositopenia level in pediatric patients with Dengue Hemorrhagic Fever and Dengue Fever.

Abstract

Background: Dengue virus infection is a global health problem. Dengue Hemorrhagic Fever (DHF) has become a global attention because of children death due to plasma leakage. Plasma leakage is a symptom of critical phase in children that occurs on the 3rd to 5th day. Neutrophil lymphocyte (NL) ratio and platelet count change can be seen in critical phase of plasma leakage which can help process of patient’s therapy. Objective: This study aimed to compare the NL ratio and platelets count on plasma leakage in children with DHF. Methods: This study was an analytical study with cross-sectional design. The sample was children with DHF at RSUD Dr.Pirngadi Medan. The data of this study were analyzed by computer software, used an unpaired t test with p <0.05. Results: This study found that the most age of DHF was 6-10 years old, most gender are male and DHF grade 1. Unpaired t test found the average difference between NL ratio and platelet with value of p = 0.043 (p<0.05). Conclusion: Plasma leakage average on NL ratio is higher than plasma leakage average on platelets that occurs in children with DHF at RSUD Dr.Pirngadi Medan.
