Abstract

Background: patient-oriented health services are the primary strategy for health service organiza­tions. A good service is determined by the fact that the services provided can meet the patient's needs. Patient satisfac­tion is an integral part of the quality assurance of health service. This study aims to analyze the quality of tangible health ser­vices with patient satisfaction in receiving health services.Subjects and Method: Meta-analysis was performed by searching journals from direct databases such as Google Scholar, Science Direct, and PubMed using the key­words service quality, patient satisfaction, and heat service. The inclusion criteria in this study were full text from 2010-2021, with a cross-sectional study. The data analysis was carried out by RevMan 5.3.Results: Nine articles have been analyzed, showing that the quality of service had direct evidence of an effect on patient satis­faction in accessing health services (aOR= 4.17; 95% CI= 1.78 to 9.78; p <0.001).Conclusion: The quality of direct evidence (tangible) affects patient satisfaction in accessing health services.Keywords: tangible, service, patient satisfaction.Correspondence: Siti Mar'atul Mu­na­waroh. Abstract

Analgesia perioperatif untuk operasi caesar bertujuan untuk memastikan kenyamanan ibu, memfasilitasi pengalaman pembedahan yang lancar, dan mendorong pemulihan yang sukses. Enam puluh pasien digunakan dalam studi komparatif yang dirancang untuk menilai kualitas analgesia perioperative pada operasi seksio sesaria elektif dengan anestesi spinal ketika morfin ditambahkan pada penggunaan kombinasi fentanyl dan lidodex. Dua kelompok responden terdiri masing-masing 30 subjek dibandingkan dalam analisis prosedur: A (fentanyl dosis standar 25 μg) dan B (morfin dosis standar 100 μg dan fentanyl dosis standar 25 μg). Skor nyeri numerik dicatat pada awal dilakukan sayatan, pernyataan responden selama intra operasi dan pernyataan responden saat akan dipindahkan ke ruang rawat lanjutan. Kedua kelompok responden memiliki manajemen yang sama pada awal sayatan pembedahan. Pada intra operasi kelompok morfin memiliki manajemen nyeri yang jauh lebih baik (uji Mann-Whitney U, p < 0,035) begitu pula pada evaluasi menjelang responden dipindahkan ke ruang rawat lanjutan (uji Mann-Whitney U, p < 0,000). Berdasarkan hasil dari analisis dan pembahasan dapat ditarik kesimpulan bahwa penambahan ajufan morfin dalam obat yang disuntikkan secara intratekaluntuk pembiusan regional anestesi Sub Arachnoid Block (SAB) pada pasien dengan pembedahan seksio sesaria sangat efektif menekan timbulnya nyeri selama dan setelah tindakan operatif dibuktikan dengan nilai p value kurang dari 0.050

Abstract

Spinal anestesi adalah prosedur pemberian obat anestesi untuk menghilangkan rasa sakit pada pasien yang akan menjalani pembedahan dengan menginjeksikan obat anestesi, Coloading ternyata lebih efektif dalam insiden mempertahankan tekanan darah tetap normal dengan pemberian cairan koloid, Tujuan : untuk mengetahui Perbedaan Coloading Cairan Koloid (terastarch) dengan Kristaloid (ringer laktat) terhadap Tekanan Darah Post Spinal Anestesi pada pasien Bedah Ortopedi di Rumah Sakit Umum Islam Kustati Surakarta. Metode Penelitian : menggunakan metode kuantitatif dengan desain cross sectional, teknik observasional analitik data dianalisis dengan menggunakan uji Mann Whitney U Test. Hasil : penelitian ini dilakukan dari Februari s/d Maret 2024, berdasarkan 300 pasien dengan jumlah sampel 20 responden terdapat 10 responden dengan cairan koloid dan 10 responden dengan cairan kristaloid, Mann Whitney U Test didapatkan Sig.(2-tailed) 0,002 (< 0,05). Simpulan : H0 ditolak dan Ha diterima, yang berarti ada perbedaan Coloading Cairan Koloid (terastarch) dengan Kristaloid (ringer laktat) Terhadap Tekanan Darah Post Spinal Anestesi

