Abstract

Hemolytic anemia is anemia caused by the premature destruction of red blood cells. Transfusion with hemolytic anemia is a dilemmatic indication and is still based on clinical decisions. Several studies assessed the effectiveness of transfusions in hemolytic anemia patients with incompatible crossmatching showed effectiveness. This study aimed to analyze crossmatching with transfusion effectiveness in hemolytic anemia patients.  Retrospective cohort observational study with medical record data from January 2019 to May 2022 of hemolytic anemia patients who got transfusion therapy. The variables included age, gender, hemoglobin levels before and after transfusion, crossmatching test, and transfusion effectiveness based on increased hemoglobin after transfusion 1 g/dL of 1 Pack Red Cell (PRC) unit. One hundred three (103) patients with hemolytic anemia 45 received PRC, 12 males (26.7%) and 33 females (73.3%). Ages 19 - < 45 years subjects had the most transfusions. Hemoglobin levels before and after transfusion showed significant differences (p-value =0.000) in the patients with a diagnosis of hereditary, unspecified, and autoimmune hemolytic anemias. The correlation between the effectiveness of transfusions with gender and age was not found (p=0.521, p=0.240). The correlation between crossmatching with the effectiveness of transfusion was not found (p=0.089). Hemolytic anemia patients with transfusion showed an increase in hemoglobin levels between before and after transfusion even with incompatible crossmatching. The correlation between the results of crossmatching with transfusion effectiveness in patients with hemolytic anemia was not found. Good premedication treatment can provide effective transfusion.

Abstract

Ischemic stroke is a condition triggered by cerebral perfusion disorders with the risk factor of blood clotting disorders that can cause neurological deficits through a complex pathophysiological process of thrombus formation. The role of platelet aggregation as a blood clotting parameter has the potential to be used as a marker for the level of neurological deficit in patients with ischemic stroke in conjunction with the National Institutes of Health Stroke Scale (NIHSS). The study aimed to observe the correlation between platelet aggregation and the degree of neurological deficit calculated using NIHSS. The study utilized a cross-sectional design with 51 research subjects diagnosed with ischemic stroke based on CT Scan results, followed by platelet aggregation testing. Patients with a diagnosis of hemorrhagic stroke and cardiogenic ischemic stroke were excluded from this study. This study found variations in platelet aggregation among groups with different degrees of stroke severity. However, it was not statistically significant (p=0.427). Correlation analysis showed a mild positive correlation between platelet aggregation and the degree of neurological deficit (r=0.157), although it was not statistically significant (p=0.270). There are variations in platelet aggregation in assessing neurological deficits using NIHSS. There is a positive correlation between the percentage of platelet aggregation and the level of neurological deficit in patients with ischemic stroke.

Abstract

BACKGROUND: Permenkes has health efforts to ensure that school-aged children and adolescents receive health education through school and outside of school, so that every child can live a clean and healthy life, have healthy life skills, and good social skills so they can learn, grow and develop harmoniously and optimally to become qualified human resources. One of the school health efforts that can be done is screening for anemia, blood type, and urinary tract infection. METHODS: This research is a descriptive observational study using a sample of 4th to 6th-grade students. RESULTS: Of the 93 subjects, 37 (40%) were male and 56 (60%) were female. The mean age of the patients was 10.2 years with a standard deviation of 1.057 years and an age range of 9-12 years. A total of 36 (38%) children had blood type O, 26 (28%) had blood type B, 22 (24%) had blood type A, and 9 (10%) had blood type AB. Subjects who experienced anemia were 12 (13%) and those who experienced UTI were 9 (10%). CONCLUSION: The blood type of students at SDN 12 Biraeng is dominated by blood type O and it was found that 13% of students had anemia and 10% of students had urinary tract infections.
