Abstract

Role of Binahong (Anredera cordifolia (Ten.) Steenis) leaves as hepatoprotector and antioxidant was identified on CCl4 - induced white rats. Unrandomized control trial method was used in this study. Ten male Wistar white rats with 200 - 250 gr of body weight were divided into two groups. Group I as a control group was treated with standard food and 1 ml aquadest orally for 12 days. Group II as a treatment group was given standar food and daily single dose pre treatment of Binahong (Anredera cordifolia (Ten.) Steenis) leaves juice 289,8 mg/200 gr of body weight for 12 days. Carbon tetrachloride (1 ml/kg body weight, i.p.) injected into rats in the day 13th to induce liver cells damage. ALP test used ALP KIT reagent had been done twice, 1st day and 14th day. The result shows mean of ALP level before and after treatment in control group were 95,94+1,52 and 110,15+1,07; in treatment group were 92,65+3,05 and 92,91+0,68. Paired T-test shown significant difference before and after treatment (p 0,05). Independent T test shown a significant difference ALP increasing between two group, while ALP serum level in treatment group lower than control group. This study proved that Binahong (Anredera cordifolia (Ten.) Steenis) leafs juice 289,8 ml /200gr body weight each day could decrease hepatotoxicity effect of carbon tetrachloride by maintain normal level of ALP in white rats Wistar strain.Manfaat daun Anredera cordifolia (Ten.) Stenis sebagai hepatoprotektor dan antioksidan diteliti pada tikus putih induksi CCl4. Unrandomized control trial digunakan pada penelitian ini. Sepuluh tikus putih galur wistar (Rattus norvegicus) jantan dengan berat badan 200-250 g dibagi menjadi dua kelompok. Kelompok kontrol diberi perlakuan dengan diberi pakan standar dan 1 ml aquadest setiap hari selama 12 hari. Kelompok perlakuan diberi pakan standar ditambah jus daun binahong dosis tunggal sebesar 329,21 mg diberikan lewat oral setiap hari selama 12 hari. Karbon tetraklorida (1 ml/kg berat badan, i.p.) disuntikkan ke tikus pada hari ke 13 untuk menginduksi kerusakan sel hepar. ALP test dilakukan dua kali, pada hari pertama dan hari ke-14. Hasil penelitian menunjukkan rata-rata kadar ALP serum pada kelompok kontrol sebelum dan sesudah perlakuan adalah 95,94+1,52 IU/l dan 110,15+1,07 IU/l; pada kelompok perlakuan adalah 92,65+3,05 IU/l dan 92,91+0,68 IU/l. Paired T-test menunjukkan perbedaan signifikan sebelum dan sesudah perlakuan (p 0,05). Independent T-test menunjukkan perbedaan signifikan (p 0,05) peningkatan ALP sebelum dan sesudah perlakuan pada kedua kelompok dimana kadar ALP pada kelompok perlakuan lebih rendah daripada kelompok kontrol. Disimpulkan bahwa pemberian jus daun Binahong (Anredera cordifolia (Ten.) Steenis) 329,21 mg setiap hari dapat meningkatkan efek hepatoprotektif dengan menjaga kadar normal ALP pada tikus putih galur Wistar

Abstract

Background. Systemic Lupus Erythematosus (SLE) requires comprehensive and integrated treatment because it can manifest in various organ systems, both non-major and major organs. Pulse doses Methylprednisolone used as induction therapy, it provides dramatic improvement in prognosis in severe lupus through its nongenomic effects. Not all SLE patients who received pulse dose methylprednisolone therapy gives complete response, several factors may influence the therapeutic response, one of which is thought to influence the difference in therapeutic response is the number of major organ involvement.Objectives. To determine the relationship between the number of major organ involvement and therapeutic response of pulse dose methylprednisolone therapy in SLE patients at Dr. Sardjito Hospital Yogyakarta.Methods. This study used a retrospective cross-sectional study. The subjects of this study were adult patients with SLE treated in the internal medicine ward at Dr. Sardjito Hospital Yogyakarta and met the inclusion and exclusion criteria from January 1, 2016 to December 31, 2019. The data on the characteristics of the research subjects were taken from the patient's medical records and laboratory data before giving pulse methylprednisolone.Results. A total of 88 research subjects were taken from medical records. Patients with major organ involvement 1 experienced more complete response (53.8%), major organ involvement 2 mostly partial response (66.7%), major organ involvement 3 (48.6%) and major organ involvement 4 (53.8%) no response, and major organ involvement 5 (75%) partial response. There is a significant relationship with the number of major organ involvement with therapeutic response p<0.001. The correlation coefficient r=0.382 means that the more major organ involvement the less therapeutic response. The results of multivariate analysis showed that only the number of major organ involvement had a dominant effect on the therapeutic response, p=0.001. Regression coefficient 0.797.Conclusion. The number of major organ involvement significantly affects the success of pulse dose methylprednisolone therapy in SLE patients.
