Abstract

Abstract. Total Knee Arthroplasty (TKA) is an effective surgical treatment in patients with moderate to severe knee arthritis. This surgical intervention has the potential to cause blood loss of up to 1000cc, which can lead to a decrease in haemoglobin levels and postoperative functional abilities. There are many methods to reduce the amount of blood loss after TKA surgery, one of the most effective and efficient methods is to position the knee flexion 45° for 24 hours post-operative TKA. This study was the application of Evidence Based Nursing (EBN) with a sample of 10 that meet theInclusion criteria were patients with knee osteoarthritis stage III and IV and underwent TKA Total Knee Arthroplasty (TKA) is an effective surgical treatment in patients with moderate to severe knee arthritis. This case series aims to present the result of applying evidence-based leg positioning after TKA to reduce blood loss with a sample of 10 that meet the Inclusion criteria were patients with knee osteoarthritis stage III and IV and underwent TKA (Total Knee Arthroplasty) surgical procedures. The results show that all respondents are female, most of them are <60 years old (60%), and BMI in the overweight category is 70%, the amount of Hidden Blood Loss is in the range of 420cc-700cc, 30% of respondents have post-op Hb levels <10gr/dl, and 60% of respondents have a ROM value of 90° on day 5 Post Operation. The Implementation of EBN Knee flexion position for 24 hours post TKA effective to reduce the amount of blood loss and improve the patient's functional ability by facilitating the patient to start rehabilitation exercises early

Abstract

Diabetic foot ulcers are one of the chronic complications of diabetes, leading to disease complexity. As a result, DFU may provoke problems in fulfilling spiritual needs that affect the healing process. This study aimed to analyze the factors related to the spiritual needs of diabetic ulcer patients in Indonesia. This cross-sectional study was conducted on 104 diabetic foot ulcers patient. The patients were recruited from 5 cities in Indonesia using a purposive sampling method from June until August 2022. The instruments consisted of a sociodemographic characteristics form and the Spiritual Needs Questionnaire (SpNQ) to explore patients' Spiritual needs. Then, bivariate and multivariate binary logistics were performed. There is a significant relationship between spiritual needs and age, marital status, length of the wound, and degree of diabetic ulcer (p < 0.05). It is recommended that healthcare workers need to pay attention to the factors that influence the spiritual needs of diabetic foot ulcer patients. Abstract

Ulkus kaki diabetik merupakan salah satu komplikasi tersering pada pasien diabetes. Perilaku pencarian layanan kesehatan sangat penting untuk merencanakan perawatan dan manajemen diabetes setelah ditegakkan diagnosis sehingga komplikasi dapat diminimalkan. Tujuan dari penelitian ini adalah untuk melihat hubungan antara demografi dengan perilaku mencari penyembuhan ke fasilitas pelayanan kesehatan professional. Penelitian kuantitatif dengan desain analisis korelatif menggunakan rancangan penelitian cross-sectional. Total sampel sebanyak 112 subjek, pengumpulan data dilakukan dengan menggunakan instrumen Health Seeking Behaviour. Pasien ulkus diabetikum berjenis kelamin perempuan lebih banyak (60,7%), usia subjek terbanyak berada dalam rentang 35 sampai 60 tahun (75%), Status subjek yang sudah menikah sebanyak 78,5%. Terdapat hubungan yang signifikan antara Health Seeking Behaviour dan Persepsi manfaat, Persepsi hambatan, Persepsi keparahan, serta Persepsi kerentanan dengan p-value<0,05. Tenaga Kesehatan di klinik/rumah sakit agar dapat memberikan pendidikan kesehatan terkait penyakit ulkus diabetikum serta meningkatkan kesadaran mereka terhadap pentingnya mencari pertolongan penyembuhan ke fasilitas pelayanan kesehatan profesional.
