Abstract

ABSTRACTIntroduction: The biggest problem in oral health is dental caries and periodontal disease. The way to prevent dental and mouth problems is to keep oral hygiene. One of them is adequate plaque control. Plaque control can be done by brushing teeth and using toothpaste. Currently, many herbal ingredients have been used as antibacterials such as gambir (Uncaria gambir). Gambir mainly contains catechins and tannins, which have the ability to inhibit the growth of bacteria. The purpose of this study was to determine the antimicrobial potential of toothpaste containing gambir (Uncaria gambir). Methods: The total sample was 30, and it divided into three groups. Samples were toothpaste containing ten percent of Uncaria gambir extract, fluoride, placebo toothpaste, Streptococcus mutans (Gram-positive), Phorphyromonas gingivalis (Gram-negative), and Candida albicans. Antimicrobial activity of toothpaste with gambir (Uncaria gambir) extract against microorganisms was observed with the formation of inhibitory zone diameter in agar. Data were analyzed by using SPSS with one-way ANOVA and Tukey test. Results: The results showed that toothpaste containing gambir extract could inhibit the Streptococcus mutans (p<0.05), Phorphyromonas gingivalis (p<0.05), Candida albicans (p<0.05) compared significantly with placebo (p<0.05). Antimicrobial toothpaste containing gambir extract had no different effect from toothpaste containing fluoride. However, toothpaste containing gambir extract had significantly different effect than toothpaste containing fluoride. Conclusion: It can be concluded that toothpaste containing gambir (Uncaria gambir) extract has antimicrobial potential against Streptococcus mutans, Phorphyromonas gingivalis, and Candida albicans. These findings showed that the extract of Uncaria gambir possesses potent antimicrobial activity.

Abstract

Candy is a type of food that contains sugar which is high in calories. Candy consumption can lower the pH of saliva, which can trigger dental caries. Gambir (Uncaria gambir) is a type of plant that contains active compounds catechins and tannins, which can inhibit the colonisation of acid-causing bacteria in the oral cavity. This study aimed to determine the effect of gambir can-dy consumption on salivary pH; an experimental study with a pretest-posttest approach. The research subjects were 25 Sriwi-jaya University students, whose saliva samples were taken as much as 5 mL before and after consumption of candies containing gambier extract; using the spitting method and measured with a digital pH meter. Paired t-test was conducted for data analysis which showed that there was a significant increase in mean salivary pH before and after consuming gambir candy (p<0.05). It was concluded that consumption of candies containing gambir extract can maintain and increase salivary pH.

Abstract

Lesi ulserasi merupakan penyakit jaringan lunak mulut yang banyak dijumpai. Pendekatan perawatan terhadap lesi ulserasi dengan menggunakan bioadhesif bertujuan untuk mendapatkan efek terapi maksimal dari suatu zat aktif. Tanaman gambir (Uncaria gambir) adalah suatu jenis tanaman yang banyak dikembangkan sebagai obat herbal karena memiliki senyawa aktif seperti katekin. Tujuan dari penelitian ini adalah untuk mengevaluasi efek mukobioadhesif ekstrak gambir terhadap lesi ulserasi rongga mulut. Penelitianini menggunakan metode pretest-posttestcontrol group. 24 sampel dibagi menjadi dua kelompok, kelompok yang diberi mukobioadhesif ekstrak gambir dan kelompok yang diberi mukobioadhesif plasebo selama 7 hari dengan aturan pakai 3 kali sehari. Tingkat keparahan rasa sakit yang diukur dengan skala VAS, luas lesi ulserasi, dan durasi penyembuhan dievaluasi dan dianalisis dengan SPSS ver. 22. Hasil penelitian menunjukkan bahwa penggunaan mukobiadhesif ekstrak gambir mampu mengurangi rasa sakit dan luas lesi, serta mampu menurunkan durasi penyembuhan secara signifikan dibandingkan dengan plasebo. Dapat disimpulkan bahwa penatalaksanaan mukobioadhesif ekstrak gambir berpotensi dalam mengurangi rasa sakit dan mempercepat penyembuhan lesi ulserasi rongga mulut.

Abstract

Poor oral hygiene might lead to Candida albicans infection. Since long term used of synthetics antifungal therapy could lead to fungal resistance, an alternative of antifungal therapy was needed. It has been reported that Gambier (Uncaria gambir ROXB) contains catechins and tannins which have antifungal effects invitro, but research using Gambier extract as a toothpaste for daily oral hygiene maintenance has not established. This study was done to determine the antifungal effect and the synergy of the combination of 10% gambier and fluorides in toothpaste preparation against Candida albicans. A true experimental laboratory study with a post-test-only design group was established. Three groups namely 10% gambier extract toothpaste, the combination of 10% gambier extract and 0.0048% fluorides toothpaste, lastly the 0.0048% fluoride toothpaste were tested. Gambier extract was obtained using the soxhletation method and the separation of the solvent from the extract was carried out by the distillation method. Agar Well method antifungal test was used to measure the inhibition zone value of tested groups. Data were analyzed using one-way Anova. Inhibition test of tested groups showed that only combination of gambier and fluorides toothpaste was effective in inhibiting Candida albicans with an average inhibition zone value of 8.35 mm. Toothpaste preparation of 10% Gambier extract and 0.0048% fluorides combination produce a synergistic therapeutic effect in antifungal therapy.
