Abstract

Money Laundering Crime as a further criminal act has placed and involved two main perpetrators, namely active actors as parties trying to hide or disguise the origin of assets that are the result of criminal acts in various ways, and passive actors as parties who receive Money Laundering Crime assets. The return of Money Laundering Crime assets is not only through coercive measures, but also requires a voluntary return by the receiving party. In practice, voluntary returns create uncertainty when the receiving party (passive actor) from the beginning does not know the origin of the assets received. Through various money laundering cases in Indonesia in June-July 2022, the Authors are very interested in analyzing the extent of obligations related to the return of Money Laundering Crime assets. This research uses normative juridical methodology with Law approach and concept approach. The purpose of this study is to determine the obligations that must be carried out by passive trafficking actors based on Law No. 8 of 2010.

Abstract

Pedagang konvensional adalah individu atau kelompok yang sedang berada di suatu lokasi terbuka terjadi kegiatan transaksi jual beli dengan memungkinkan proses tawar-menawar. Dengan zaman yang semakin modern keberlanjutan pedagang konvensional dapat dipengaruhi dengan adanya penjualan online. Oleh karena itu penulis tertarik untuk melakukan penelitian untuk mengetahui seberapa besar pengaruh adanya penjualan online terhadap keberlanjutan pedagang konvensional di kecamatan Sekupang kota Batam. Metode penelitian yang digunakan adalah metode kuantitatif dengan teknik pengambilan data yaitu data primer melalui kuisioner. Metode analisa data dilakukan dengan menggunakan uji statistik regresi linear sederhana. Berdasarkan analisis data yang telah dilakukan diketahui adanya penjualan online mempengaruhi keberlanjutan pedagang konvensional di kecamatan Sekupang kota Batam.

Abstract

Atrial septal defects (ASDs) constitute a common form of congenital heart defect (CHD) and can lead to significant morbidity and mortality in adulthood if left untreated. While advancements in prenatal screening and fetal echocardiography have facilitated early interventions, some cases may go undetected until adulthood, manifesting as symptoms like progressive shortness of breath. A recent case involved a 22-year-old woman with a history of ASD since childhood, presenting with cyanosis, digital clubbing, and peripheral edema. Examination revealed cardiomegaly, right atrial enlargement, and right ventricular hypertrophy. Diagnostic tests, including chest X-ray, electrocardiogram, and echocardiography, confirmed features consistent with Eisenmenger's syndrome, characterized by severe pulmonary hypertension and a large secundum ASD. Effective management requires a multidisciplinary approach involving cardiologists, pulmonologists, cardiac surgeons, and internists. It is crucial to address the underlying causes, as medications targeting pulmonary vessels may not consistently provide reliable results. Timely intervention and collaboration among specialists are paramount in improving outcomes for individuals with ASDs and preventing complications associated with Eisenmenger's syndrome

Abstract

Penyebab umum dari efusi pleura eksudatif adalah tuberkulosis (TB). Sebagian besar kasus efusi pleura yang disebabkan oleh TB akan sembuh secara spontan; Meski demikian, jika pasien tidak diobati dengan terapi anti-tuberkulosis, terdapat kemungkinan sekitar 50% untuk menjadi TB aktif dalam 5 tahun ke depan. Tergantung dari aktivitas reaktivasi infeksi, efusi pleura yang timbul mungkin kecil, dan dapat sembuh secara spontan atau cukup besar untuk menyebabkan gejala seperti demam, nyeri dada pleuritik, dan sesak napas. TB pleuritis dapat timbul dengan infeksi primer atau akibat reaktivasi infeksi. Cairan pleura disebabkan oleh respons hipersensitivitas intrapleural terhadap antigen mikobakteri yang dilepaskan dari fokus yang telah terbentuk melalui ruptur ke dalam ruang pleura. Pengobatan TB pleura sama dengan pengobatan TB paru dengan paduan 2RHZE/4RH. Cairan dievakuasi dengan seoptimal mungkin, sesuai keadaan pasien dan bila diperlukan evakuasi cairan dapat diulang. Laporan kasus ini melaporkan seorang pasien laki-laki berusia 25 tahun datang ke RS Sumber Waras dengan keluhan utama batuk sejak 1 bulan dan demam hilang timbul sejak 2 minggu sebelum masuk RS. Batuk dirasakan terus menerus sepanjang hari dan saat tidur. Pasien juga mengeluhkan keluhan lain seperti penurunan berat badan dan juga keringat malam hari. Pada pemeriksaan fisis paru didapatkan vokal fremitus kiri melemah, redup pada basal paru kiri, dan juga terdengar ronki pada paru kanan. Diagnosis ditegakkan berdasarkan anamnesis, pemeriksaan fisis, dan pemeriksaan penunjang.

Abstract

Dysphagia is a difficulty in swallowing either solid food or liquid. Esophageal stricture being one of the most common causes of dysphagia can be caused by inflammation, fibrosis, or neoplasia damaging the esophageal lumen. If left untreated, dysphagia can lead to malnutrition, aspiration, and decreased quality of life. Another common cause of dysphagia is esophageal motility disorder, an impairment in peristalsis of the esophageal. Calcium channel blockers such as nifedipine have been reported to improve dysphagia and may serve as alternative therapy while waiting for endoscopic intervention. A 27-year-old man presented with a two-month history of progressive dysphagia to solids, semisolids, and liquids, after an episode of hematemesis. Esophagogram showed esophageal stricture with impaired peristaltic. Contrast-enhanced CT result was suggestive of an intraluminal esophageal stricture. The patient was given sublingual nifedipine before meals as an alternative therapy while waiting for endoscopic intervention, and showed gradual sympomatic improvement. Dysphagia caused by long-segment esophageal stricture can be exacerbated by secondary motility disorder. While definitive treatment requires endoscopic intervention, sublingual nifedipine offered effective short-term symptomatic relief by reducing esophagogastric junction pressure and improving bolus transit through the narrowed lumen.

Abstract

Diffuse Large B-Cell Lymphoma (DLBCL) is the most common type of non-Hodgkin’s lymphoma (NHL) in the elderly population. DLBCL is potentially curable with immunochemotherapy but becomes more difficult to cure as patients get older leading to poor prognosis. Palliative care is essential for patients with this condition to relieve their suffering and improve their quality of life. Treatment strategies of DLBCL in elderly patients remain a challenge for physicians and the patients. An 87-year-old male presented with difficulty swallowing for the past four months, worsened since last week. Throat pain, obstruction sensation, and confined to bed due to weakness. The patient has been diagnosed with DLBCL and refused chemotherapy. Dark-colored residue was observed in the nasogastric tube. There is an enlargement of right cervical lymph-node. His laboratory workup shows Hb 8,9mg/dL, histopathological of the oropharyngeal tissue showed Diffuse Large B-Cell Lymphoma, activated B-Cell subtype. Patient was treated through multidisciplinary approach to improve overall condition and his quality of life. The management of elderly patients with DLBCL requires multidisciplinary approach that considers patient preferences and quality of life. Elderly Prognostic Index are needed to evaluate patient condition, prognosis, and guide therapeutic decisions. Palliative therapy is an approach aimed at improving the quality of life of patients facing life-threatening illness through early identification and comprehensive management of pain, physical, psychosocial, and spiritual problems. Management of DLBCL in elderly patients requires balancing risks and benefits considering frailty, comorbidities, and overall functional status. Regardless of therapeutic choice, palliative and supportive care should be provided through a multidisciplinary approach to relieve suffering and preserve quality of life. Treatment decisions should be made involving the patient and their family, focusing on the ultimate goals of care in accordance to their wishes.
