Abstract

BACKGROUNDThe application of Standard Precautions (SP) has become a significant challenge for healthcare workers, especially in developing countries, endangering their safety and increasing their exposure to blood-related pathogens. This study was aimed at exploring the factors related to nurses’ compliance with the practice of SP.METHODSA cross-sectional study was conducted on 120 nurses working at a hospital in Palembang. Health Belief Model components of the subjects were recorded through questionnaires. A 12-point observation form assessed the nurses’ SP compliance. Multiple logistic regression models were used to explore factors associated with nurses’ compliance.RESULTSThe results showed that 56.7% of participants had good compliance, although the five moments of hand hygiene still needed to be improved. The study revealed that the proportion of the operating room and emergency room nurses who complied with the SP was larger than the proportion of those who worked at the other wards (OR=2.57, 95% CI 1.51-4.36). The nurses who had received training also showed a larger proportion of compliance with SP than those who had not been trained (OR=2.70, 95% CI 1.07-6.79).CONCLUSIONNurses’ behavior to SP was significantly associated with the adequacy of the training and work unit. It is suggested that the practice of SP was also influenced by enabling factors and subjective norms. Adequate training of nurses, provision of infection prevention equipment, and assessment of occupational exposures need to be introduced.

Abstract

The hospital as a health service institution for the community is a place of work that has a high risk of safety and health. The fulfillment of the right to Occupational Safety and Health for health workers is very necessary, because health workers are of course vulnerable to occupational safety and health hazards. Charitas Hospital as one of the COVID-19 referral hospitals in Palembang City has a very high potential danger of COVID-19 transmissionThis study aims to find out how the regulation of Charitas Hospital can guarantee the fulfillment of Occupational Health and Safety rights. The approach method used in this research is the sociological juridical method.Based on the Legislation which discusses Human Rights, Employment, and Health there are no regulations that overlap, redundant, or there is a void in the legislation governing the rights of Occupational Safety and Health of nurses in Isolation Rooms during a pandemic. COVID-19. In the event of a vacancy in laws and regulations due to the COVID-19 pandemic, the Government may issue policy regulations (beleidsregel) such as Presidential Instructions, Circulars, Guidelines, Technical Standards, and Protocols and Charitas Hospital Palembang to make internal policies (SOPs, Instructions for Technical, Technical Standards) and adding Nursing Staff Bylaws related to handling arrangements during a pandemic so that the Hospital is ready if at any time a similar incident occurs.

Abstract

Enactment of JKN in hospital changes the payment system from retrospective payment (fee for service) into prospective payment (INA-CBG's). As a healthcare facility, RK Charitas Hospital has a role to provide not only quality but also to consider cost effective of services. This study aimed to analyze costs and identify the determinants of the inefficiency of hemodialysis services in patients with chronic renal failure at RK Charitas Hospital. This is a descriptive analysis research using primary and secondary data. Approach of cost analysis is Activity Based Costing (ABC) with "Bottom Up" method. ABC method is used to allocate costs by identifying cost drivers of hemodialysis services. Operational cost is the biggest expense in the hemodialysis services. Analysis of the inefficiency factors uses the calculation of Value Stream Mapping (VSM). The composition of value added (VA) compared to non-value added (NVA) is 17.73%: 82.27%. Lean implementation on hemodialysis services could eliminate waste.

Abstract

Background: Hospital employees face higher risks of work-related diseases and injuries compared to other professions. Ergonomic hazards such as repetitive movements, awkward postures, forceful exertion, prolonged static positions, vibration, extreme temperatures, and work stress contribute significantly to musculoskeletal disorders (MSDs), including cervical spasm, low back pain, scoliosis, and carpal tunnel syndrome. At Charitas Hospital Palembang, 2022 medical check-up data reported cervical spasm (0.3%), low back pain (1.6%), scoliosis (0.5%), and carpal tunnel syndrome (0.2%). To promote awareness of ergonomic hazards and preventive strategies among hospital staff and encourage integration of ergonomics into occupational health systems. A hospital-based seminar titled “Implementation of Occupational Safety and Ergonomics in Hospitals” was conducted on April 29, 2023, involving approximately 120 clinical and non-clinical staff. The program consisted of lectures, discussions, and practical examples. Participant feedback was collected through structured evaluations. Over 85% of participants found the material relevant and useful, while 90% expressed satisfaction with the delivery. The seminar demonstrated that even low-cost, short-duration interventions can effectively raise awareness, strengthen safety culture, and improve staff productivity.

Abstract

Fitness-for-work assessment is one of the core responsibilities in occupational medicine, particularly when dealing with workers experiencing health problems. This process requires not only accurate clinical judgment but also involves complex bioethical dimensions. This article aims to analyze the bioethical challenges faced by occupational physicians in determining fitness for work, with emphasis on the principles of autonomy, beneficence, non-maleficence, justice, and conflicts of interest. This study employs a qualitative approach through a systematic literature review and case reflections, complemented by semi-structured interviews with fifteen occupational health practitioners and human resource managers from the logistics, mining, and manufacturing sectors in Indonesia, conducted between January and May 2025. Data were analyzed thematically using a clinical bioethics framework. The findings reveal that medical decisions in this context often conflict with socio-economic pressures, productivity demands, and the limitations of Indonesia’s labor system. Power imbalances between employers and workers, low health literacy, and weak legal protection for medical professionals further exacerbate ethical dilemmas. Therefore, a multidisciplinary and systemic approach is needed—one that integrates ethical values into labor policy, occupational health training, and organizational governance. Fitness-for-work assessments should not merely serve as administrative procedures but must uphold justice, protect workers’ dignity, and reinforce the integrity of the medical profession.
