Abstract

Kepuasan pasien indikator penting dalam pelayanan kesehatan yang merupakan penilaian pasien terhadap pelayanan kesehatan dengan membandingkan harapan dan kenyataan pelayanan kesehatan yang diterima. Pengukuran kepuasan pasien dilakukan menggunakan Importance Performance Analysis (IPA) dan KANO. Intergrasi kedua metode memungkinkan dalam pengukuran kepuasan pasien. Tujuan penelitian mengetahui penerapan metode integrasi IPA dan KANO.Jenis penelitian deskriptif di Puskesmas Cisaat Kabupaten Sukabumi Februari sampai Desember 2018. Populasi seluruh pasien yang berkunjung ke Puskesmas, sampel sebanyak 305 responden menggunakan aksidental sampling. Pengumpulan data menggunakan kuesioner dengan 28 item pertanyaan. Reliabilitas diatas 0,6. Analisis data menggunakan IPA, KANO, integrasi IPA dan KANO.Hasil berdasarkan IPA atribut 13, 14, 17, 21 pada kuadran I, atribut 1, 2, 3, 4, 5, 6, 15, 16, 18, 23 pada kuadran II. Metode KANO, kategori must be adalah 1, 8 sedangkan attaractive adalah3, 6, 9, 11. Integrasi IPA-KANO, kategori survival 1, 18, Major Weapon 2, 4, 5, 15, 16, 23.Kesimpulan, metode IPA didasarkan kepentingan dan kinerja. Metode KANO pada pengkategorian atribut seberapa baik produk memuaskan pasien. Metode Integrasi IPA-KANO pada kepentingan dan kinerja serta pengkategorian atribut seberapa baik produk memuaskan pasien. Metode Integrasi IPA-KANO lebih akurat dan lebih baik. Diharapkan metode integrasi IPA-KANO sering untuk mengukur kepuasan pasien.

Abstract

The family is the smallest unit of society that plays an important role in realizing the welfare of the population which is the ideal of development. The outbreak of Covid-19 has resulted in various imbalances in the line of life. Indirectly, this has an impact on the resilience of the family as the smallest social unit. If family resilience is disrupted, it can cause less than optimal family functions. In 2020, BPS Sukabumi City recorded an increase in the unemployment rate of 3.81% from 2019. The purpose of this study is to determine the relationship between religiosity and coping strategies with family resilience during the Covid-19 pandemic. This type of research is correlational with a cross sectional approach. A sample of 372 respondents used cluster random sampling. The instrument used is a questionnaire. Data analysis used simple linear regression and multiple linear regression. The results showed a relationship between communication patterns and family resilience (0.000), a relationship between religiosity and family resilience (0.000), a relationship between coping strategies and family resilience (0.000) and a simultaneous relationship between religiosity and coping strategies with family resilience (0.000). The conclusion in this study is that there is a relationship both partial and simultaneous related to religiosity and coping strategies with family resilience

Abstract

Diabetes melitus merupakan penyakit kronis yang memerlukan intervensi berkelanjutan untuk menjaga kestabilan kadar gula darah dalam tubuh, salah satu intervensi tersebut ialah dengan melakukan diet, sehingga kepatuhan pasien dalam melakukan diet sangat diperlukan, diantara faktor yang dapat mempengaruhi kepatuhan diet ialah dukungan keluarga dan motivasi pasien. Tujuan penelitian ini untuk mengetahui korelasi dukungan keluarga dan motivasi pasien dengan kepatuhan diet pasien diabetes melitus tipe 2 di Desa Nagrak Utara Wilayah Kerja Puskesmas Nagrak Kabupaten Sukabumi. Jenis penelitian menggunakan metode korelasional dengan pendekatan cross-sectional, menggunakan sampel sebanyak 92 responden dengan menggunakan teknik Cluster sampling yang kemudian menggunakan analisa data Chi Square. Hasil penelitian menunjukan bahwa sebagian besar (72,8%) responden memiliki keluarga yang mendukung dan sebagian besar (67,4%) responden memiliki motivasi tinggi dan sebagian besar (57,6%) responden patuh terhadap diet diabetes, dengan hasil uji statistik menunjukan terdapat hubungan dukungan keluarga kepatuhan diet pasien diabetes melitus (P-value = 0,010), terdapat hubungan motivasi dengan kepatuhan pasien diabetes (P-value = 0,000). Dapat disimpulkan bahwa terdapat hubungan dukungan keluarga dan motivasi pasien dengan kepatuhan diet pasien diabetes melitus tipe 2 di wilayah kerja Puskesmas Nagrak Kabupaten Sukabumi.

