Abstract

Basalioma merupakan kanker kulit non-melanoma yang paling sering ditemukan. Basalioma umumnya tumbuh lambat, menyebabkan destruksi lokal, jarang bermetastasis. Penyakit ini berkembang pada area terpapar sinar UV, umumnya di hidung. Tulisan ini melaporkan kasus seorang pria 48 tahun dengan basalioma di daerah nasal. Dilakukan eksisi luas dan rekonstruksi paramedian forehead flap, dikombinasi dengan hinge flap dan cartilage graft. Hasil pemeriksaan patologi anatomi menunjukkan basalioma berpigmen, dengan ulserasi fokal keratotik, batas irisan bebas. Manajemen yang tepat dapat meningkatkan kualitas hidup baik secara estetik maupun mencegah kekambuhan. Basalioma is the most common non-melanoma skin cancer. Basalioma is generally a slow growing tumor, locally destructive and rarely metastasizes. This tumor developed mostly in areas exposed to UV, nose is the most common location. This paper reports a 48-year-old male with basalioma in the nasal area. Extensive excision was carried out followed by reconstruction with paramedian forehead flap combined with hinge flaps and auricular cartilage graft. Pathological examination shows pigmented basalioma with focal keratotic ulceration, edge of incision was free from tumor. Appropriate management is needed to improve aesthetically and to prevent recurrence.

Abstract

Summary: An increasing amount of evidence from clinical and experimental studies suggests that biopsies, thought to assist with definitive diagnoses, may aggravate tumor progression. We present a case of fast-growing giant verrucous squamous cell carcinoma, observed two weeks following biopsy. A 38-year-old man presented with a nose tumor that recurred two weeks after a biopsy and debridement to remove a lesion that was thought to be an abscess. Wide excision was performed on the tumor, frozen sectioned; the defect was reconstructed with a two-step forehead flap. The rapid tumor growth observed in this case raises the question of whether biopsies or other interventions may accelerate the growth of malignant tumors. Verrucous squamous cell carcinoma is slow-growing with a predilection for oral and nasal mucosa and rarely metastasizes. The patient presented here is an unusual case, with a tumor rapidly growing within fifteen days. A two-staged paramedian forehead flap is the workhorse of nasal reconstruction. Even though the benefit of doing biopsies overall outweighs the potential side effects, further research is required to determine whether biopsies affect the progression of the tumor. Early differential diagnosis is also crucial, as it may affect the patient's plan of treatment and prognosis. Giant tumors on the facial area may cause a functional and cosmetic disturbance. A two-step forehead flap produces a satisfactory cosmetic appearance for the patient one-year post-procedure.

Abstract

Highlights: The majority of SCC cases were observed in males (86%), emphasizing a higher susceptibility of males to this form of cancer compared to females (14%). The study identified an increased risk of SCC particularly among individuals in the 46-55 and 56-65 age groups. The study revealed that a significant proportion of Morbus Hansen patients with SCC had a normal weight, while the occurrence of overweight and obesity was relatively low. Maintaining a healthy body weight may play a role in reducing the risk of SCC in this patient population. Abstract: Introduction: Leprosy, a chronic systemic infection caused by Mycobacterium Leprae, has been associated with the development of cutaneous neoplasms, such as squamous cell carcinoma. The aim of this study was to examine the characteristics of patients diagnosed with squamous cell carcinoma arising from Morbus Hansen (leprosy). Methods: A retrospective analysis of medical records from Dr. Sitanala Central Hospital was conducted, focusing on patients diagnosed with squamous cell carcinoma arising from Morbus Hansen over a 10-year period from 2011 to 2021. Descriptive analysis was performed on the collected data. Result: In this study, out of the twenty-one patients, the majority were male (86%), and the most common age group affected was between 46 and 55 years, accounting for 43% of the cases. Most patients had Morbus Hansen type BL (borderline lepromatous) and had been living with the condition for an average of 23 years. Additionally, a significant number of patients had a normal body mass index (52%) within the range of 20-24.9. Anemia was the main comorbidity observed, affecting 46% of the patients. Conclusion: Examining the clinical profile of these patients helps healthcare providers identify specific characteristics associated with this condition, further research with more comprehensive data is necessary to ensure accuracy. Future studies could also explore potential connections between hypertension, anemia, and squamous cell carcinoma arising from Morbus Hansen.

Abstract

Introduction : Meningoencephalocele is one form of neural tube defect that forms a sac containing brain tissue, meninges, and cerebrospinal fluid (CSF). It can cause structural deformities and many other clinical symptoms that come after. Many medical consultations are needed for the case. The collaboration between neurosurgeon and plastic surgeon plays a significant role in managing this case.Case Report : A case of a 10-year old boy with an apple-size lump in his nasofrontal region, nasal congestion, and central visual field disturbance. The patient has not undergone an examination or surgery in a long time because he lives in a rural area. Radiological results show herniaton brain tissue, meninges, and CSF protruding through the defect in the frontal bone of the cranium that suggest meningoencephalocele as a diagnosis.Discussion : The definitive management for this case is through surgery. Selecting a surgical technique for meningoencephalocele is essential for optimizing the outcomes and minimizing the risks. The Chula technique approach was chosen for the implementation of this surgery. In this case, we describe the modified Chula technique with stages adjusted according to the patient’s condition. Plastic surgery complements many aspects of reconstruction and restores aesthetic subunits. Managing this case was a challenge until the recovery period. The surgery was successfully performed, and patient went home without any serious complications.Summary: Caution is necessary in handling surgery and involves aesthetic creativity within it. Long-term monitoring is necessary for post-operative results since many clinical aspects and social developments must be regularly evaluated.