Abstract

Background: The COVID-19 pandemic has spread around the world and is impacting the mental health of everyone, including healthcare workers. Health workers are at the forefront of treating patients infected with COVID-19. are at high risk of experiencing mental health problems during the COVID-19 pandemic. The purpose of this study was to estimate the magnitude of the effect of COVID-19 patient care on the level of anxiety and depression in nurses in hospitals, with a meta-analysis of primary studies conducted by previous authors.Subjects and Method: This study is a systematic review and meta-analysis with the following PICO, population: nurses. Intervention: serving COVID-19 patients. Comparison: does not serve COVID-19 patients. Outcome: anxiety and depression. The articles used in this study were obtained from three databases, namely Google Scholar, Pubmed, and Science Direct. Keywords to search for articles “Anxiety” AND “Depression” AND “Nurses” OR “Healthcare worker” AND “COVID-19”. The included article is a full-text English language with a cross-sectional study design from 2020 to 2021 and reports the adjusted Odds Ratio (aOR) in a multivariate analysis. The selection of articles is done by using PRISMA flow diagram. Articles were analyzed using the Review Manager 5.3 application.Results:  A total of 12 cross-sectional studies from America, Asia, and Europe were selected for systematic review and meta-analysis. The data collected showed nurses who served COVID-19 patients had a risk of experiencing anxiety as much as 1.75 times compared to nurses who did not serve COVID-19 patients and this result was statistically significant (aOR= 1.75; 95% CI= 1.29 to 2.37; p<0.001 ). A meta-analysis of 12 studies from America, Asia and Europe showed that nurses caring for COVID-19 patients had a 1.1 times risk of developing depression compared to nurses who did not serve COVID-19 patients, but this result was not statistically significant (aOR= 1.10; 95% CI= 0.53 to 2.30; p= 0.80).Conclusion: The effect of caring for COVID-19 patients increases the risk of anxiety and depression in nurses in hospitals.Keywords: Anxiety, Depression, Nurses, COVID-19Correspondence: Andang Sudarmono. Abstract

Shivering, or Post-Anesthesia Shivering (PAS), is one of the most common side effects of spinal anesthesia. Preventing shivering is crucial to avoiding complications such as patient discomfort, increased wound pain, delayed wound healing, elevated oxygen consumption, increased carbon dioxide production, and a higher metabolic rate. This study aims to identify both pharmacological and non-pharmacological interventions for shivering prevention. A mixed-method study with a sequential explanatory model was conducted in the Operating Theatre of a Type B hospital in Central Java over one month. The study involved 30 patients as respondents for the observational study and 9 anesthesia practitioners for the qualitative study. Data collection was carried out through patient observations and in-depth interviews with anesthesia practitioners. Shivering prevention interventions included pharmacological methods, with Pethidine (76.7%) as the most commonly used drug, followed by Tramadol, Dexamethasone, Ketamine, and Fentanyl. Additionally, non-pharmacological methods were implemented, including room temperature regulation (93.3%) and warmer blanket administration (93.3%). The qualitative study revealed that Pethidine was the preferred pharmacological choice for shivering prevention. Meanwhile, non-pharmacological strategies, such as warm infusions, oxygen administration, additional layers on the operating table, head caps, covering non-operated body parts, radiation, relaxation techniques, and deep breathing exercises, also played a significant role in reducing the incidence of shivering in spinal anesthesia patients. The findings of this study can serve as a guideline for shivering prevention management in IBS. Further research with a larger sample size, stricter control of shivering-related factors, and comparative studies on the effectiveness of various pharmacological and non-pharmacological interventions is recommended for a more comprehensive understanding.

Abstract

Tujuan penelitian ini adalah untuk mengkaji berbagai teknik anestesi yang dapat digunakan dalam pembedahan pinggul. Fokus penelitian ini adalah berbagai faktor yang menjadi pertimbangan dalam menentukan teknik anestesi pada pembedahan pinggul. Data ilmiah yang relevan dengan tujuan penelitian dikumpulkan, dievaluasi, dan disintesis melalui metode Sistematic Literature Review (SLR). Penelitian ini juga memaparkan hasil temuan peneliti di Rumah Sakit yang disesuaikan dengan topik pembahasan. Kesimpulan yang didapatkan adalah neuraksial anestesi menjadi teknik yang banyak digunakan dalam pembedahan pinggul karena memberikan kontrol nyeri yang efektif dan menurunkan risiko komplikasi respiratori namun perlu diperhatikan bahwa teknik anestesi harus disesuaikan dengan kondisi klinis pasien, durasi operasi, dan risiko komplikasi yang mungkin terjadi.