Abstract

Background: Intradialytic mean arterial pressure (MAP) status is one of the most common cardiovascular indicators among patients with chronic kidney disease undergoing maintenance hemodialysis. Poor adherence to fluid restriction and excessive interdialytic weight gain (IDWG) are considered major contributors to hemodynamic alterations during dialysis sessions. Purpose: To examine the factors associated with intradialytic mean arterial pressure (MAP) among patients receiving maintenance hemodialysis. Method: An analytical cross-sectional study was conducted among patients with chronic kidney disease undergoing maintenance hemodialysis at a hospital in Indonesia. A total of 136 participants were recruited through a total sampling approach. Adherence to fluid restriction was measured using the Fluid Control in Hemodialysis Patients Scale (FCHPS), a 28-item instrument rated on a 5-point Likert scale, which demonstrated high internal consistency (Cronbach’s α = 0.88). Interdialytic weight gain (IDWG) was calculated as the percentage increase in body weight relative to the preceding post-dialysis dry weight. Intradialytic blood pressure was assessed using the mean arterial pressure (MAP) recorded two hours after initiation of dialysis. Data analysis was performed using multiple linear regression to examine associations between adherence and clinical outcomes. Results: Descriptive analysis showed a mean IDWG of 3.67% of dry body weight, a mean fluid restriction adherence score of 42.70, and a mean intradialytic MAP of 102.44 mmHg. Multiple linear regression demonstrated that interdialytic weight gain was positively associated with intradialytic MAP (β = 0.282, p = 0.001), whereas adherence to fluid restriction was negatively associated (β = −0.245, p = 0.003). Together, these variables explained 15.1% of the variance in intradialytic MAP (F = 13.000, p < 0.001). Conclusion: Adherence to fluid restriction and interdialytic weight gain are important variables associated with mean arterial pressure stability in patients undergoing maintenance hemodialysis. Continuous nursing education and behavioral interventions aimed at improving fluid management are essential to reduce the risk of acute cardiovascular complications in this patient population.

Abstract

Tuberculosis (TB) patients living in disaster-prone areas may experience interconnected food, nutritional, socioeconomic, and healthcare vulnerabilities that can be exacerbated by disasters. However, an instrument that comprehensively assesses food and nutrition vulnerability in this population remains limited. This study aimed to develop and validate the multidimensional Food Security-Nutrition Vulnerability (FOSENUVU) instrument for assessing food and nutrition vulnerability among TB patients living in disaster-prone areas. A methodological study was conducted through sequential instrument development and psychometric validation. Initial item development involved a literature review, healthcare worker interviews, item construction, and expert assessment. Content validity was evaluated by five experts using the Content Validity Index. The resulting 30-item instrument was administered to 300 adult pulmonary TB patients living in disaster-prone areas of Sukabumi Regency, Indonesia, selected using purposive sampling. Validity analysis was carried out in stages: content validity was assessed by five experts using the Content Validity Index (CVI), followed by item validity testing using corrected item-total correlation. Furthermore, construct validity was analysed using Exploratory Factor Analysis (EFA) and Confirmatory Factor Analysis (CFA), whilst convergent validity was assessed using Average Variance Extracted (AVE) and Composite Reliability (CR). Expert assessment demonstrated 90% agreement, supporting the content validity of the instrument. The 30-item FOSENUVU showed strong internal consistency, with Cronbach’s alpha of 0.979 overall and 0.872–0.894 across six conceptual domains. EFA identified a dominant single-factor structure explaining 60.5% of the total variance, with factor loadings ranging from 0.709 to 0.825. CFA confirmed the one-factor model with excellent fit (χ²/df=1.046, CFI=0.997, TLI=0.997, RMSEA=0.012, and SRMR=0.022). Convergent validity was supported by an AVE of 0.605 and composite reliability of 0.979. All 30 items were retained. FOSENUVU demonstrated strong content validity, internal consistency, structural validity, and convergent validity. The final 30-item instrument provides a multidimensional yet integrated measure of food and nutrition vulnerability among TB patients in disaster-prone areas.
